During the past 30 years, obesity in the United States has more than doubled among young children aged 2-5 years and adolescents aged 12-19 years, and it has more than tripled among youth aged 6-11 years. Currently, more than 9 million children 6 years of age and older are considered to be obese. The sequelae of obesity among children and youth are also rapidly increasing, including an increased risk of type 2 diabetes, hypertension, metabolic syndrome, asthma, and social and psychological consequences including low self-esteem and depression. To develop a prevention-focused action plan to reduce the number of obese children and youth in the United States, the Institute of Medicine organized three regional symposia, and held its second regional symposium in Atlanta, Georgia on October 6-7, 2005. Progress in Preventing Childhood Obesity: Focus on Communities highlights the recurring themes that emerged from the symposium for accelerating change and moving forward with obesity prevention efforts: empower communities and neighborhoods, change the environment, forge strategic partnerships, garner and mobilize political support, educate stakeholders, identify leaders and build on cultural assets, collect and disseminate local data, evaluate programs and interventions, and translate successful interventions to other communities. Approximately 90 individuals active in childhood obesity prevention efforts in the southeastern region of the United States who represented a range of stake holder perspectives and innovative practices in local communities including students, community leaders, physicians, health educators, clergy, teachers, and state and federal government officials were invited to participate in the symposium. The contents of this summary reflect specific examples presented and discussed during the symposium, and unless otherwise noted, the general perspectives of the participants. This summary, along with two other symposia summaries, and a more detailed discussion of insights and regional examples, will be incorporated in the IOM committee's final report on progress in preventing childhood obesity that will be released in the fall of 2006.

Book excerpt: Purpose: Obesity in children and adolescents is a worldwide epidemic that is anticipated to worsen over time (Budd & Volpe, 2006; Chatterjee, Blakely, & Barton, 2005; Hossain, Kawar, & El Nahas, 2007). Within the past 30 years, the prevalence of childhood obesity in the United States has more than doubled among children and quadrupled in adolescents (Ogden, Carroll, Kit, & Flegal, 2014). People often have inaccurate perceptions of their body size and weight status and this may play a key role in the prevalence of obesity especially for ethnic minorities (Gillard, Lackland, Mountfored, & Egan, 2007; Hendley, et al., 2011). In order to combat child obesity, it is imperative for future research to explore the phenomenon of parent education, behaviors, perceptions and the impact on child weight status within a strong theoretical conceptual model. The purpose of this dissertation is to build a theoretical conceptual model and testable model to understand the associations among African American parent education, behaviors, and perceptions of their child's body size and the impact on their child's weight status. Methods: The School Physical Activity and Nutrition survey (SPAN) was developed using a repeated cross-sectional study design to estimate changes in body mass index (BMI) by three major racial/ethnic groups (African Americans, Hispanics, and whites/others) in Texas public school children (Hoelscher, Day, Lee, Frankowski, Kelder, Ward & Scheurer, 2003). For this study, only African American 4th grade students (n=372) and their parents (n=372) will be used. Data analyses included descriptive statistics, multiple regression, moderation, and mediation analysis. Results: Findings suggest that a parent's level of education had a direct impact on health knowledge and discrimination. Parent perceptions of their child's body size was directly associated with child weight status, but parent weight status moderated this relation. The number of children living in the home had a direct impact socioeconomic status (SES), SES had a direct impact on stress, and stress had a direct impact on parenting rules and discipline. The more physical activity parents did with their children, the lower their child's weight status, however, parent weight status mediated this relation. Lastly, as the consumption of more healthy foods increased, so did the child's weight status. Implications: The findings of this study offer a comprehensive picture of how parent education, perceptions and behaviors impact child weight status. These results highlight suggested areas that may be used by social work practitioners, researchers, and policy makers when working African American parents and their children. This study also suggests a final robust theoretical conceptual model of theory and evidence-based literature to guide future research.

African American families are often multi-generational, with extended family members, especially women, caring for child relatives. Yet little is currently known about the influence of these female extended family members (EFMs) on African American children's dietary and physical activity behaviors. The purpose of this study was to examine relationships between the amount of time an African American child spent with an EFM, the frequency of child physical activity and eating around the EFM, and family members' weight status. Data were collected and analyzed as part of a larger study examining relationships in African American families (n=47). Participating families consisted of the mother, her child aged 8-12 years, and an EFM. Participants were recruited from community organizations in Guilford County, North Carolina. Family Systems Theory guided the research in accordance with current literature which suggests that family-based lifestyle interventions achieve better outcomes than education or information only interventions. Eighty-eight percent of non-pregnant mothers (n=44) and 87% of non-pregnant EFMs (n=45) were classified as overweight or obese as defined by Body Mass Index (BMI) compared to 47% of the children in the study as defined by Body Mass Index For Age (BMI-For-Age). EFMs comprised 10 grandmothers, two great grandmothers, one great aunt, 19 aunts, and 15 cousins. No significant association was found between time spent with EFMs and frequency of child eating and physical activity behaviors. School nurses are well positioned to assess, intervene, and evaluate efforts to positively impact this population, however school nurse practice for obesity varies greatly. The purpose of this dissertation was to examine concepts related to the role of school nurse providing care to school age children with obesity using both quantitative and qualitative methods. Study 1 examined the self-reported physical and psychosocial quality of life of school age children with obesity and included descriptive comments from the school nurses providing their case management. The study found that the students with obesity had lower self-reported quality of life when compared to a healthy weight sample and comments from the school nurses reported the complex physical and psychosocial dynamics for students with obesity in the school setting. These findings supported that school-based interventions need to consider both physical psychological issues experienced by students to lead to successful outcomes. Study 2 investigated the experiences of school nurses providing care to students with overweight or obesity including perceived barriers and ways that school nurses successfully implement interventions with these students. The findings of this study suggest that limited time, heavy workload, limited resources, and supervisor pressure may prevent obesity interventions thus leading school nurses to use a "seize the moment" approach where they use any contact time with students to provide interventions and suggest that activities requiring small amounts of time may be best suited for successful implementation by school nurses. Nurses in this study described experiences associated with moral distress, a concept that has not been previously studied in school nurses. Study 3 examined the level of moral distress that exists in school nurses and its relationship to common moral dilemmas and school nurse characteristics. School nurses were found to experience moral distress and it was strongly related to many of the moral dilemmas experienced by school nurses. This study provides an awareness of this issue for school nurses and includes policy implications for school nursing practice.

As a result of childhood overweight and obesity, children are developing numerous issues related to their weight status. The primary purpose of this quantitative study is to determine whether a significant relationship exists between the academic, social, and physical self-concept and academic achievement of elementary school children. In addition, the study also sought to assess the differences and predictability, if any, between the academic achievement of underweight, appropriate weight, overweight, and obese Caucasian and African American elementary children. Two hundred and four subjects were administered a demographic data sheet, the Multidimensional Self Concept Scale, and the Children's Physical Self-Concept Scale. For the purpose of exploring the possibility of a significant relationship between the academic, social, and physical self-concept and academic achievement of elementary school children, the Pearson Product Moment Coefficient was utilized. Additionally, a MANOVA and a multiple linear regression were used to examine the possible differences and the predictability of the demographics, self-concept, and academic achievement of elementary school children. Findings suggest three of the hypotheses of the study were significant. Significant relationships between self-concept and academic achievement were indicated. Furthermore, significant differences were established between the self-concept of underweight, appropriate weight, overweight, and obese elementary school children. Also established in this study, was the predictability of the self-concept of underweight, appropriate weight, overweight, and obese elementary school children.

Nationwide, roughly nine million children over six years of age are obeseâ€"with elevated risks of both health conditions, such as diabetes and hypertension, and poor quality of life, possibly throughout adulthood. The Institute of Medicine (IOM) report, Preventing Childhood Obesity: Health in the Balance, was released in September 2004 and identified promising approaches for obesity prevention efforts and a set of recommendations for a variety of stake holders and sectors. The IOM is building on its previous work by initiating a new study to assess progress in childhood obesity prevention efforts. In 2005, the IOM organized three regional meeting in the Midwest, South, and Western United States to galvanize obesity prevention efforts of local, state, and national decision-makers, community and school leaders, grassroots organizations, and industry including the food, beverage, restaurant, leisure, and entertainment industries. In collaboration with the Kansas health Foundation (KHF), the IOM held the study's first regional symposium in Wichita, Kansas on June 27-28, 2005. The symposium was structured to include three panels that focused on challenges and innovations for obesity prevention and school policies, school programs, and additional steps that can be taken by numerous stakeholders to overcome barriers to progress. Three break-out sessions focused on creating and strengthening linkages with other sectors to promote childhood obesity prevention including links between schools and home, community, and health care; links between schools and industry; and links between schools and the built environment. This brief summary highlights the recurring themes for accelerating change and moving forward with obesity prevention efforts that emerged from the symposium; forge strategic partnerships; empower local schools and communities; educate stakeholders; evaluate obesity prevention efforts; document the benefits of obesity preventions; innovate to address barriers; use a systems approach; and develop a long-term strategic plan. The findings of this summary, along with those of two other symposia, and a more detailed discussion of insights and regional examples will be incorporated in the committee's final report that will be released in 2006.

Book excerpt: ABSTRACT: Childhood obesity has become an epidemic in the United States over the past decade. Not only does obesity present medical problems for children, but these children can also expect to have increased morbidity and mortality from obesity as adults. Obesity prevention could greatly reduce health care spending in the U.S. and improve the quality of life among Americans. This retrospective, cross-sectional study addressed the prevalence of obesity in children entering public school kindergarten in Lincoln and Gaston Counties (N=2944), both located in the Piedmont region of North Carolina. Results revealed that 15% of kindergartners in Lincoln and Gaston County were obese and an additional 15% of children were at risk for obesity. There were no significant differences in prevalence of obesity or at risk for obesity between boys and girls or among racial groups. No relationship between obesity and socioeconomic status was found. However, odds ratios revealed that children who were obese were 2 times more likely to have high-normal blood pressure and 3.5 times more likely to have hypertension. Recommendations for prevention and treatment efforts to reduce obesity are made.
