Book excerpt: Background: Much of the nutrition research surrounding bone health in athletes has been limited to the influence of eating disorders, calorie restriction, or poor energy balance in conditions such as the Female Athlete Triad or Relative Energy Deficiency in Sport (RED-S). From other population research, it is purported that dietary factors such as calories, protein, calcium, and vitamin D are critically important to bone health. However, less research has explored the role of these factors in the bone health of adult pre-menopausal athletes. Objective: The purpose of this study was to analyze the relationship of select nutritional variables with BMD at various skeletal sites. Methods: This study recruited a total of 101 adult pre-menopausal female recreational runners. Baseline laboratory visits included anthropometry, dual-x-ray absorptiometry (DXA), a three-day food record, and phlebotomy. Thirty-nine subjects participated in the follow up laboratory visit which included the same procedures except for phlebotomy. The 3-day food record was entered into ESHA Food Processor (SQL version 10.2) to determine average daily intake of calories, calcium, and protein while 25-hydroxyvitamin D levels were obtained from blood. DXA scan sites included non-dominant hip, femoral neck, lumbar spine, distal forearm, and total body. Statistical analyses employed SPSS (version 28) to determine if significant correlations exist between nutritional variables of interest with BMD. Additionally, change variables from calories and dietary protein intake were compared to BMDs from baseline to follow-up. Results: The select nutritional variables were not significantly correlated with BMD. However, calories did show a trend for a small positive correlation with TB (r=0.188;p=0.060). Regression models for prediction of total body (TB) BMD demonstrated lean body mass (LBM), calories, and protein as a reasonable prediction model. Conclusions: Despite poor correlations between nutritional variables and BMD sites, prediction models indicated the importance of LBM, calories, and protein as potential nutritional variables to help target BMD outcomes.

BMD was measured using dual-energy X-ray absorptiometry (DXA). Data on dietary intake was collected from Block 2005 Food Frequency Questionnaires (FFQs). Peak bone development is thought to be achieved in the young adult years of an individual's life and can be indicative of risk of osteoporosis later in life. This study aims to explore the relationship between bone mineral density (BMD) and body composition measurements from a college aged non-student athlete population (n = 61) to that of a student-athlete population (n = 39). Furthermore, sex specific differences between and within the subgroups were evaluated. No statistical difference was found for BMD, weight, lean mass, and bone mineral content between subgroups. However, sex-specific differences were present for all variables of interest. Pearson's correlation coefficient results indicate that there was a moderate to strong relationship between BMD and lean mass for all groups.

Book excerpt: The Female Athlete Triad (Triad), a syndrome of low energy availability, menstrual dysfunction, and low bone mineral density (BMD), affects up to 60% of exercising girls and women. As such, understanding the physiology underlying the Triad is imperative in order to identify therapeutic targets and develop effective treatment strategies to prevent long-term health detriments in afflicted women. Bone health is a major clinical outcome of concern in female athletes and exercising women with the Triad, as bone loss or suboptimal peak bone mass development may predispose women to osteoporosis and related fractures later in life. To garner a better understanding of factors contributing to bone health and to inform guidelines for treatment targets in female athletes and exercising women with the Triad, this purpose of this dissertation was 1) to assess the unique and combined effects of energy status and estrogen status on volumetric BMD (vBMD), bone geometry, and estimated bone strength in exercising women, 2) to assess the unique and combined effects of energy status and estrogen status on bone turnover in exercising women, 3) to evaluate predictors of change in bone turnover dynamics across a 12-month nutritional intervention in women with exercise-associated menstrual disturbances, and 4) to determine if bone turnover dynamics at baseline impact BMD outcomes in women with exercise-associated menstrual disturbances participating in a nutritional intervention. Key findings of the studies that made up this dissertation implicate that energy status is a main driving factor dictating vBMD, bone geometry, estimated bone strength, and bone metabolism in exercising women. The results indicate that improving energetic status will not only improve bone turnover at the cellular level, but that bone turnover favoring bone formation over bone resorption will also predict improved BMD in the following year. The findings support the guidelines put forth by the Female Athlete Triad Coalition that endorse nutritional therapy as the first line of treatment for the Triad, as low energy availability is the root cause of the clinical sequelae, including menstrual dysfunction and compromised bone health.
