Book excerpt: The number of female athletes participating in sport has risen substantially over the past 30 years. The rise in participation has translated into a parallel rise in injuries, especially those associated with overuse. Injury prevention and improved performance are paramount in the minds of sports health care professionals and strength and conditioning specialists; so finding ways to identify injury risk has gained increased attention in recent years. The purpose of this study was to examine the relationship between lower extremity injury and performance on both the modified Functional Movement Screen, (FMS) Standard FMS, and computerized Balance Error Scoring System (BESS). Data from a large prospective study was extracted for this project. Specifically, data from a cohort of female student-athletes participating in 5 high risk sports of Basketball, Field Hockey, Lacrosse, Soccer, and Volleyball was derived and analyzed using Receiver Operant Characteristic (ROC). A modified-FMS score was developed from four of the seven FMS tests completed as a part of baseline testing. These 4 tests include the Deep Squat, Hurdle Step, In-Line Lunge, and the Active Straight Leg Raise, with a total possible score of 12. BESS scores were derived from the MobileMatTM BESS from Tekscan (Tekscan, South Boston, MA). The total BESS error score across all 6 testing conditions was used in the correlational analysis. Injuries occurring as a result of participation in their respective sport were monitored using SportsWare On-Line (SWOL) injury tracking software (Computer Sports Medicine, Inc. Stoughton, MA). Lower extremity injury served as the independent variable in this project, while the scores from the FMS methods along with the total BESS error score served as the dependent variables. Overall 20 athletes sustained an injury in our cohort, with the means between the injured and non-injured groups for the three dependent variables being FMS-4 (8.1±1.1 and 8.0±1.2), FMS-7 (15.4±1.3 and 15.0±1.6), and BESS (18.6±4.5 and 17.4±5.3) respectively. There were no significant findings in our ROC analysis. The area under the curve for our three methods was FMS-4 (.510), FMS-7 (.567), and BESS (.577), indicating no useful diagnostic capability for any of the three variables for lower extremity injury. Despite some previous reports indicating injury prediction capabilities for the FMS, our finding do not support the usefulness of such a tool in our cohort of female student-athletes. Although the BESS has demonstrated some utility in the management of sport-related concussions, its ability to predict lower extremity injury is minimal.

Book excerpt: With an estimated 60 million youth participating in organized sports in the United States (Ahlquist, et al., 2020), it is imperative to examine the risks associated with participation in order to ensure that the experience is adaptive for young people. One trend that is increasingly prevalent among youth is specializing in one sport. Some evidence exists within the extant literature that suggests a positive relationship between sport specialization and injury. In order to examine this association further, a sample size of 88 collegiate athletes from a NCAA Division III institution, were surveyed to determine their youth specialization status: (i.e., light, moderate, high) and the amount of competition and practice time that they lost during their collegiate career. The data was analyzed using an ANOVA in order to explore between group differences in amount of time lost due to lower extremity injury. The results of this study evidenced no significant difference between any of the specialization groups and their time loss due to injury. Additional research is needed given how these findings depart from extant research.

Book excerpt: Context: Pre-participation exams are conducted at the beginning of the respected sports seasons to help identify any biomechanical deficiencies of athletes before the upcoming season. There have been multiple ways to suggest if an athlete has any biomechanical deficiency but not one gold standard test to determine who is at a higher risk of injury due to these deficiencies. For this study, biomechanical deficiencies were assessed using four methods: the modified functional movement screen (MFMS), the weight bearing lunge test (WBLT), the anterior reach of the star excursion balance test (SEBT-A) and body mass index (BMI). Objective: To determine if the MFMS, WBLT, SEBT-A, and BMI can identify individuals at higher risk of a non-contact lower-extremity injury within the population of a division I collegiate football. Secondary objective is to determine if differences in scores were seen between injury groups (contact or non-contact) and player position (skilled or linemen) groups. Design: Prospective cohort study. Settings: University of Toledo football athletic complex. Participants: A total of 82 division I collegiate football players (82 males) participated in this study. Methods: Participants underwent a pre-season screening which included the MFMS, SEBT-A and WBLT. A total of 3 acceptable MFMS trials were performed scored on a 0-3 scale, and then an averaged. The MFMS consists of the deep squat, hurdle step, inline lung, and active straight leg raise. For the WBLT, participants performed the test until maximum dorsiflexion range of motion distance of the ankle was met; they were progressed at 1cm increments until max reach distance was met. For the SEBT-A, 4 practice trials were performed followed by 3 successful recorded trials, test was done bilaterally. Normalized scores from each extremity were averaged together to determine maximum reach distance. Participants injured extremity reach distance side from the injured group were normalized and used for analysis. Following pre-season screening, injuries were tracked for the remainder of the season and defined as an injury to the ankle or knee resulting from participation in an organized, intercollegiate practice or competition, the injury requiring medical attention by a team-certified athletic trainer or physician, and the resulting in restriction of the student athlete’s participation or performance for one or more calendar days beyond the day of injury. Independent t-test was used to analyze all data with alpha set at 0.05. Results: 17 athletes who sustained a lower-extremity injury during practice or competition (ages 18.2+0.5, height = 185.24+4.62cm, weight=94.64+10.81kg) and 65 players who did not experience a lower-extremity injury (age 20.01+1.3 years, height=186.5+7.55cm, weight=106.35+ 19.24kg.). There were no significant differences found using the MFMS, SEBT-A, and WBLT to indicate risk of injury or differences in scores. BMI of linemen (33.79 kg/m2+3.16 kg/m2) vs. skilled player (27.69kg/m2+2.45 kg/m2) was shown to be significantly greater (p-value > 0.00). Conclusion: The data suggest it is still undetermined if the MFMS, SEBT-A, and the WBLT are valid pre-participation screening tools to determine non-contact, lower-extremity injuries in collegiate football athletes. Further research is needed to determine the usefulness of these pre-participation screens.

Book excerpt: Background: Since lower extremity injuries are the most prevalent of football injuries, it is important to examine the ability of current pre-participation examination systems in identifying athletes who are at potential risk for injury. Two current assessment tools being utilized to identify athletes at risk for lower extremity injury are: the Functional Movement Screen (FMS) and the Star Excursion Balance Test (SEBT). Purpose: The purpose of this research was to compare the ability of the FMS and SEBT to differentiate between college football players that do and do not suffer an acute lower extremity injury, as well as determine cut-off scores that allowing the creation of prediction models for lower extremity injury. Methods: One-hundred and eighty Division I college football players were screened using both the FMS and SEBT as part of their pre-participation examination. Throughout the season injuries were recorded and players were placed into 2 groups: injured and non-injured. Independent t-tests were performed to compare the means of the injured and non injured groups for the FMS and each component of the SEBT. A 1-way ANOVA was used to compare the influence of the mechanism of injury: contact injured, non-contact injured and non-injured. Receiver operator characteristic curves were formulated and cut-off scores that maximized sensitivity and specificity were obtained for each of the FMS and SEBT measures. Results: The SEBT anterior reach score was the only test that had a significant difference (P = 0.035) between injured and non-injured groups. In addition, the SEBT anterior reach score was associated with a significant difference (P = 0.015) between groups that were non-contact injured and those who were non-injured. Conclusion: The SEBT anterior reach test appears to be a better prediction tool for risk of lower extremity injury in collegiate football players.
