Book excerpt: During the course of operations at the Phillips Petroleum Houston Chemical Complex in Pasadena, Texas, on October 23, 1989, an explosion and ensuing fire occurred which resulted in 23 known dead and one missing. In addition, more than 100 other people were injured in varying degrees. Metal and concrete debris was found as far as six miles away following the explosion. Information available at the time of this report indicated that there was a failure in either a line or valve which carried ethylene and/or isobutene. The line was reported as being approximately 10 inches in diameter and possibly carrying as much as 700 pounds per square inch pressure. It is not known if a mechanical failure took place or if human error was a factor. Regardless, a failure of a high pressure line carrying these types of flammable products can create a large, enveloping, explosive cloud within seconds.

Litchfield, vice president of The Goodyear Tire & Rubber Company, purchased 16,000 acres west of Phoenix to grow long staple cotton for use in the pneumatic tires the company manufactured. With this, the Southwest Cotton Company was formed. This huge undertaking required drilling wells and building power lines, roads, canals, and, of course, housing for workers. The war years brought Goodyear Aircraft, built by the U.S. Department of Defense on land leased from Southwest Cotton Company, and Litchfield Naval Air Facility. With the arrival of Goodyear Aircraft and the navy base, homes, apartments, and basic retail services sprang up. The town of Goodyear was incorporated on November 19, 1946, with a population of 1,250. Named an All-America City in 2008, Goodyear is now a thriving community of 58,000 residents and the spring training home of Major League Baseball's Cleveland Indians and Cincinnati Reds.

Book excerpt: On January 30, 2007, a propane explosion at the Little General Store in Ghent, West Virginia, killed two emergency responders and two propane service technicians, and injured six others. The explosion leveled the store, destroyed a responding ambulance, and damaged other nearby vehicles. On the day of the incident, a junior propane service technician employed by Appalachian Heating was preparing to transfer liquid propane from an existing tank, owned by Ferrellgas, to a newly installed replacement tank. The existing tank was installed in 1994 directly next to the store's exterior back wall in violation of West Virginia and U.S. Occupational Safety and Health Administration regulations. When the technician removed a plug from the existing tank's liquid withdrawal valve, liquid propane unexpectedly released. For guidance, he called his supervisor, a lead technician, who was offsite delivering propane. During this time propane continued releasing, forming a vapor cloud behind the store. The tank's placement next to the exterior wall and beneath the open roof overhang provided a direct path for the propane to enter the store. About 15 minutes after the release began, the junior technician called 911. A captain from the Ghent Volunteer Fire Department subsequently arrived and ordered the business to close. Little General employees closed the store but remained inside. Additional emergency responders and the lead technician also arrived at the scene. Witnesses reported seeing two responders and the two technicians in the area of the tank, likely inside the propane vapor cloud, minutes before the explosion. Minutes after the emergency responders and lead technician arrived, the propane inside the building ignited. The resulting explosion killed the propane service technicians and two emergency responders who were near the tank. The blast also injured four store employees inside the building as well as two other emergency responders outside the store.

Explosions and fires killed 15 people and injured another 180, alarmed the community, and resulted in financial losses exceeding $1.5 billion. The incident occurred during the startup of an isomerization1 (ISOM) unit when a raffinate splitter tower2 was overfilled; pressure relief devices opened, resulting in a flammable liquid geyser from a blowdown stack that was not equipped with a flare. The release of flammables led to an explosion and fire. All of the fatalities occurred in or near office trailers located close to the blowdown drum. A shelter-in-place order was issued that required 43,000 people to remain indoors. Houses were damaged as far away as three-quarters of a mile from the refinery.
