Book excerpt: At a time when Americans are debating the pros and cons of recent health care reforms, Concierge Medicine offers an alternative to save primary care medicine. Here, the author outlines an increasingly popular, though controversial, system that offers a high level of care to patients who still need and value a relationship with their personal physician. Dr. Knope introduces concierge medicine, which encourages patients to contract directly with physicians for personalized care that is not determined by insurance coverage but rather by the patient and doctor together. For those considering an individualized health care model that can be more affordable, cost effective and straightforward, Dr. Knope offers practical advice for finding, interviewing, and contracting with a concierge doctor.

Book excerpt: Concierge medicine has been changing the quality of healthcare since its introduction in the 1990s. This healthcare model has improved the views of both physicians and patients on the delivery of healthcare. Physicians are finding themselves overloaded with too many patients and little time for them all. Due to this, physicians have extended work hours in order to meet their patients' needs, which has decreased job satisfaction in healthcare providers. Not only are physicians dissatisfied, patients too are finding themselves unhappy with the delivery of their healthcare. The overload of patients has prompted long wait times in offices and short visits. With the introduction of concierge medicine, physicians and patients are reverted back to a higher quality of healthcare. Concierge practices promote little to no wait times along with longer, more personalized visits to meet patient needs. The issue surrounding concierge medicine is that it is not easily accessible to all. This systematic literature review aimed to examine what makes the concierge medicine model so popular. The results of this study show that patients and physicians establish stronger realtionships through concierge medicine which allows for better healthcare. The results further implicate that there is a need to expand access to concierge care for middle and lower class individuals, which can be accomplished through public insurance providers. The Affordable Care Act has played a role in expanding accessibility, however this may be in jeopardy as the current administration in office is looking to dismantle the act completely. Medicare has made strides in recent years to incorporate concierge care for its patients, resulting in a need for a policy implication that will incentivize concierge physicians to be enrolled under the list of Medicare physicians. A second policy implication is recommended to update the guidelines for Health Savings Accounts, created under the Medicare Prescription Drug, Improvement, and Modernization Act in 2003, in order to allow the funds saved to pay for concierge care. As concierge care continues to gain popularity, accessibility needs to be expanded in order to give patients the ability to access a higher quality form of healthcare that has proven to improve the view on healthcare for both patients and physicians.
