Dupre also attacked Dejerine's view, affirming that there were many hypochondriacs, many possessed with obsessions, many nosophobiacs, who were not and could not be helped by any amount of suggestion. Brissaud, Ballet, Souques, and Meige also assailed Dejerine's statement. In the dis cussion the weight of opinion favored the conclusion that, outside of hysteria, there are no morbid states at present well defined, in which one may meet disturbances present ing exactly the same characters as hysteria itself. If the patient be tuber culous, then according to Pirquet, there appears within 48 hours a papule surrounded by a hyperemic zone 1 inch to 11} inches in diameter. The disappearance of this pap ule is Often followed by a brownish pigmentation of the skin which may persist for some time. The reaction has various degrees Of severity, from a very small papule to a large and intensely inflamed area. However, the se verity Of the reaction is not a measure Of the extent of the tuberculous process. The reaction is not attended by gen eral symptoms such as fever and malaise. Adamson.4 The patient, an attractive young woman, was not suspected by Adamson of hysteria. Her illness began with the unexplained appearance of patches of weeping eczema on the face. These continued to recur on the cheeks, forehead, neck, arms, and later thighs and legs despite treatment and covering with dressing. The crop never failed for a year, the condition being some times better, sometimes worse. In the initial stage there was an erythemous flush which rapidly became moist, suppurated and healed with a crust. The eruption was often square or linear, sometimes round. A dermatol ogist consulted pronounced the case one of dermatitis herpetiformis. The patient went to a specialist without results. Another dermatologist regarded it as a case of dermatitis artefacta. The parents were skeptical; watch ing by a nurse failed. The patient was removed at length to a nursing home. Here three days after admission, a movement under the bedclothes led to the detection of the lady's hand holding most unsuspiciously a handkerchief, but in the handkerchief was a small ragged piece of pumice-stone! Thereupon this protracted dermatosis came quickly to a complete end. The patient later feigned hemoptysis and bowel obstruction, and was detected. The left knee-jerk was exaggerated, and the para doxie reflex was obtained with the greatest facility. Ba binski's toe-sign, Oppenheim's reflex, and ankle-clonus were all absent. An operation by Da Costa restored the patient to health, and in six weeks the paradoxic reflex, looked for by Gordon, was found wanting, as was also the increased knee-jerk. Gordon believes that his early con elusion is warranted, and it is here repeated: The para doxie reflex is a sign of value in -the beginning of an organic affection of the motor system or in cases of irrita tion. While the Babinski sign is of inestimable value in cases with a definitely established lesion of the motor sys tem, the paradoxic reflex, to say the least, is a Sign of transient irritation or of a sligh't lesion of the same motor pathway. Book excerpt: Today’s medicine is spiritually deflated and morally adrift; this book explains why and offers an ethical framework to renew and guide practitioners in fulfilling their profession to heal. What is medicine and what is it for? What does it mean to be a good doctor? Answers to these questions are essential both to the practice of medicine and to understanding the moral norms that shape that practice. The Way of Medicine articulates and defends an account of medicine and medical ethics meant to challenge the reigning provider of services model, in which clinicians eschew any claim to know what is good for a patient and instead offer an array of “health care services” for the sake of the patient’s subjective well-being. Against this trend, Farr Curlin and Christopher Tollefsen call for practitioners to recover what they call the Way of Medicine, which offers physicians both a path out of the provider of services model and also the moral resources necessary to resist the various political, institutional, and cultural forces that constantly push practitioners and patients into thinking of their relationship in terms of economic exchange. Curlin and Tollefsen offer an accessible account of the ancient ethical tradition from which contemporary medicine and bioethics has departed. Their investigation, drawing on the scholarship of Leon Kass, Alasdair MacIntyre, and John Finnis, leads them to explore the nature of medicine as a practice, health as the end of medicine, the doctor-patient relationship, the rule of double effect in medical practice, and a number of clinical ethical issues from the beginning of life to its end. In the final chapter, the authors take up debates about conscience in medicine, arguing that rather than pretending to not know what is good for patients, physicians should contend conscientiously for the patient’s health and, in so doing, contend conscientiously for good medicine. The Way of Medicine is an intellectually serious yet accessible exploration of medical practice written for medical students, health care professionals, and students and scholars of bioethics and medical ethics.
