Legal settlements with the companies held responsible led the federal government to ask the National Academy of Sciences to form and administer a 30-year program to enhance oil system safety, human health, and environmental resources in the Gulf of Mexico and other U.S. continental shelf areas where offshore oil and gas exploration and production occur or are under consideration. The new Gulf Research Program will receive $500 million to support activities using three broad approaches: research and development, education and training, and environmental monitoring. The Gulf Research Program: A Strategic Vision establishes the Program\'s foundation and introduces its mission, goals, and objectives. Opportunities for the Gulf Research Program: Community Resilience and Health summarizes a Gulf Research Program workshop held on September 22-23, 2014, in New Orleans, Louisiana. The workshop examined opportunities to improve the health, well-being, and resilience of communities in the Gulf region through discussions with about 50 participants with diverse expertise and experience. They are centers of research, conservation, education, and public outreach, often embedded in natural environments that range from remote to densely populated urban locations. Because they lack traditional university departmental boundaries, researchers at field stations have the opportunity to converge their science disciplines in ways that can change careers and entire fields of inquiry. Field stations provide physical space for immersive research, hands-on learning, and new collaborations that are otherwise hard to achieve in the everyday bustle of research and teaching lives on campus. But the separation from university campuses that allows creativity to flourish also creates challenges. Sometimes, field stations are viewed as remote outposts and are overlooked because they tend to be away from population centers and their home institutions. This view is exacerbated by the lack of empirical evidence that can be used to demonstrate their value to science and society. Enhancing the Value and Sustainability of Field Stations and Marine Laboratories in the 21st Century summarizes field stations' value to science, education, and outreach and evaluates their contributions to research, innovation, and education. This report suggests strategies to meet future research, education, outreach, infrastructure, funding, and logistical needs of field stations. Today's technologies - such as streaming data, remote sensing, robot-driven monitoring, automated DNA sequencing, and nanoparticle environmental sensors - provide means for field stations to retain their special connection to nature and still interact with the rest of the world in ways that can fuel breakthroughs in the environmental, physical, natural, and social sciences. The workshop gathered about 40 participants from the energy industry, state and federal government, academia, and nongovernmental organizations to examine two broad issues that were seen as time-sensitive opportunities in light of significant investments in the Gulf for restoration and accelerating development of energy resources in the deep Gulf: monitoring ecosystem restoration and deep water environments. As participants explored potential opportunities for the Program to consider, they noted the essential role that communication and outreach play in successful monitoring, and the importance of applying an ecosystem service approach to monitoring, forging partnerships among stakeholders, and supporting efforts to organize and manage monitoring data.

The Deepwater Horizon platform explosion and oil spill significantly damaged Gulf ecosystems and led to the largest ecological restoration investment in history. The unprecedented number and diversity of restoration activities provide valuable information for future restoration efforts, but assessment efforts are hampered by many factors, including the need to evaluate the interaction of multiple stressors and consider long-term environmental trends such as sea level rise, increasing hurricane intensity, and rising water temperatures. This report offers a comprehensive approach to assess restoration activities beyond the project scale in the face of a changing environment. A main component of this approach is using different types of scientific evidence to develop "multiple lines of evidence" to evaluate restoration efforts at regional scales and beyond, especially for projects that may be mutually reinforcing (synergistic) or in conflict (antagonistic). Because Gulf of Mexico ecosystems cross political boundaries, increased coordination and collaboration is needed, especially to develop standardized data collection, analysis, synthesis, and reporting. With these improvements, program-level adaptive management approaches can be used more effectively to assess restoration strategies against the backdrop of long-term environmental trends.

Book excerpt: Experiencing a single disaster - a hurricane, tornado, flood, severe winter storm, or a global pandemic - can wreak havoc on the lives and livelihoods of individuals, families, communities and entire regions. For many people who live in communities in the U.S. Gulf of Mexico region, the reality of disaster is starker. Endemic socioeconomic and health disparities have made many living in Gulf of Mexico communities particularly vulnerable to the effects of weather-climate hazards. Prolonged disaster recovery and increasing disaster risk is an enduring reality for many living in Gulf of Mexico communities. Between 2020 and 2021, seven major hurricanes and a severe winter storm affected communities across the region. As a backdrop to these acute weather events, the global COVID-19 pandemic was unfolding, producing a complex and unprecedented public health and socioeconomic crisis. Traditionally, the impacts of disasters are quantified individually and often in economic terms of property damage and loss. In this case, each of these major events occurring in the Gulf of Mexico during this time period subsequently earned the moniker of "billion-dollar" disaster. However, this characterization does not reflect the non-financial human toll and disparate effects caused by multiple disruptive events that increase underlying physical and social vulnerabilities, reduce adaptive capacities and ultimately make communities more sensitive to the effects of future disruptive events. This report explores the interconnections, impacts, and lessons learned of compounding disasters that impair resilience, response, and recovery efforts. While Compounding Disasters in Gulf Coast Communities, 2020-2021 focuses on the Gulf of Mexico region, its findings apply to any region that has similar vulnerabilities and that is frequently at risk for disasters.

Book excerpt: Social isolation and loneliness are serious yet underappreciated public health risks that affect a significant portion of the older adult population. Approximately one-quarter of community-dwelling Americans aged 65 and older are considered to be socially isolated, and a significant proportion of adults in the United States report feeling lonely. People who are 50 years of age or older are more likely to experience many of the risk factors that can cause or exacerbate social isolation or loneliness, such as living alone, the loss of family or friends, chronic illness, and sensory impairments. Over a life course, social isolation and loneliness may be episodic or chronic, depending upon an individual's circumstances and perceptions. A substantial body of evidence demonstrates that social isolation presents a major risk for premature mortality, comparable to other risk factors such as high blood pressure, smoking, or obesity. As older adults are particularly high-volume and high-frequency users of the health care system, there is an opportunity for health care professionals to identify, prevent, and mitigate the adverse health impacts of social isolation and loneliness in older adults. Social Isolation and Loneliness in Older Adults summarizes the evidence base and explores how social isolation and loneliness affect health and quality of life in adults aged 50 and older, particularly among low income, underserved, and vulnerable populations. This report makes recommendations specifically for clinical settings of health care to identify those who suffer the resultant negative health impacts of social isolation and loneliness and target interventions to improve their social conditions. Social Isolation and Loneliness in Older Adults considers clinical tools and methodologies, better education and training for the health care workforce, and dissemination and implementation that will be important for translating research into practice, especially as the evidence base for effective interventions continues to flourish.

Book excerpt: Since 2004, the U.S. government has supported the global response to HIV/AIDS through the President's Emergency Plan for AIDS Relief (PEPFAR). The Republic of Rwanda, a PEPFAR partner country since the initiative began, has made gains in its HIV response, including increased access to and coverage of antiretroviral therapy and decreased HIV prevalence. However, a persistent shortage in human resources for health (HRH) affects the health of people living with HIV and the entire Rwandan population. Recognizing HRH capabilities as a foundational challenge for the health system and the response to HIV, the Government of Rwanda worked with PEPFAR and other partners to develop a program to strengthen institutional capacity in health professional education and thereby increase the production of high-quality health workers. The Program was fully managed by the Government of Rwanda and was designed to run from 2011 through 2019. PEPFAR initiated funding in 2012. In 2015, PEPFAR adopted a new strategy focused on high-burden geographic areas and key populations, resulting in a reconfiguration of its HIV portfolio in Rwanda and a decision to cease funding the Program, which was determined no longer core to its programming strategy. The last disbursement for the Program from PEPFAR was in 2017. Evaluation of PEPFAR's Contribution (2012-2017) to Rwanda's Human Resources for Health Program describes PEPFAR-supported HRH activities in Rwanda in relation to programmatic priorities, outputs, and outcomes and examines, to the extent feasible, the impact on HRH and HIV-related outcomes. The HRH Program more than tripled the country's physician specialist workforce and produced major increases in the numbers and qualifications of nurses and midwives. Partnerships between U.S. institutions and the University of Rwanda introduced new programs, upgraded curricula, and improved the quality of teaching and training for health professionals. Growing the number, skills, and competencies of health workers contributed to direct and indirect improvements in the quality of HIV care. Based on the successes and challenges of the HRH program, the report recommends that future investments in health professional education be designed within a more comprehensive approach to human resources for health and institutional capacity building, which would strengthen the health system to meet both HIV-specific and more general health needs. The recommendations offer an aspirational framework to reimagine how partnerships are formed, how investments are made, and how the effects of those investments are documented.
