After a critical review of the literature, Towle says that protein sensitization is now an accepted fact. The theories as to its meaning and its mechanism differ and much that has been published concerning the process lacks confirmation. There is still much work to do in allied fields, such as the diatheses and the intoxications, before full use can be made of the new cult. In the meantime, while accepting the principles of the cutaneous food tests, interpretation must await fuller, better defined understanding of the process of protein sensitization before it can attain its greatest efficiency. Protein Sensitization in Skin Diseases. A critical discussion of the various theories, a review of the literature and the results of the tests applied in a series of cases appear in an article by Highman and Michael. Of the sixty-three positive reactions, thirty-seven were to vegetable and twenty-six to animal proteins. Reactions to more than one protein occurred in eleven of the twelve cases tested. Reactions to both vegetable and animal proteins occurred in ten cases. In one case only buckwheat was tried. Suprarenal extract was of benefit in eight cases; it was not employed in five and caused collapse in one. The authors believe that urticaria and allied conditions, notably angioneurotic edema, are anaphylactic manifestations. Moreover, they believe the positive protein tests prove them to be anaphylactic. They believe that the protein food tests are of great value inasmuch as the tests indicate sensitization to a definite protein which, in treatment, is to be removed from the diet. Book excerpt: Excerpt from The Retrospect of Practical Medicine and Surgery, Vol. 6: Being a Half-Yearly Journal, Containing a Retrospective View of Every Discovery and Practical Improvement in the Medical Sciences; July December, 1842 [The chief object of this paper of Dr. Patterson's is to contradict many of the commonly received opinions of medical men with respect to the mode in which the skin becomes discoloured during the use of this drug, and also to point out a superior way of counteracting this effect. We need not particularize the spasmodic and painful diseases in which this medicine has been found efficacious; epilepsy, chorea, pyrosis, gastralgia, &c., are amongst the diseases which have been particularly alleviated by it. But its discolouring effect on the skin has always been a great objection to its continued administration. Dr. A. T. Thompson, in his Elements of Materia Medica, proposes to counteract this effect by the contemporaneous exhibition of nitric acid, on the principle that it would prevent the formation of chloride of silver. "He supposes that the nitrate is taken into the circulation undecomposed, and, arriving in that state at the capillaries of the skin, it is decomposed there and converted into chloride of silver, which is deposited in the rete mucosum. The chloride, he says, acquires a grey, leaden colour from its contact with animal matter; and as it is insoluble, it is incapable of being reabsorbed, is fixed in the rete mucosum, and a permanent stain is given to the skin; and to produce this effect, a more than usual quantity of muriates must he separated by the cuticular capillaries." The procedure consists in scarifying as for vaccination. This scarified area is treated by Pirquet with a 4 per cent. Solution Of Old tuberculin. Others use from 1 to 4 per cent. Solutions. If the patient be tuber culous, then according to Pirquet, there appears within 48 hours a papule surrounded by a hyperemic zone 1 inch to 11} inches in diameter. The disappearance of this pap ule is Often followed by a brownish pigmentation of the skin which may persist for some time. The reaction has various degrees Of severity, from a very small papule to a large and intensely inflamed area. However, the se verity Of the reaction is not a measure Of the extent of the tuberculous process. The reaction is not attended by gen eral symptoms such as fever and malaise. Dupre also attacked Dejerine's view, affirming that there were many hypochondriacs, many possessed with obsessions, many nosophobiacs, who were not and could not be helped by any amount of suggestion. Brissaud, Ballet, Souques, and Meige also assailed Dejerine's statement. In the dis cussion the weight of opinion favored the conclusion that, outside of hysteria, there are no morbid states at present well defined, in which one may meet disturbances present ing exactly the same characters as hysteria itself.
