This was a non-experimental, descriptive study focusing on perceptions of persona issues, conflicts, and concerns of nurses working 12-hour shifts. Imogene King's nursing model based on general systems theory and goal attainment was the conceptual framework for this study. Data was collected utilizing a questionnaire distributed to registered nurses working at an urban hospital in Southern Ohio. Fifty nurses responded to the questionnaire including 45 staff Registered Nurses, 4 Clinical Team Managers and 1 Clinical Nurse Specialist. Overall, the research questions did not reveal any statistically significant findings, there were however several analyses worthy of discussion. Work status, personal time away from work, age ranges in relation to family obligations and the ability to pursue professional enrichment while working certain shifts were included. Data was analyzed using simple descriptive correlation statistics. This study contributes to current literature and provides information to enhance nurse job satisfaction and retention. Future theory, research and practice implications were identified.

U.S. hours of service limits have been regulated for the transportation sector for many years. In recent years, a number of states have been considering legislation to limit mandatory overtime for health care workers. The volume of legislative activity seen nationwide indicates a heightened level of societal concern and the timeliness of the issue. This document summarizes recent scientific findings concerning the relationship between overtime and extended work shifts on worker health and safety. This report provides an integrative review of 52 recently published research reports that examine the associations between long working hours and illnesses, injuries, health behaviors, and performance. The report is restricted to a description of the findings and methods and is not intended as an exhaustive discussion of all important issues related to long working hours. Findings and methods are summarized as reported by the original authors, and the study methods are not critically evaluated for quality.

Book excerpt: Overtime, including both mandatory and voluntary, is associated with nurse's fatigue, job injury or illness, increased patient mistakes, and patient mortality. There are limitations to the current literature that show correlations between long hours worked and poor nursing outcomes, which likely understate the issue. A nurse working past their regular shift either mandatorily or voluntarily, is at increased risk for injury to themselves and their patients. These risks grow the longer each nurse works without adequate rest. The solution is for a standard of Nurse Mandatory Overtime Regulation be adopted nationally. A national standard is important in order to improve nursing outcomes, as current standards vary and are used differently in each state. This new National Safety Policy on Nursing Mandatory Overtime Regulation will define safe hours that nurses should work, ensure nursing facilities follow mandatory overtime regulations, and recommend it be adopted by all states. Findings are based from current studies of multiple sources; including cross-sectional studies using qualitative and quantitative data from 2004 to present, in multiple cultures and regions that primarily studied hospital registered nurses. Results of this research show that further study is warranted which better track multiple jobs and roles of nurses, their total work hours, shift lengths, risks to health, and patient mistakes or injury. Implementing the solution to nurse mandatory overtime regulations will include finding those measures that are both popular and needed in order to garner further support from stakeholders and legislators. These measures will undergo an evaluation plan that will set out the proposed details of what will be required, who will need teaching, how, and when. Dissemination to all stakeholders will highlight the significance and impact on mandatory regulations for nurses, with outlined proposals for policy changes, budget costs, how to continue to monitor the affects of overtime changes, and monitor if changes have been made. The United States "Congress in 2007 did not pass a bill that would have prohibited mandatory overtime for nurses on a national basis. As of July 2010, the bill has not come before Congress again" (Nichols, 2010, para.6). This paper shows there is a need for nurse mandatory overtime regulation at the federal or national level. Nurses have a responsibility to work safely and have a voice to say "no" to extra work hours or shifts when their health or that of their patients may be affected.

In 2003 the organization overseeing graduate medical education adopted common program requirements to restrict resident workweeks, including limits to an average of 80 hours over 4 weeks and the longest consecutive period of work to 30 hours in order to protect patients and residents from unsafe conditions resulting from excessive fatigue. Resident Duty Hours provides a timely examination of how those requirements were implemented and their impact on safety, education, and the training institutions. An in-depth review of the evidence on sleep and human performance indicated a need to increase opportunities for sleep during residency training to prevent acute and chronic sleep deprivation and minimize the risk of fatigue-related errors. In addition to recommending opportunities for on-duty sleep during long duty periods and breaks for sleep of appropriate lengths between work periods, the committee also recommends enhancements of supervision, appropriate workload, and changes in the work environment to improve conditions for safety and learning. All residents, medical educators, those involved with academic training institutions, specialty societies, professional groups, and consumer/patient safety organizations will find this book useful to advocate for an improved culture of safety.
