Book excerpt: With more than 30 million people living with HIV/AIDS and about 2.5 million people infected in 2006 alone, the HIV/AIDS epidemic has become one of the highest priorities on the global development agenda. About 65% of the world population living with HIV/AIDS live in sub-Saharan Africa. Sub-Saharan Africa also lags behind in achieving international development goals and the burden of HIV/AIDS further exacerbates the cycle of poverty and inequality. Donor aid is often given on the assumption that the recipient country has adequate capacity in the government to manage all aspects of development: planning, fiscal management, programme design, financial control and budgeting, project implementation, accountability and monitoring and evaluation. The multiplicity of funding modalities in the donor field makes the absorption and spending of aid a challenge. The national capacity influences the ability to absorb donor aid and the role of public administration in the national development programmes cannot be over-emphasised. To attain the Millennium Development Goals (MDG) of the Millennium Declaration of the United Nations, special attention needs to be given to HIV/AIDS. Should the AIDS pandemic not be resolved properly, almost all the MDG might be in jeopardy. The Paris Declaration on aid effectiveness, harmonisation and alignment does not make any provision for safeguarding funds for the mitigation of the impact of HIV/AIDS in recipient countries. The United Nations' UNAIDS has made it clear that to effectively attain any development goals in the developing world, mainstreaming of HIV/AIDS in all development activities needs to be done. Conceptually, mainstreaming HIV/AIDS means to bring HIV/AIDS to the centre of the development agenda, which requires change at individual, departmental and organisational level. From this overview emerges the research question: Will the changing international official development assistance environment have an influence on financing the response to HIV/AIDS in the developing countries of southern Africa? This is a qualitative and descriptive study, based on a literature survey utilising a deductive approach in the fields of public administration, public financial management, development, development administration, HIV/AIDS and official development assistance (ODA). A model for a sector-based approach to financing the AIDS epidemic in the country was developed. The model is based on the many different mechanisms, best practices and lessons learned of many different organisations. It suggest one overarching body with legal status that can manage a country's HIV and AIDS programme as a cross cutting issue. The Central body will have representation of all sectors, including government, donors and multilaterals. All the role players in the field should be involved in the development, planning and implementation of the programmes.

Book excerpt: Government spending is an important component of public finance that enables government to deliver services and implement policies of the executive. This research examines government spending for HIV/AIDS treatment at the provincial sphere of government and queries whether citizens can influence public policy decision-making for HIV/AIDS treatment. The research considers the nature of the state, it being either a unitary system or a federal system (centralised or decentralised). Importantly, the research examines the effect of citizens on public policy decision-making, as they exercise their franchise to vote. Moreover, this dissertation empirically examines how government has spent in response to the HIV/AIDS epidemic and measures any influence voters may have on government spending, with spending giving indication of public policy decision-making by the government of the day. The literature review identifies techniques used by other researchers on the matter and previous research on the relationship between voters and government spending. For example, Husted and Kenny studied voters' potential to influence government spending at the provincial [state] sphere of government. The outcome of their research was inconclusive but at the least suggests those variables that might effect government spending - variables that might be used for this dissertation. For example, institutional and latent groups are identified, to examine voting effects and other influences on policy decisions. The literature review discusses collaborative and voter-collective actions, for explanatory effects on policy decisions as well. Essentially, variables and specifications for a regression model are suggested to facilitate a test of hypothesis that voters do not have the ability to influence provincial government spending for HIV/AIDS policy. The research design for this dissertation is therefore evaluative and generally measures a programme. Indeed, that programme is one of HIV/AIDS treatment in South Africa. Bivariate and multivariate relationships are determined to begin to answer the research question. Can voters effect public policy decision made on HIV/AIDS treatment? Put yet another way, that question is whether the electorate, as reflected by voter turnout, can influence provincial spending on HIV/AIDS treatment? The hypothesis is: The electorate does not have the potential to influence provincial spending for HIV/AIDS treatment. Spending, or lack thereof, reflects a public policy decision of government. Notably, reference to the government of the day implies the ruling [South] African National Congress (ANC) party. Conclusively, after a number of iterations that involved running several bivariate and multivariate regression models, the outcome was that voters could influence provincial spending for HIV/AIDS treatment. The hypothesis was rejected that the electorate does not have the potential to influence provincial spending and HIV/AIDS.

Book excerpt: HIV/AIDS continues to take a tremendous toll on the populations of many countries, especially in sub-Saharan Africa. In some countries with high HIV prevalence rates, life expectancy has declined by more than a decade and in a few cases by more than two decades. Even in countries with HIV prevalence of around 5 percent (close to the average for sub-Saharan Africa), the epidemic can reverse gains in life expectancy and other health outcomes achieved over one or two decades. This volume highlights work conducted under the umbrella of a World Bank work program on The Fiscal Dimension of HIV/AIDS, including country studies on Botswana, South Africa, Swaziland, and Uganda. It covers four aspects of the fiscal dimensions of HIV/AIDS: First, it aims for a comprehensive analysis of the fiscal costs of HIV/AIDS, with a wider scope than a costing analysis focusing on only the policy response to HIV/AIDS. Second, it embeds the analysis of HIV/AIDS costs in a discussion of the fiscal context, and interprets these costs as a quasi-liability, not a debt de jure, but a political and fiscal commitment that binds fiscal resources in the future and cannot easily be changed, and very similar to a pension obligation or certain social grants or services. Third, it develops tools to assess the (fiscal dimensions of) trade-offs between HIV/AIDS policies and measures that take into account the persistence of these spending commitments. Fourth, most of the fiscal costs of HIV/AIDS are ultimately caused by new infections, and this study estimates the fiscal resources committed (or saved) by an additional (or prevented) HIV infection. Building on these estimates, the analysis here is able to assess the evolving fiscal burden of HIV/AIDS over time.
