Book excerpt: Parents of children diagnosed with an autism spectrum disorder face significant stressors and challenges; however, little research has investigated ways to effectively address their psychological distress and adjustment issues. This study used a between-subject and within-subject repeated measures design to test the effects of an 8-week Acceptance and Commitment Therapy (ACT) or treatment as usual (TAU) group. Treatment completers included 13 mothers in the ACT condition and 4 mothers in TAU. They were assessed three weeks before the intervention, one week after, and three months post-intervention. Limited data for between-group comparison demonstrated only a significant difference on the frequency scale of the Automatic Thoughts Questionnaire (ATQ), in which frequency of automatic thoughts increased for mothers in the TAU condition. For mothers in the ACT condition only, repeated measures ANOVAs revealed significant decreases from baseline to post-intervention on the Parental Distress Index of the Parental Stress Index-Short Form. Baseline to post-intervention decreases were seen for the GSI of the Brief Symptom Inventory-18 (BSI-18), with some regression to baseline at follow-up but overall reductions maintained. Similar significant findings were also demonstrated with increases in the Positive Aspects of Caregiving and decreases in the ATQ total score and the believability scale. No statistically significant changes were seen on the Depression Index of BSI-18, the Acceptance and Action Questionnaire II, or the Five Facets of Mindfulness Questionnaire. In exploratory analysis, experiential avoidance correlated positively with multiple scales of a self-administered measure of executive functioning, including a measure of one's ability to shift attention rapidly. Additionally, mothers who reported significantly greater levels of externalizing problem behaviors also reported significantly higher degrees of parental distress. This research suggests that an ACT-based treatment delivered in group format may be of assistance in helping parents better adjust to the difficulties in raising children with autism.

Book excerpt: Great national attention has been afforded to the development of effective treatment approaches for individuals with autism. However, a void lies in the support for the parents and guardians of individuals with autism. The psychological and emotional implications for a parent that raises a child with autism, from diagnosis to long-term care, are profound and have detrimental effects. Acceptance and Commitment Therapy (ACT) is an avenue of treatment that differs from traditional approaches. The aim of ACT is to modify the way in which the individuals interact with their language as opposed to changing that language and psychological processes that accompany it. Experiment 1 sought to examine the effects of a two-day, four-hour intensive ACT-based training utilizing the components of ACT vs. that of a Control Group. Participants were matched to the ACT Group or the Control Group, based on BDI-II and AAQ-II scores. In addition to the self-report measures, Galvanic Skin Response (GSR) was assessed at pre-scheduled times throughout the course of the experiment for participants that consented to provide it. Results of Experiment 1 were indicative of statistically significant differences with respect to a number of dependent measures both within and across subjects. Visual analysis of Galvanic Skin Response (GSR) measures between the two experimental groups denotes small-moderate physiological changes within those parents exposed to the ACT training. These results indicate the introduction of ACT training for parents served to impact psychological flexibility and decrease experiential avoidance, as well as improve physiological responding in the presence of aversive stimuli. In order to compare a mindfulness-based training approach to a full ACT model, we utilized a probe design imbedded in a non-concurrent additive multiple baseline design (A-B-BC) in Experiment 2 with three parents of individuals with autism. Training phases included the mindfulness processes of the ACT model (B) and the full ACT model (BC) involving both mindfulness and behavior-change processes. Similar to Experiment 1, parents provided GSR at various times over the course of Experiment 2. We saw moderate changes on self-report measures or GSR with the implementation of the Mindfulness Only Phase of the MBD, and in some cases, reports of psychological flexibility and experiential avoidance got worse. The introduction of the ACT Phase served to increase psychological flexibility and decrease experiential avoidance for 2 of 3 participants for each of the self-report measures. Additionally during the ACT Phase, participants' GSR output showed physiological improvements, increased psychological flexibility, and decreased experiential avoidance in the presence of aversive stimuli beyond those improvements of the Mindfulness Only Phase. Previous research has investigated the efficacy of an ACT-based training in which the total training contact hours fall in the double digits. Results of the current study showed that only 4 hours of ACT-based training served to facilitate psychological flexibility and decrease experiential avoidance, as well as positively impact physiological responses to aversive stimuli in most participants. Additionally, we saw a greater influence on psychological flexibility and experiential avoidance with the introduction of the full ACT model as compared to the Mindfulness-Only training, suggesting the importance of the inclusion of behavior-change processes (i.e., committed action and values) to a training for parents of individuals with autism.

It not only effects the individuals with the diagnosis but also the family. Parental stress is higher in parents of children with disabilities, including those with ASD. Therefore, the purpose of this study was to evaluate the efficacy of providing a brief Acceptance and Commitment Therapy (ACT) training to parents of children diagnosed with ASD via an online platform and evaluating subsequent changes in physiological and self-report stress measures. A non-concurrent multiple baseline was used across three experimental participants and two control participants. All three experimental participants experienced a decrease in heart rate between baseline and intervention phases. At the follow-up probe, two of the three experimental participants had a heart rate lower than their original resting heart rate. Control participants, who did not receive the ACT intervention, experienced either an increase in heart rate or no change during the baseline phase when compared to their original resting heart rate. This provided evidence for the efficacy of a brief online ACT training in lowering parental stress levels.

Book excerpt: Research consistently demonstrates parents of children diagnosed with autism spectrum disorder (ASD) experience more parental stress than parents with children in any other developmental category (Hayes & Watson, 2013). Previous research has begun investigating the use of ACT protocols as an intervention to increase psychological flexibility and reduce stress among this population (Hahs, Dixon, and Palilunas, 2018). Due to evidence provided in the literature demonstrating ACT as an effective intervention for parents of individuals with ASD, the current study conducted an evaluation of a brief ACT training in comparison with a traditional parent support group for this population. Participants of the study were twenty caregivers of an individual diagnosed with ASD. Each intervention consisted of two, two-hour sessions. Participants were provided with the Acceptance and Action Questionnaire-II and Parental Stress Scale pre and post intervention. Independent t-tests were ran in order to determine if mean change scores differed on the questionnaires across the ACT and TAU groups. Results of the independent t-test indicated that the difference in mean change scores of the AAQ-II across groups was statistically significant, however the difference in mean change scores of the PSS were not. Clinical implications are offered.
