Available in PDF, EPUB and Kindle. Book excerpt: Adhesions can cause a wide range of problems, complaints and hazards, even after simple abdominal procedures, such as appendectomy, with complications ranging from recurrent discomfort and pain to intestinal obstruction. Postsurgical adhesions increase the risk of following operations of the abdominal and thoracic cavity. They impair peritoneal dialysis and chemotherapy and play a crucial part in laparoscopic procedures. Adhesion-related problems account for a large amount of clinical work and have a significant socioeconomic impact. This book presents the current knowledge on the aetiopathogenesis of adhesion formation as well as the available methods for their prevention and control. Experts in the field contribute to clinical standards for preventive measures to control the formation of postoperative adhesions

Available in PDF, EPUB and Kindle. Book excerpt: Recent years have seen important advances in the technology and techniques available to surgeons performing gynecologic surgery as well as reconstructive of clinical pelvic procedures. These developments took place in a wide variety settings from regional teaching centers to private clinical facilities. In 1996, the leading investigators from around the world gathered to discuss the present status of pelvic surgery and adhesion prevention with a look toward the future of patient care. This volume contains the proceedings of that meeting: the Third Interna tional Congress on Pelvic Surgery and Adhesion Prevention. Each chapter in cludes the material presented at the congress as well as a timely update of the authors' latest research and clinical thinking. Presentation integrating basic and clinical science provide the basis for con sidering peritoneal repair after surgery including the interaction of growth factors and other biochemical messengers. Research has increased the understanding of mesothelial reepithelialization and has led to new surgical technologies to reduce adhesion fonnation. A state-of-the-art review of emerging surgical adjuvants for adhesion prevention is provided, including discussion of barriers, gels, and poly mers as well as "designer" drugs effective at modifying the peritoneal response to injury. Assessment of clinical outcome in a wide variety of gynecologic surgical procedures brings into focus the benefits available as a result of these new tech nologies.

Available in PDF, EPUB and Kindle. Book excerpt: Peritoneal Surgery addresses the response of the peritoneum to injury and the prevention of post-surgical adhesions resulting from general and gynecologic surgery. Adhesions, or scar tissue binding two normally separate surfaces, form when the peritoneum, the membrane covering the abdominal wall and protecting the inner organs, is damaged during surgery, inflammation, or injury. Negative complications from adhesions include pelvic pain, infertility, intestinal obstruction, multiple surgical complications resulting from surgery to remove or pull them apart, and therefore a greater surgical workload and economic burden to the healthcare system. In this book, experts in the field address peritoneal repair, the role of surgical technique to prevent adhesions, adhesion formation, complications of adhesions, and developing technology in the prevention of adhesions.

Available in PDF, EPUB and Kindle. Book excerpt: ENTERING ON A CREATIVE AND ENTERPRISING PROJECT, DRS. DIZEREGA and Rodgers have taken an innovative look at the peritoneum. They have provided an interesting, informative, and stimulating text about an organ that is rarely considered independently-usually being thought of only as a part of other organs or organ systems. The peritoneum is an active membrane that serves as both a secretory organ and a structure that modulates diffusion and osmosis. Both of these important functions are described in great detail. The text is divided in classic fashion. The authors first examine the peritoneal anatomy from both macro and cellular viewpoints, during which exploration it becomes clear that what appears simply to be a lacy covering over abdominal organs actually is a complex structure. Fur thermore, during the discussion on its embryologic development the au thors make comprehensible the complexity confronting the student of the peritoneum. The authors then proceed to the practicalities associated with this im portant organ. To surgeons, for example, the key to the peritoneum is understanding the organ's repair mechanism, as it is adhesions formed on the peritoneal surfaces that interfere with the surgeon's hope of success.

Book excerpt: Primary and incisional ventral hernias are common conditions often encountered in surgical practice. Because of the frequency of this problem it has come to be managed by surgeons in general, regardless of the type of hospital or the conditions dealt with in their daily practice. Laparoscopic surgery has demonstrated to have an important role among the different technique described to repair ventral hernia with less recurrent rate, less morbidity and less overall cost than open conventional repair, with all the advange of the laparoscopic approach. As a result the indications for this surgical technique are currently being debated since the advantages are evident and progressive implementation is ensured. Now is the time to analyze the usefulness, results, technical variants, anatomic, physiologic and scientific basis and implications involved in implementation of laparoscopy as the technique of choice.

Microlaparoscopy is an essential instructional guidebook for any gynecologist or general surgeon wishing to perform laparoscopic surgery today in a minimally invasive and cost-effective manner. This important book is a detailed construction of all aspects of microlaparoscopy, including diagnostic and operative procedures under local anesthesia. By serving as an instructional tool. Microlaparoscopy is invaluable to clinicians who have not yet performed microlaparoscopy and a key reference to those who are experienced with the procedure. Microlaparoscopy includes: * The utility of minimally invasive techniques in conscious pain mapping * Evaluation and treatment of infertility and assisted reproductive technology * Historic concepts revisited to broaden the scope of understanding for the surgeon * The distinction between operations appropriate for office surgery and those of greater surgical complexity Microlaparoscopy presents meticulous detail on subjects ranging from basic equipment and instrument needs to analgesics, anesthetics, and safety precautions. Additionally, the author describes ways of providing compassionate patient care in pre-and post-op stages. An expert in the field and a pioneer of some of the techniques described. Book excerpt: "In the preface to this impressive and well-produced book, the editors state that their aim is not to describe a new surgical specialty, since most surgeons will soon need to be "geriatric surgeons," but to assemble a comprehensive account that will allow "all providers of healthcare to the elderly to understand the issues involved in choosing surgery as a treatment option for their patients." This is a useful book that deserves to do well. I hope that the editors and their publisher will have the stamina to make this the first of several editions, as it is clear that updated information about surgery in the elderly will be required to keep pace with this important field." This book was released on 2006-01-16 with total page 507 pages. Available in PDF, EPUB and Kindle. Book excerpt: The second SAGES (Society of American Gastrointestinal Endoscopic S- geons) manual was intended to be a companion piece for the successful ?rst SAGES manual, edited by Carol Scott-Connor, that was published more than 4 years ago. Originally, the goal was to concentrate on tersely covered or often ignored aspects of the preoperative preparation of the patient and the operating room as well as the postoperative care of patients undergoing minimally in- sive operations. It was also our intention to include a section for each procedure where several different port placement schemes would be presented and brie?y discussed. Unique to this manual, the impact of the patient’s body habitus (short or long, narrow or wide) on port placement is also taken into account for many of the procedures. Also unique are chapters devoted to hypothermia, port wound closure, and the management of subcutaneous emphysema and abdominal wall hemorrhage caused by trocars. Naturally, the surgeon tends to focus on the technical aspects of the pro- dure, such as the operative tasks to be carried out, the order of operation, and the position of the surgeon and assistant. However, it is critical that the surgeon be aware that the CO pneumoperitoneum, far more so than laparotomy, results 2 in multiple physiologic alterations that, if not compensated for by the anest- siologist and surgeon, may endanger the patient or prevent the laparoscopic c- pletion of the procedure.
