Available in PDF, EPUB and Kindle. Book excerpt: The first edition of this book appeared in 1982. In the preface to that first edi tion, I wrote 'This book is based on the lecture course in renal physiology which I give to medical students at the University of Birmingham. The pur pose of the book is primarily to set out the principles of renal physiology for preclinical medical students, and it is therefore concerned mainly with normal renal function. However, diseases or abnormalities in other body systems may lead to adaptations or modifications of renal function, so that a good knowl edge of renal physiology is essential to the understanding of many disease states, for example the oedema of heart failure or liver disease, or the conse quences of haemorrhage and shock. ' The new edition is still based on the lec tures which I continue to give at Birmingham University, but over the years the course has gradually changed, to being a system based course covering all aspects of the kidney - the anatomy, physiology, pharmacology and pathology. The new edition of the book, which has been extensively revised and rewritten, reflects this. However, it continues to offer a concise, easily readable format, primarily intended for undergraduate medical and medical science students.

Book excerpt: To promote effectiveness and minimize possible toxicity, the dosage of certain medications must be adjusted in persons with compromised kidney function. Failure to enjoin appropriate dosage adjustments in patients with abnormal or rapidly changing kidney function continues to lead to reports of drug toxicity involving a broad array of renally eliminated medications. This updated edition captures nearly 200 new drugs that have been approved by the FDA since the initial publication of Renal Pharmacotherapy. It also covers new evidence that has emerged regarding the need to adjust dosage of certain older medications that are eliminated by the kidneys. Additionally, it presents new data that are being continuously derived in the areas of patient-specific dose individualization for drugs of all types. Comprehensive, convenient, and evidence-based, this reference closes several identified knowledge gaps and will continue to be the leading collection of dosage recommendations for patients with compromised kidney function.

Similarities as well as the unique differences between the kidney and other organ systems are emphasized, and fundamental content is considered in an applied setting. For example, why and how does glomerular filtration change in response to hemorrhage or dehydration? Each chapter includes the presentation and resolution of a short clinical case.

Available in PDF, EPUB and Kindle. Book excerpt: The kidney is innervated with efferent sympathetic nerve fibers reaching the renal vasculature, the tubules, the juxtaglomerular granular cells, and the renal pelvic wall. The renal sensory nerves are mainly found in the renal pelvic wall. Increases in efferent renal sympathetic nerve activity reduce renal blood flow and urinary sodium excretion by activation of α1-adrenoceptors and increase renin secretion rate by activation of β1-adrenoceptors. In response to normal physiological stimulation, changes in efferent renal sympathetic nerve activity contribute importantly to homeostatic regulation of sodium and water balance. The renal mechanosensory nerves are activated by stretch of the renal pelvic tissue produced by increases in renal pelvic tissue of a magnitude that may occur during increased urine flow rate. Under normal conditions, the renal mechanosensory nerves activated by stretch of the sensory nerves elicits an inhibitory renorenal reflex response consisting of decreases in efferent renal sympathetic nerve activity leading to natriuresis. Increasing efferent sympathetic nerve activity increases afferent renal nerve activity which, in turn, decreases efferent renal sympathetic nerve activity by activation of the renorenal reflexes. Thus, activation of the afferent renal nerves buffers changes in efferent renal sympathetic nerve activity in the overall goal of maintaining sodium balance. In pathological conditions of sodium retention, impairment of the inhibitory renorenal reflexes contributes to an inappropriately increased efferent renal sympathetic nerve activity in the presence of sodium retention. In states of renal disease or injury, there is a shift from inhibitory to excitatory reflexes originating in the kidney. Studies in essential hypertensive patients have shown that renal denervation results in long-term reduction in arterial pressure, suggesting an important role for the efferent and afferent renal nerves in hypertension.
