Traditional bullying, cyber bullying, sexual assault and body dysphoria's effect on depression and increased suicide risk are the focus of this dissertation. Data from the 2015 Youth Risk Behavior Surveillance System (YRBSS) by the Center for Disease Control (CDC) is analyzed for this purpose. Logistic regression and multiple regression are used to explore the relationships between these variables. Adolescent females appear to be at increased risk of depression and suicide due to school bullying, cyber-bullying, sexual assault and body dysphoria. Results from the YRBSS indicate that many high school students are engaged in health-risk behaviors associated with suicide: a leading cause of death among teens in the United States. The results support the hypotheses that adolescent female victims of traditional bullying, sexual assault and body dissatisfaction are more at risk of depression and suicidal thoughts, plans and implementation. Cyberbullying had a stronger impact on whether boys reported that they felt sad and hopeless, made a suicide plan and were a suicide risk than it did for girls.

In addition, substance use is also common among adolescents in the United States. Both the experience of depressive symptoms and suicidality and of having been bullied have been associated with substance use. However, the context in which these adolescent experiences and behaviors are occurring are changing (e.g., changing state-level cannabis laws, the advent of electronic vapor products, increased use of social media, and the COVID-19 pandemic), warranting updated, comprehensive investigations in this area. We aimed to 1) determine if depressive symptoms and suicidality are associated with current use of alcohol, electronic vapor products, cannabis, and prescription pain medicine among adolescents; 2) determine if the associations of depressive symptoms and suicidality with use of alcohol, electronic vapor products, cannabis, and prescription pain medicine have changed over time in adolescents; 3) determine if the experience of being bullied electronically or at school is associated with the current use of alcohol, electronic vapor products, cannabis, and prescription pain medicine among adolescents; and 4) describe the impact of COVID-19 on adolescent-reported substance use, social media use, and depressive symptoms. We utilized 2019 Youth Risk Behavior Study (YRBS) data to assess cross-sectional associations of the experience of having been bullied, depressive symptoms, and suicidality with substance use and YRBS data from 1991 to 2019 to assess for changes in cross-sectional associations of depressive symptoms and suicidality with substance use over time. We applied appropriate survey weights and utilized multivariable logistic regression to model associations between past 12 month depressive symptoms and suicidality indicators (considering suicide, suicide plans, suicide attempts, and injurious suicide attempts) as independent variables in Aims 1 and 2, past 12 month experience of being bullied electronically and at school as independent variables in Aim 3, and past 30 day ever use of substances (cannabis use, alcohol use, binge drinking, electronic vapor product use, or prescription pain medicine misuse) as dependent variables in Aims 1, 2, and 3. Due to lack of time points for which data on past 30 day prescription pain medicine was available, we additionally considered lifetime prescription pain medicine misuse as a dependent variable in Aim 2. We presented odds ratios (ORs), adjusted ORs (aORs), and corresponding 95% confidence intervals (CIs) for unadjusted, partially adjusted (including covariates for sex, racial/ethnic identity, sexual identity, and age), and fully adjusted (additionally including other substance use) models. To assess the impact of the COVID-19 pandemic (Aim 4), we conducted a scoping review and a survey-based study. For the scoping review, we utilized Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) guidelines and searched PubMed, PsycINFO, and Embase. Inclusion criteria were quantitative or qualitative studies among 12 to 17 year-olds or their caregivers and provided results that were specific to adolescents (age 10 to 19 years or middle school to high school) in the United States and data collected at least at one time point since March 1, 2020 (reflecting the beginning of the impact of the pandemic on daily life for the public). For the survey-based study, we conducted an anonymous, REDCap-based online survey of adolescents (ages 13 to 19 years) in the United States assessing retrospective reports of social media use, substance, and depressive symptoms in April 2019, April 2020, and April 2021. We summarized participant responses with frequencies and percentages and assessed for differences over time with Fisher’s exact tests. In weighted analyses of 2019 YRBS participants, 36.2% of participants reported depressive symptoms in the past 12 months and 30.9% of participants reported any use of electronic vapor products in the past 30 days. Book excerpt: Sexual violence has a significant impact on the health and well-being of millions of people globally, with experiences of sexual violence associated with increased depressive symptoms, anxiety and PTSD, as well as increased sexual risk behavior and reduced sexual assertiveness. However, much remains unknown regarding viable targets for decreasing the negative consequences associated with experiences of sexual violence. While experiences of sexual violence are a significant predictor of negative consequences, greater attention is needed in identifying potential mediating variables that may help explain why those who have experienced sexual violence may be more likely to experience these negative outcomes. Previous research has demonstrated that body image dissatisfaction is a modifiable construct. If body image dissatisfaction is a modifiable construct that may reduce the likelihood of experiencing negative consequences associated with sexual violence then there is impetus for examining this construct as a potential target for treatment. The current study demonstrated that survivors of sexual violence endorsed higher levels of body image dissatisfaction– an association no identified study has thus far established. Furthermore, the mediational analyses demonstrated that for women experiences of sexual violence are significantly associated with greater body image dissatisfaction, which in turn, was predictive of increased depressive symptoms, anxiety, PTSD, and reduced sexual assertiveness. These results indicate that body image dissatisfaction may be a possible mechanism by which negative outcomes may be exacerbated. The current study suggests that there may be clinical implications in terms of prevention and treatment towards reducing body image dissatisfaction and consequently negative outcomes associated with sexual violence. Overall, the implications of the current study are encouraging as they may inform future research and provide the opportunity to empower assault survivors by reducing the deleterious effects of sexual violence.

Book excerpt: Depressive disorders affect 11% of the U.S. adolescent population, and suicide is the second leading cause of death among adolescents. The prevalence of sexually transmitted infections (STI) among U.S. adolescents ranges from 14-43.9%, and while rates of adolescent pregnancy have declined in the U.S. in the last decade, the U.S. rates are still high relative to other developed countries. There are currently no studies that examine the association between depression, suicidality, and risky sexual behavior (RSB) among adolescents. Using national data from the CDC's 2009 and 2011 Youth Risk Behavior Surveys (YRBS), our objectives were to: determine if adolescents who are depressed and suicidal are more likely to engage in risky sexual behavior than those without depression and those with depression without suicidal behaviors. Participants in this cross-sectional study were adolescent YRBS respondents in grades 9-12 who were sexually active (N=12,311). Depression was measured through a single question, ("During the past 12 months, did you ever feel so sad or hopeless almost every day for two weeks or more in a row that you stopped doing some usual activities"). Depression with suicidality was measured as a positive response to the question regarding depression, plus a positive response to one or more of the three questions about suicidality ("During the past 12 months, did you ever seriously [consider/plan/attempt] suicide"), and analyzed as a trichotomous variable. RSB was assessed using binary variables for each behavior: condom nonuse, birth control nonuse, drug/alcohol use at last sexual intercourse, or having had 4+ sexual partners. An additional dichotomous variable was created to examine the odds of engaging in any RSB. Logistic regression was conducted to determine the association between depression, suicidality, and RSB, and results were stratified by gender. Participants who were not sexually active (N=16,994) or missing information on depression/suicidality status or RSB (N=2,409) were excluded from analysis. Depressive symptoms were reported by 44.0% of sexually active girls and 25.5% of sexually active boys, and suicidal ideation was reported by 27.1% of girls and 18.0% of boys. Sixty-two percent of sexually active boys and girls engaged in at least one risky sexual behavior. Boys who were depressed and suicidal were 1.63 (95% CI: 1.22-2.18) times more likely to engage in risky behavior as those who were depressed only. They were 2.35 (95% CI: 1.81-3.05) times more likely to have not used a condom the last time they had sex, 2.39 (95% CI: 1.81-3.16) times more likely not to have used birth control, and 2.02 (95% CI: 1.36-2.49) times more likely to have used alcohol or drugs the last time they had sex, compared to boys who were depressed only. Girls who were depressed and suicidal were 1.49 (95% CI: 1.20-1.84) times more likely to engage in risky behavior as those who were depressed only. They were 1.39 (95% CI: 1.15-1.70) times more likely to have not used a condom the last time they had sex, 1.44 (95% CI: 1.17-1.76) times more likely not to have used birth control, and 1.42 (95% CI: 1.13-1.75) times more likely to have had 4+ sexual partners, compared to girls who were depressed only. Eighty-one percent of boys who reported binge-drinking reported engaging in at least one RSB, compared to 56.6% of boys who were not binge-drinkers, and 76.8% of girls who reported binge-drinking reported engaging in at least one RSB, compared to 57.0% of girls who were not binge-drinkers. Adolescents who are depressed and suicidal are at increased odds for engaging in RSB. Although causality cannot be established, involvement in any RSB is cause for concern and should be part of the clinical assessment of depression and suicidality among adolescent boys and girls.

Methods: We used data from a community sample of adolescents (n = 465) to describe differences between heterosexual and SMY in depression symptom counts assessed over the adolescent period and lifetime history of suicide attempts by late adolescence. We used negative binomial regression to adjust for relevant confounders of the association between sexual identity and depressive symptoms. We used logistic regression to investigate the association between sexual identity and lifetime odds of suicide attempt, while adjusting for relevant confounders. We conducted a stratified analysis to investigate the role bullying plays in depressive symptoms among heterosexual and SMY. We also examined whether maternal connection, social support, or emotion dysregulation modified the association between sexual identity and depressive symptomology. There is an implication that individuals who are bullied must have "asked for" this type of treatment, or deserved it. Sometimes, even the child who is bullied begins to internalize this idea. For many years, there has been a general acceptance and collective shrug when it comes to a child or adolescent with greater social capital or power pushing around a child perceived as subordinate. But bullying is not developmentally appropriate; it should not be considered a normal part of the typical social grouping that occurs throughout a child's life. Although bullying behavior endures through generations, the milieu is changing. Historically, bulling has occurred at school, the physical setting in which most of childhood is centered and the primary source for peer group formation. In recent years, however, the physical setting is not the only place bullying is occurring. Technology allows for an entirely new type of digital electronic aggression, cyberbullying, which takes place through chat rooms, instant messaging, social media, and other forms of digital electronic communication. Composition of peer groups, shifting demographics, changing societal norms, and modern technology are contextual factors that must be considered to understand and effectively react to bullying in the United States. Youth are embedded in multiple contexts and each of these contexts interacts with individual characteristics of youth in ways that either exacerbate or attenuate the association between these individual characteristics and bullying perpetration or victimization. Recognizing that bullying behavior is a major public health problem that demands the concerted and coordinated time and attention of parents, educators and school administrators, health care providers, policy makers, families, and others concerned with the care of children, this report evaluates the state of the science on biological and psychosocial consequences of peer victimization and the risk and protective factors that either increase or decrease peer victimization behavior and consequences.
