Book excerpt: The purpose of this book is an examination of the anatomy and motility of the pylorus in view of many divergent views. Anatomical findings of Cunningham, Forssell and Torgersen et al. are discussed and integrated with radiological forms of movement. It appears that the pyloric ring is not a "sphincter" but that it forms part of a more intricate sphincteric mechanism. Evidence for a sphincteric cylinder is presented. Its cyclical activity may open and close the aperture and result in propulsion and retropulsion. The role of gastroscopy, manometry, ultrasonography and radionuclides in pyloric motility and gastric emptying is discussed. Anatomical and functional alterations of the sphincteric mechanism in pathological conditions, e.g. gastritis, gastric ulceration and pyloric carcinoma are described. This may aid in the clarification of the pathogenesis.

Available in PDF, EPUB and Kindle. Book excerpt: Advanced Perioperative Crisis Management is a high-yield, clinically-relevant resource for understanding the epidemiology, pathophysiology, assessment, and management of a wide variety of perioperative emergencies. Three introductory chapters review a critical thinking approach to the unstable or pulseless patient, crisis resource management principles to improve team performance and the importance of cognitive aids in adhering to guidelines during perioperative crises. The remaining sections cover six major areas of patient instability: cardiac, pulmonary, neurologic, metabolic/endocrine, and toxin-related disorders, and shock states, as well as specific emergencies for obstetrical and pediatric patients. Each chapter opens with a clinical case, followed by a discussion of the relevant evidence. Case-based learning discussion questions, which can be used for self-assessment or in the classroom, round out each chapter. Advanced Perioperative Crisis Management is an ideal resource for trainees, clinicians, and nurses who work in the perioperative arena, from the operating room to the postoperative surgical ward.

Available in PDF, EPUB and Kindle. Book excerpt: This volume reviews the most recent knowledge in the field of gastrointestinal motility in health and disease. The topics addressed include basic as well as clinical data concerning the motor functions of the entire gut: the lower oesophageal sphincter and the gastro-oesophageal reflux; the gastric emptying and the role of the pylorus; the motility of the biliary tract and its disorders; the cyclic motor activity of the gut and intestinal transit; the colonic and ano-rectal motility. There are also impOltant contributions in physiology and pharmacology relating to the neurohumoral regulation of the gut, and the function of digestive smooth muscle. Several papers explore the nature of the linkage between brain and gut. a link which has long been deduced by clinicians but not, until recently, systematically explored by scientists. The individual papers, selected from a large number of submissions, have been subject to 'peer-review' by an inter national committee which includes both cliniCians and basic scientists. Therefore this book should serve as an up to date source of information for researchers concerned with basic sciences as well as for clinicians in gastroenterology, medicine and surgery. C. Roman v This volume is dedicated to the memory of two friends and colleagues: Professor Dr J. HELLEMANS Professor Dr H. MONGES Acknowledgments This was the 9th of this series of symposia held alternatively in Europe and North America, and the first to be held in France.

Book excerpt: The ASCRS Textbook of Surgery of the Colon and Rectum offers a comprehensive textbook designed to provide state of the art information to residents in training and fully trained surgeons seeking recertification. The textbook also supports the mission of the ASCRS to be the world’s authority on colon and rectal disease. The combination of junior and senior authors selected from the membership of the ASCRS for each chapter will provide a comprehensive summary of each topic and allow the touch of experience to focus and temper the material. This approach should provide the reader with a very open minded, evidence based approach to all aspects of colorectal disease. Derived from the textbook, The ASCRS Manual of Surgery of the Colon and Rectum offers a “hands on” version of the textbook, written with the same comprehensive, evidence-based approach but distilled to the clinical essentials. In a handy pocket format, readers will find the bread and butter information for the broad spectrum of practice. In a consistent style, each chapter outlines the condition or procedure being discussed in a concise outline format – easy to read, appropriately illustrated and referenced.

Available in PDF, EPUB and Kindle. Book excerpt: The purpose of this volume is to serve as a useful, up-to-date reference manual and guide for the diagnostic and therapeutic approaches to common adult and pediatric gastrointestinal motility problems. In order to enhance the understanding of these disorders, a problem-oriented approach has been chosen, and whenever possible the authors have provided clinical case scenarios to illustrate their message. Gastrointestinal Motility has evolved from an esoteric laboratory tool to a sophisticated diagnostic technique that is now widely used clinically to guide management of complex gastrointestinal problems. Today it is the most rapidly growing subspecialty within gastroenterology. Previously, many of the gastrointestinal motility problems were either ignored or attributed to a disturbance of 'psyche'. With the growing knowledge and understanding of how gastrointestinal muscle and nerve dysfunction can cause disease, we are at the threshold of a revolution in our approach to the diagnosis and treatmentof gastrointestinal motility disorders. Practitioners and trainees in gastroenterology, radiologists, internists, family practitioners, as well as general surgeons, each of whom encounter patients with motility problems on a daily basis, will find this volume of use. The authors, experts in the field, have provided a succinct, practical, and balanced approach that can be used in any laboratory.

Book excerpt: Three distinct types of contractions perform colonic motility functions. Rhythmic phasic contractions (RPCs) cause slow net distal propulsion with extensive mixing/turning over. Infrequently occurring giant migrating contractions (GMCs) produce mass movements. Tonic contractions aid RPCs in their motor function. The spatiotemporal patterns of these contractions differ markedly. The amplitude and distance of propagation of a GMC are several-fold larger than those of an RPC. The enteric neurons and smooth muscle cells are the core regulators of all three types of contractions. The regulation of contractions by these mechanisms is modifiable by extrinsic factors: CNS, autonomic neurons, hormones, inflammatory mediators, and stress mediators. Only the GMCs produce descending inhibition, which accommodates the large bolus being propelled without increasing muscle tone. The strong compression of the colon wall generates afferent signals that are below nociceptive threshold in healthy subjects. However, these signals become nociceptive; if the amplitudes of GMCs increase, afferent nerves become hypersensitive, or descending inhibition is impaired. The GMCs also provide the force for rapid propulsion of feces and descending inhibition to relax the internal anal sphincter during defecation. The dysregulation of GMCs is a major factor in colonic motility disorders: irritable bowel syndrome (IBS), inflammatory bowel disease (IBD), and diverticular disease (DD). Frequent mass movements by GMCs cause diarrhea in diarrhea predominant IBS, IBD, and DD, while a decrease in the frequency of GMCs causes constipation. The GMCs generate the afferent signals for intermittent short-lived episodes of abdominal cramping in these disorders. Epigenetic dysregulation due to adverse events in early life is one of the major factors in generating the symptoms of IBS in adulthood.
