Book excerpt: Background: Public health nurses (PHN) work long hours, carry high caseloads, manage with fewer resources, and must deal with increasing technology while ensuring the provision of quality care. Since the time of Lillian Wald, public health nursing has undergone a major transformation in the types of services provided in the community due to the complexity and acuity of the current patient population. PHNs play a significant role in helping patients to effectively manage their illness in order to prevent unplanned hospitalizations. Illness, absenteeism, turnover, and poor job satisfaction are end products of stress. The purpose of this DNP project was to examine the impact of care coordination and caseload on job satisfaction and stress on the PHNs who are providing care to patients in the community. Methods: Descriptive study using mixed methods of data collection and analysis. A purposive sample of nurses who have had experience working in home care and who have provided skilled nursing care for a minimum of one year completed self-reported surveys. Perceived stress was measured with the Expanded Nursing Stress Scale. Job satisfaction was assessed using the Measurement of Job Satisfaction Tool. Analysis of the data collected from the surveys included descriptive and inferential statistics. Correlation coefficients, two-sample t-tests and content analysis were used to summarize the data and compare group differences between nurses with different caseloads. Results: However, due to a small sample size, the correlation and t test results showed inconsistency with content analysis. Content analysis provided more insight and represented more accurately the characteristics of a small sample size. Conclusion/ Clinical relevance: There exists a gap in the research regarding the impact of these factors on PHN and additional research is needed to explore the relationships between work conditions and PHN stress and impact on patient care. The results of this study support the idea that high caseloads impact job satisfaction and stress levels of nurses working in public health. Implementing changes to improve work conditions would provide PHNs with the time needed to focus on quality patient care which would ultimately lead to improved patient outcomes. In addition, the data collected from this project could influence change as it relates to the current model of care delivery in public health.

Through this approach, every qualified health and human service professional has a valued place setting at case management’s ever-expanding table. Each of the competencies are presented as mutually exclusive and uniquely defined however, all are complementary and call on the practitioner to conduct work processes in a wholly integrated manner. While appearing in order for the acronym’s sake, they are not necessarily sequential. Ultimately, case management is an iterative process. When united in a comprehensive and strategic effort, the COLLABORATE competencies comprise a purpose-driven, powerful case management paradigm. The agility of this model extends to use of key concepts that include both action-oriented verbs and nouns, which are significant elements in any professional case management endeavor. To date, case management practice models have been driven by care setting and/or business priorities. Unfortunately, this exclusivity has contributed to a lack of practice consistency due to shifting organizational and regulatory priorities. However, this is only one reason for a fragmented case management identity. COLLABORATE recognizes and leverages these important influencers as critical to successful practice and quality client outcomes. Herein lies an opportunity to demonstrate the value drawn from diverse expertise of case managers comprising the collective workforce. However a critical prefacing stage of this endeavor involves defining a core practice paradigm highlighting case management as a profession. The diverse and complex nature of population health mandates that case management intervene from an interprofessional and collaborative stance. While inherent value is derived from the variety of disciplines, this advanced model unifies case management’s unique identity. Now is the time to define and adopt a competence-based model for professional case management. COLLABORATE provides this framework. This text is presented in four sections: Section 1: Historical validation of why this practice paradigm is critical for case management to advance to a profession; Section 2: Presentation of the COLLABORATE paradigm, with a chapter to devoted to each distinct competency and the key elements; Section 3: Practical application of the book’s content for use by the individual case manager and at the organizational level; and The Epilogue: Summarizes the COLLABORATE approach in a forward-looking context. Work quality of health care workers has to be considered in an overall evaluation of quality of care. This work is based on a stress theory, and includes large studies of health care workers. A partly general questionnaire was used and continuously developed. The thesis identified predictors of healthy work and tried to identify supporting individual and organizational coping resources modifying the interaction between work environment, and health and well-being of staff. The thesis has pointed out the importance of individual and professional as well as organisatonal factors to understand the work-worker-health interaction among health care employees. Paper I more generally studied the work environment and health of a national sample Swedish registered nurses but also more specificly the importance of gender and medical specialty for perceived work environment, health and work relations. Paper II emphasized the importance of job satisfaction and job influence as key factors for perceived work environment. health and work relations of Swedish registered nurses. As criteria for work quality, predictors of job satisfaction were identified and used. Anxiety and depression, intellectual discretion, job demand, job control and general health were shown to be of relevance for work quality. Paper III. This paper studied preferences for important psychosocial workplace improvements, based on responses to an open-ended question from registered nurses and physicians. Work climate, organization, workload, and competence and development seemed to be the most important areas for improvements. Book excerpt: It is unknown whether the presence of care coordination promotes satisfaction of caregivers with communication among doctors and other health care providers of Children with Special Healthcare Needs (CSHCN), according to caregiver report. Furthermore, there is limited information in the literature concerning individuals identified as care coordinators for families of CSHCN (McAllister, Presler, & Cooley, 2007; Blumberg et al., 2008). Binary logistic regression analyses were conducted to identify these associations in a subset of families of CSHCN; those with children diagnosed with Autism Spectrum Disorders (ASDs), as reported in the National Survey of CSHCN (Blumberg et al., 2008; Bramlett et al., 2010). Findings indicate that when care coordination is present, caregivers of children with ASDs are nearly twice as likely to be satisfied with communication among health care providers. Families who fell at or above the national poverty level are also were also more likely to report satisfaction with this communication. These initial results account for a small portion of the variance included in caregiver satisfaction. Further research is needed to understand factors that may impact satisfaction, as well as elements of care coordination, specifically, the position of the care coordinator.
