In 2001, the U.S. Department of Health and Human Services made it their priority to work toward eliminating mental health disparities, reducing barriers, and improving access to mental health treatment. Unfortunately in 2012, Sue, Cheng, Saad, and Chu reported little progress in eliminating disparities in access to mental health services for the Asian American population. Their report concluded that the role of acculturation-related factors related to help-seeking attitudes requires further research. The current study used a meta-analytic methodology to investigate acculturation and enculturation-related factors that may be related with help-seeking attitudes. This meta-analytic approach is of particular significance, as the overall magnitude of the relationship between acculturation help-seeking attitudes has been inconsistent in existing research. This study examined the relationship of the predictor variables: acculturation and enculturation, with the outcome variable, help-seeking attitudes. Results indicated a minimal relationship between acculturation and help-seeking attitudes. Ethnic subgroup differences were identified which may account for the heterogeneity of the relationship between acculturation and help-seeking attitudes in prior research. Results also indicated a significant negative relationship between enculturation and help-seeking attitudes. The enculturation-related variables of stigma toward mental health, loss of face, and filial piety were negatively correlated with help-seeking attitudes. Enculturation did not moderate the relationship between acculturation and help-seeking attitudes suggesting a bilinear process of acculturation. Additionally, generational status was a significant predictor of help-seeking attitudes. These findings may prove to be important targets for outreach intervention efforts aimed at increasing psychological help-seeking attitudes among Asian Americans. Implications, limitations, and future directions for research are discussed. Keywords : Asian American, acculturation, enculturation, help-seeking attitudes.

The Acculturation Rating Scale for Mexican Americans, Modified for Asian Americans (ARSMA), consists of four subfactors: Reading/Writing, Language, Social Interaction and Ethnic Identity. The attitude measure, the Attitude Toward Seeking Professional, Psychological Help Scale, consists of four factors: Stigma Tolerance, Confidence in Mental Health Practitioners, Recognition of Need and Openness. These two self report measures were administered to participants who identified themselves as Asian-American in the San Francisco Bay Area (N=104). The author hypothesized that there is a positive relationship between acculturation and positive help seeking attitude. An exploratory hypothesis was made that there will be an interaction between factors of the acculturation measure, ARSMA, and factors of the attitude measure, ATSPPHS. The results indicated that there is a negative relationship between the acculturation factor Reading/Writing and the attitude factor Confidence in Mental Health Practitioners. These findings suggest that, as Asian-Americans ability to read and write in English decreases, their confidence in mental health practitioners increases. As for the exploratory hypothesis, a hierarchical regression showed a significant interaction between ARSMA factors Language and Social Interaction and Reading/Writing and Social Interaction on the ATSPPHS Factor Confidence in Mental Health Practitioners. These findings suggest that participants who scored lower on language have lower confidence in mental health professional as their level of social interactions with individuals outside of their ethnic group increased. Also, participants who scored lower on Reading/Writing had lower confidence in mental health professionals as their level of social interaction with individuals outside of their ethnic group increased.

Book excerpt: Purpose: This study investigated the relationship between multiethnic identities and help-seeking attitudes among Asian Americans. Hypothesis: Asian Americans with a strong sense of American identity will show a more favorable help seeking attitude and those with a strong sense of ethnic identity will show a less favorable help seeking attitude. Women have a greater help seeking attitude compared to men. Methods: Participants (N=101) completed an online survey about ethnic identity, American identity, and help seeking attitudes. Results: Ethnic identity and American identity did not show a significant relationship. Ethnic identity and American identity did not yield a significant relationship with help-seeking attitudes. Lastly, gender did not produce a significant association with help-seeking attitude. Discussion: American identity and ethnic identity moved in the positive direction, so the theory of biculturalism was examined. The positive direction between ethnic identity and help-seeking attitudes supports prior research that ethnic identity might be a protective factor that assists in seeking help. On the other hand, American identity was inverse to help-seeking attitudes, which is contrary to numerous studies about the Westernized perspective of help-seeking attitudes. Possible implications to social work is the consideration of incorporating ethnic identity discovery during therapy sessions and celebrating cultural diversity within our society.

Asian Indians are the third largest Asian subgroup in the United States; thus, it is important to understand their culture and lifestyle. Collectively, they are more likely to consult family and close friends rather than seek help from mental health professionals. Asian Indians may not choose to discuss these problems because they feel it is unnecessary, or it could bring shame to the family. The purpose of this study was to look at the effects of acculturation on the help-seeking attitudes of Asian Indian parents and caregivers when considering seeking help for their children. Inclusion criteria included parents and caregivers of children ages 7-17, who are 18 years of age or older, of Asian Indian ancestry, have resided in the U.S. for at least one year, and who have been the primary caregiver for at least 6 months. Measures used examined culture, acculturation, help-seeking attitudes, and some basic demographic information. The survey was available online as well as on paper to be returned to the researcher. A total of 89 participants, the majority of whom were mothers and well-educated, completed the survey. Participants were recruited at community events, at a temple in Houston, and through family members and friends by emailing the online link. Analyses of the data indicated that acculturation does not impact openness to seek mental health services, level of mental health stigma, and intentions to seek mental health services. It was also shown that openness and stigma do not mediate the relationship between acculturation and the intention to seek mental health services. Finally, the top sources of help indicated by the Asian Indians in this study are intimate partners, mental health professionals, and doctors or general practitioners. There were participants who had taken their children to see a professional before, but opinions varied regarding its helpfulness. It is hoped that this study will provide valuable information to inform mental health professionals about an understudied population and to continue to emphasize the importance of understanding diversity and what that means for school psychology and the mental health field.

Book excerpt: Research has consistently identified low rates of counseling use among Asian international students. Empirical findings suggest that Asian foreign students are less likely to seek counseling than their American peers (Huyn, Quinn, Madon, & Lustig, 2007; Nilsson, Berkel, Flores, & Lucas, 2004; Yakushko, Davidson, & Sanford-Martens, 2008). Research also indicates that when Asian international students do seek counseling, they typically attend few therapy sessions (Nilsson et al., 2004; Yakushko et al., 2008). Several calls have been made to examine different cognitive and cultural predictors of counseling use in this population (Heggins & Jackson, 2003; Ibrahim & Ingram, 2007; Mori, 2000; Sheu & Fukuyama, 2007). Possible cognitive predictors include help-seeking intentions, counseling attitudes, and stigma concerns. Potential cultural predictors include acculturation, Asian values, and loss of face concerns. The purpose of this study was to integrate the above predictors using the Theory of Reasoned Action (TRA, Ajzen & Fishbein, 1980; Fishbein & Ajzen, 2009). In order to test the applicability of the TRA, data were collected from 295 Asian international students. All participants completed an online survey including a demographic questionnaire and measures of help-seeking intentions, counseling attitudes, stigma concerns, acculturation, Asian values, and loss of face concerns. Subsequently, three TRA models were tested: a Non-Mediated, a Partially Mediated, and a Fully Mediated Model (Holmbeck, 1997). Additional analyses included bivariate correlations and bootstrapping analysis of indirect effects (Shrout & Bolger, 2002). The results indicated partial support for the TRA as an integrated model of Asian international students’ help-seeking. Path analysis indicated that the Partially Mediated Model offered the best data fit. In addition, participants’ endorsement of Asian values was associated with more negative counseling attitudes, which then resulted into lower help-seeking intentions. Contrary to the hypotheses, however, acculturation did not predict any of the help-seeking constructs of interest. In addition, participants’ stigma and loss of face concerns were positively associated with their help-seeking intentions, which is inconsistent with prior theory and research. Overall, the findings of the present study indicate that Asian international students’ help-seeking intentions may be more complex than what is represented by the TRA.
