The massive death toll and burden of disease worldwide caused by unsafe drinking water is a compelling reason to value the privilege of having safe drinking water delivered to individual homes. On rare occasions, that privilege has been undermined in affluent nations by waterborne disease outbreaks traced to the water supply. Using the rich and detailed perspectives offered by the evidence and reports from the Canadian public inquiries into the Walkerton (2000) and North Battleford (2001) outbreaks to develop templates for understanding their key dimensions, over 60 waterborne outbreaks from 15 affluent countries over the past 30 years are explored as individual case studies. Recurring themes and patterns are revealed and the critical human dimensions are highlighted suggesting insights for more effective and more individualized preventive strategies, personnel training, management, and regulatory control. Safe Drinking Water aims to raise understanding and awareness of those factors that have most commonly contributed to or caused drinking-water-transmitted disease outbreaks - essentially a case-history analysis within the multi-barrier framework. It contains detailed analysis of the failures underlying drinking-water-transmitted disease epidemics that have been documented in the open literature, by public inquiry, in investigation reports, in surveillance databases and other reliable information sources. Book excerpt: According to a 2012 report released by the World Health Organization (WHO) and the United Nations Children's Fund (UNICEF), roughly 780 million people around the world lack access to clean drinking water and an estimated 2.5 billion people (roughly 40% of the world's population) are without access to safe sanitation facilities. The United States has long supported efforts to improve global access to clean water, sanitation, and hygiene (WASH). In 2000, for example, the United States signed on to the Millennium Development Goals, one of which includes a target to halve the proportion of people without access to safe drinking water and basic sanitation by 2015. In 2002, the United States also participated in the 2002 World Summit on Sustainable Development, which emphasized the need to address limited access to clean water and sanitation among the world's poor. The 109th Congress enacted legislation to advance these global goals through the Senator Paul Simon Water for the Poor Act of 2005 [P.L. 109-121, (Water for the Poor Act)]. In March 2012, the U.S. Agency for International Development (USAID) announced that it had joined the Sanitation and Water for All partnership—a coalition of governments, donors, civil society and development groups committed to advancing sustainable access to clean drinking water and sanitation. Congressional support for the act was motivated, in part, by calls to augment funding for WASH programs and improve the integration of WASH activities into broader U.S. foreign aid objectives and programs, as well as global health efforts. The act called for USAID to bolster support for WASH programs, further synthesize WASH activities into global health programs, and contribute to global goals to halve the proportion of people without access to clean water and sanitation by 2015. In the 111th Congress, the Senator Paul Simon Water for the World Act of 2010 was introduced, but not enacted. That bill would have amended the Water for the Poor Act and addressed several concerns observers raised regarding the Water for the Poor Act, particularly by creating senior leadership within USAID to address water and sanitation issues, assessing U.S. water and sanitation programs, and strengthening reporting requirements. A new bill, introduced in the 112th Congress as the proposed Water for the World Act (S. 641), awaits action by the Senate Committee on Foreign Relations. Several agencies contribute to U.S. efforts to improve global access to clean drinking water and sanitation, of which programs implemented by the Millennium Challenge Corporation (MCC) and USAID make up roughly 90%. In FY2010, for example, the United States invested $953 million on water and sanitation programs worldwide, including $898 million provided by USAID and MCC. Appropriations for water projects are provided to USAID annually, while MCC receives multi-year funding for its country compacts that include support for water projects. As such, spending by MCC on water projects may vary significantly from year to year and may not be requested annually. The President requested $302 million for USAID's water activities for FY2012 and Congress appropriated not less than $315 million for international water and sanitation programs through the FY2012 Consolidated Appropriations. The FY2013 request for USAID's water and sanitation efforts was slightly lower at $299.1 million. This report addresses congressional efforts to address limited access to clean drinking water and sanitation, outlines related programs implemented by USAID and MCC, and analyzes issues related to U.S. and international drinking water and sanitation programs that the 112th Congress might consider.

Although many of the statements in the Declaration were vaguely worded, making it hard to measure progress or success, the Plan of Implementation of the Summit, agreed by the delegates to the conference, clearly stated that: "we agree to halve, by the year 2015, the proportion of people who are unable to reach or to afford safe drinking water and the proportion of people who do not have access to basic sanitation". Given the United Nations' predicted growth in global population from 6.1 billion in 2000 to 7.2 billion by 2015, this commitment will pose formidable challenges. To meet it, by the end of just a decade and half, approximately 6.6 billion people will need to have access to safe drinking water supplies. This is more than the current population of the world, and involves not only maintaining existing levels of supply but also providing new or upgraded services to 1.7 billion people. The challenge for sanitation is equally daunting: 5.8 billion people will need to be serviced, including new access provision for 2.1 billion. Even if these ambitious targets are met, representing a major achievement for the global community, there will still be approximately 650 million people in the world without access to safe drinking water and 1.4 billion without sanitation. What is clear is the magnitude of the problem facing the international community in terms of water supply and sanitation. Continuation of the status quo and the type of progress made during the 1990s will not permit the Johannesburg targets to be met. Instead it will be necessary to promote a combination of many different, new and innovative approaches, each of which will contribute towards the overall targets. These approaches must include technological advances that identify new sources and improve the quality of those already in use; managerial techniques that increase the efficiency and effectiveness of service delivery at both micro and macro scale; and fiscal approaches that tap into additional financial resources to make improvements affordable. In the past each of these aspects was seen as primarily the responsibility of government, which supported research into technology, managed supply and disposal systems and provided the funds to pay for them. This view has changed – beginning in the 1980s and increasing in the 1990s with growing moves towards privatisation of many aspects of the water sector. Underpinning this has been a shift away from seeing water as a public good that is essential for life, with subsidised supply provided as part of an overall welfare system, to a more market-oriented approach where the state, although still responsible for maintaining universal access to water services, uses market forces to meet this aim. The Business of Water and Sustainable Development aims to illustrate the range of approaches that will be necessary if the percentage of the global population having access to adequate and safe water and sanitation is to be increased in line with the brave assertions from Johannesburg World Summit on Sustainable Development. Some of approaches will be large-scale "Western-style" improvements involving the creation of new business models, their effectiveness assessed by traditional approaches of fiscal and social analysis. Such schemes may be instigated and partly funded by governments, but are increasingly turning to the private sector for money and expertise. In contrast, many smaller communities would be better served by following another path to improved water supply and sanitation. Because of their size, location or traditions they may achieve better results through the adoption of local small-scale solutions. Non-governmental organisations have been very active in this area, but to extend their operations many are seeking to adopt a more business-like model. All water supply and waste disposal agencies, large or small, need to support and encourage continued research into technological solutions that seek out better, more sustainable ways to use our increasingly scarce supplies of good-quality fresh water.

The Roundtable on Environmental Health Sciences, Research, and Medicine of the Institute of Medicine convened a workshop in October 2007, summarized in this volume, to address objectives related to Sustainable Water, Sanitation, and Hygiene Services. One of the objectives of the workshop was to think about the interdependence of environmental health and human health as connected through water. Organizations cannot discuss water without considering the interrelationship of sanitation and hygiene. It is the convergence of these strategies that promotes healthy outcomes for both individuals and the environment. A second objective of the workshop was to consider how planning, management, and interdisciplinary approaches-including technology, social behavioral issues, gender, health, environment, economic, and political aspects-can be integrated to arrive at sustainable solutions. Many organizations and agencies are trying to forge a path toward sustainable practices in water, but the various sectors utilizing and governing water services are not interconnected. More integration and a greater understanding of holistic approaches are needed.
