Book excerpt: This report looks extensively at changes in the activities of GPs from 1998 to 2008 in the light of numerous government initiatives and changes in the GP workforce and in the population. The report also raises some potential concerns about the costs from the continued rapid growth in orders for pathology testing and the overall challenge for the GP workforce in dealing with and ageing population with complex needs.

Book excerpt: Australia's Rural, Remote and Indigenous Health 3e is a practical guide to the delivery of health care in rural and remote Australia. Drawing on personal experiences of rural and remote practitioners, historical accounts, literature analysis and epidemiology, this frank and engaging text examines the economic, social and political forces that shape healthcare in rural and remote Australia. With limited current resources to support studies in rural and remote healthcare, this title bridges the gap by offering valuable insights into Indigenous life and social determinants of health through the use of storytelling. It is the perfect guide for anyone working in or planning to work in rural, remote or Indigenous Australia; and for those undertaking cultural studies, or social policy. Only text covering both rural and remote health in Australia Placement preparation with StudentConsult video interviews of experienced rural and remote practitioners providing first-hand experience of rural and remote practice Pause and Think questions provide a framework to apply learned knowledge to practice Pause and think question boxes train the reader to critically assess and apply concepts to practical situations. New Chapter: Working with Indigenous Australians - Written by Shannon Springer, describes how to consult with Indigenous patients and draws on his experience in clinical practice. It offers consultation signposts and practical principles for working with Indigenous Australians. New Chapter: Remote health practice - Written by Sue Lenthall, this chapter examines the differences and commonalities between the remote communities and then applies this information to a case study on 'a day in the life of a remote area nurse'. It presents a remote client consultation model and a StudentConsult audio consultation between a nurse and a distant doctor, as well as tips for working in remote practice. The audio consultation gives a first-hand account of the experience of a remote area nurse managing an emergency case. Video and audio content - video interviews of experienced rural, remote and Indigenous health practitioners as well as students. The videos contain first-hand experience of rural and remote practice as well as tips to help prepare those new to rural or remote practice.

Available in PDF, EPUB and Kindle. Book excerpt: This book explores dominance in Australia’s medical culture through the positioning of international medical graduates (IMGs). It argues that IMGs are ‘othered’ and ultimately positioned as an underclass, a positioning validated and reinforced by the intersecting inequalities of class, race and nation. It also suggests that the positioning of IMGs is organised through the dimensions of structural power, hegemonic power and interpersonal power, which allow an exploration of power relations between the structures of the health system, the Australian medical profession and the agency of IMGs. The Australian narrative presented to the world espouses a community of social justice and human rights. Instead, an historical lens traces the formation and persistence of difference represented in ethnocentrism, racism and xenophobia from 1788 to the present. The research presented is multidisciplinary in scope. An anti-oppressive theoretical framework enables the voices of lived experience to penetrate throughout and a social justice platform engages the participants and the reader into the interwoven conversations. The data set comprises a focus group, 10 individual interviews with IMGs and a selection of inquiry submissions revealing rich and sometimes shocking evidence to paint a stark picture. Other medical voices join the conversation via media responses to revelations of experiences not only by IMGs but also by Australian-trained doctors. It exposes a toxic culture endemic with bullying and sexual harassment. This book is of interest to practitioners, researchers and administrators in the fields of medical education, human resource management, legal studies, health sciences, social sciences, health services, government departments, universities and hospitals, as well as those tasked with duty of care and the provision of a safe workplace. The voices gifted to this study raise awareness of current issues within medicine in Australia at a very personal level and begin to formulate a policy and practical response to address these disturbing revelations.

Available in PDF, EPUB and Kindle. Book excerpt: The book provides a summary of results from the 14th year of the BEACH program, a continuing national study of general practice activity in Australia. From April 2011 to March 2012, 984 general practitioners recorded details about 98,400 GP-patient encounters, at which patients presented 153,218 reasons for encounter and 152,286 problems were managed. For an 'average' 100 problems managed, GPs recorded: 70 medications (including 57 prescribed, six supplied to the patient and seven advised for over-the-counter purchase); 11 procedures; 24 clinical treatments (advice and counselling); six referrals to specialists and three to allied health services; orders for 31 pathology tests and seven imaging tests. A subsample study of more than 31,000 patients suggests prevalence of the following measured risk factors in the attending adult (18 years and over) patient population: obesity - 27 per cent; overweight - 35 per cent; daily smoking - 15 per cent; at-risk alcohol consumption - 25 per cent. One in four people in the attending population had at least two of these risk factors. A companion publication, A Decade of Australian General Practice Activity 2001-02 to 2010-11 is also available.
