The murmur is generally transmitted, but in only ten cases is there cardiac enlarge mentor pulmonary accentuation; but the absence of these signs is explained by the physical facts. We cannot expect much increase in area where the heart though frequently dilated, is atrophied rather than hypertrophied, and the right ventricle must often be too weak to give rise to pulmonary accentuation. In pernicious anaemia where pulmonary accentuation does occur (as in most advanced cases) it is probably dependent on the dyspnoea, due. Case II. - Katie F., a young woman, was admitted into the Penn sylvania Hospital on March 6, 1897, having been ill for about five days. She had been previously in good health, and had never had either measles or scarlet fever. On admission to the hospital her tempera ture was noted at but it soon rose to the urine showed a trace of albumin, and one finely granular cast was found. The skin was very fair, so that any spots could have been readily detected, but none were observable. The next day there appeared on the face and arms a bright red, erythematous rash, which lessened somewhat after sponging, and was not always equally distinct, seeming to come and go, though never wholly leaving. There was no rash of the kind on the body, but it was specially observed that pressure on the skin everywhere produced a red spot which only slowly faded. The temperature ranged between 103 and On the 9th great disturbance of respiration was perceived, the breathing fell to twelve in the minute, and was jerky and irregular, and the erythematous rash which had disappeared showed itself again on the face and arms, giving the patient the appearance of scarlet fever - an appearance which was increased by a redness of the tonsils and the pharynx, the latter being covered with tenacious mucus. She was very ill, but the temperature-range was not materially different; during the night it ranged from to and she was very delirious. Book excerpt: Excerpt from Gaillard's Medical Journal, Vol. 70: January, 1899 Captain Hughes said: Besides abortive and undefined short cases of fever, there is a short attack, often called febricula, which appears to be of a specific nature. It is common in hot countries, and attacks almost every one there, often year after year, and in a large num ber of cases at the same time of each year. It is a filth disease, and not the result of exposure to sun. The pyrexia lasts some three or four days, with pain in the back and head, foul tongue, and but little else. In the army and navy we are frightfully handicapped by two points, which the authorities in many places have not yet relinquished: (1) That there is a disease called typho-malarial fever. Though abolished from the Nomenclature at last, it still comes to the fore. There is no difference between enteric fever attacking a malarialized subject and enteric fever attacking a syphilized or tuberculous subject from a nosological point of view while the condition may be compared to a broken leg complicating a case of enteric or other attack of fever. Though the two diseases coexist, their coexistence does not constitute a new disease. (2) Enteric fever is enteric fever all the world over, and does not become a climatic disease directly we leave the confines of Europe or America. Conse quently preventive measures are called for in India as in England. An Englishman and his wife, who live long in India, may alter their habits and even appearance to suit their new environments, but for all that they and their children continue to be English. So the enteric poison may become modified in virulence, etc., by climatological or topographical conditions; but it does not cease to be enteric fever, the result of the enteric microbe, and therefore of a preventable nature. Anomalous forms of fever, which may seem to negative this view, need working out, when they will be found to have their own cause. Lastly, Mediterranean, Malta or undulant fever is a specific disease, and probably exists in many warm climates. We would ask the reader to pause for a moment and reflect upon the synopsis on the next page. To treat these diseases by taking up each subdivision as they appear in this table would weary the reader and defeat the pur pose we have at this time before us. My object is rather to point out an easy method of study by which the mind may grasp the subject, and hence I shall be able only to take up a subject here and there in my series of articles, leaving the investigator to pur sue the subject beyond the limits assigned me in this journal.
