New blood pressure (BP) diagnostic thresholds for hypertension were released, which were set at 130/80 mmHg. As a consequence, millions of individuals in the world will be diagnosed as hypertensive, recognizing that they are at greater risk of presenting a CV event. Prevention and control of high BP will become the main focus for reducing the burden of CV disease, requiring a changing of cultural beliefs in some way similar to what happened in the last century with smoking. Strategies for prevention of the rising of BP with age, and the BP reduction in individuals already with high levels, are more complex than those related to smoking control. These strategies involve solid evidence to be implemented in populations. The extensive scientific literature dealing with hypertension and BP regulation is among the top dedicated to a single disease. The chapters and contents follow the clinical reasoning pathways. The characterization of the risks of high blood pressure is presented in the first chapter, discussing the evidence that led to changes in diagnostic thresholds and to the recommendations for maintaining BP within these limits in populations. Reasons for BP rising with age will follow, identifying the causes that must be fought to preventing the incidence of hypertension. Diagnosis of hypertension deserves a special chapter. The final chapter presents the fundamentals to select drug and non-drug therapies indicated in the prevention and controlling of high blood pressure.

It is this universality that has prompted us to gather, in this book, data on arterial blood pressure obtained in different parts of the world. Moreover, cerebrovascular mortality, which is the commonest cause of death from hypertension, is decreasing in most Western countries and in Japan, and the reasons for this are still far from clear. A major problem in comparing blood pressure values from different centers is the standardization of the measurement. Complete standardization will never be achieved if one takes into account the numerous factors that may influence blood pressure in epidemiological studies. Whether blood pressures are measured by doctors or by technicians, are recorded at home, in the working place or in a hospital, in sitting, standing or supine position, and is taken during the same examination-all of these whether a blood sample factors can influence blood pressure measurement. But meals, time of day (blood pressure being higher in the evening), heart rate, cuff size, stethoscope used, digit preference, month of year, temperature, etc., can equally influence the measurement. Home reading of arterial blood pressure at standardized times is probably the best answer to all of these problems and has been used with gratifying results in a comparative study between Belgium and Korea.

Other vasopressor factors, such as catecholamines, vasopressin, and serotonin, have been studied extensively and were assumed to play additional parts in blood pressure regulation. Public and even physician interest in the subject remained minimal until the early forties, and even then there were not more than a dozen "fulltime" investigators worldwide. The first organization devoted to communicating regularly the results of hy pertension research was created in Cleveland in 1945; it became the Council for High Blood Pressure Research of the American Heart Association in 1949. The early history of hypertension is not a story of progressive achievement. Richard Bright is given great credit, followed by Mahomed, Allbutt, Riva-Rocci, Bergman and Janeway, but it was not until Volhard, Fahr and Allen that some semblance of order emerged. They were followed by a younger, much more vigorous group of investigators who in fact initiated modern research in hypertension. Two more generations have fol lowed. In the course of some fifty years, we have seen emerge a magnificent body of evidence that has prescribed good treatment and contributed a considerable understanding of the many complex mechanisms involved in the hypertensions. Treatment is now actively pro moted by public health agencies, including the World Health Organization. This is an other case of treatment preceding full understanding of the nature of the disease.
