Book excerpt: Drug overdose, driven largely by overdose related to the use of opioids, is now the leading cause of unintentional injury death in the United States. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

Book excerpt: Revised and updated edition that analyses how the Office of National Drug Control Policy employs statistics to misleadingly claim the War on Drugs is a success. First published in 2007, Lies, Damned Lies, and Drug War Statistics critically analyzed claims made by the Office of National Drug Control Policy (ONDCP), the White House agency of accountability in the nation’s drug war since 1989, as found in the six editions of the annual National Drug Control Strategy between 2000 and 2005. In this revised and updated second edition of their critically acclaimed work, Matthew B. Robinson and Renee G. Scherlen examine seven more recent editions (2006–2012) to once again determine if ONDCP accurately and honestly presents information or intentionally distorts evidence to justify continuing the drug war. They uncover the many ways in which ONDCP manipulates statistics and visually presents that information to the public. Their analysis demonstrates a drug war that consistently fails to reduce drug use, drug fatalities, or illnesses associated with drug use; fails to provide treatment for drug-dependent users; and drives up the prices of drugs. They conclude with policy recommendations for reforming ONDCP’s use of statistics, as well as how the nation fights the war on drugs. Praise for the First Edition “Lies, Damned Lies, and Drug War Statistics is surprisingly easy to read, and Robinson and Scherlen have done a huge favor not only to critics of current drug policy by compiling this damning critique of ONDCP claims, but also to anyone interested in how data is compiled, presented, and misused by bureaucrats attempting to guard their domains. It should be required reading for members of Congress.” — Drug War Chronicle Book Review “The authors have performed a valuable service to our democracy with their meticulous analysis of the White House ONDCP public statements and reports. They have pulled the sheet off what appears to be an official policy of deception using clever and sometimes clumsy attempts at statistical manipulation. This document, at last, gives us a map of the truth.” — Mike Gray, author of Drug Crazy: How We Got into This Mess and How We Can Get Out “Robinson and Scherlen make a valuable contribution to documenting how ONDCP fails to live up to basic standards of accountability and consistency.” — Ethan Nadelmann, Executive Director, Drug Policy Alliance

Book excerpt: Like most former Soviet republics, Ukraine has experienced a formidable proliferation of crime and corruption as it struggles with economic reform and the establishment of democracy.During the early 90s, Ukraine became one of the primary recipients of foreign assistance from the United States and its crime and corruption situation was increasingly seen as an impediment to economic transition and achieving a more democratic way of life. Thus in 1998, as part of a larger U.S. law enforcement assistance effort in Ukraine, the idea for a research partnership between criminologists and legal scholars in the two countries was born in this volume. The original research papers contained are the products of this ambitious research project. The realities of crime in post-Soviet Ukraine, as well as divergent methodological approaches and communication problems, presented the research partners with enormous challenges. This volume represents the culmination of that collaborative effort, and provides a singular look into the current crime situation in Ukraine, and into the potential global threat presented by Ukrainian organized crime.Contributions include analyses of the prediction and control of organized crime, trafficking in women and children for sexual exploitation, international money laundering, the transnational political criminal nexus of trafficking in women, countermeasures against economic crime and corruption, heroin trafficking, business victimization by organized crime, and understanding and combating organized crime. The Prediction and Control of Organized Crime will be critical reading for security planners, policymakers, and criminal justice officials, as well as comparative criminologists, legal scholars, and political scientists interested in organized crime and political corruption.

Book excerpt: The opioid crisis in the United States has come about because of excessive use of these drugs for both legal and illicit purposes and unprecedented levels of consequent opioid use disorder (OUD). More than 2 million people in the United States are estimated to have OUD, which is caused by prolonged use of prescription opioids, heroin, or other illicit opioids. OUD is a life-threatening condition associated with a 20-fold greater risk of early death due to overdose, infectious diseases, trauma, and suicide. Mortality related to OUD continues to escalate as this public health crisis gathers momentum across the country, with opioid overdoses killing more than 47,000 people in 2017 in the United States. Efforts to date have made no real headway in stemming this crisis, in large part because tools that already existâ€"like evidence-based medicationsâ€"are not being deployed to maximum impact. To support the dissemination of accurate patient-focused information about treatments for addiction, and to help provide scientific solutions to the current opioid crisis, this report studies the evidence base on medication assisted treatment (MAT) for OUD. It examines available evidence on the range of parameters and circumstances in which MAT can be effectively delivered and identifies additional research needed.

Book excerpt: Mental, neurological, and substance use disorders are common, highly disabling, and associated with significant premature mortality. The impact of these disorders on the social and economic well-being of individuals, families, and societies is large, growing, and underestimated. Despite this burden, these disorders have been systematically neglected, particularly in low- and middle-income countries, with pitifully small contributions to scaling up cost-effective prevention and treatment strategies. Systematically compiling the substantial existing knowledge to address this inequity is the central goal of this volume. This evidence-base can help policy makers in resource-constrained settings as they prioritize programs and interventions to address these disorders.

Available in PDF, EPUB and Kindle. Book excerpt: Ever since the Shanghai convention in 1909, the threat posed to human well-being by drug abuse has led countries around the world to take action to deal with their drug problems. There are wide variations in the policies pursued, but most countries try to reduce both the supply of and the demand for drugs. Unfortunately, there is little research consensus on the respective merits of these two approaches or about the best ways to pursue them. Consequently, control and prevention policies are mostly driven by political considerations, economic realities and cultural expectations, though research has played an important part in formulating and evaluating treatments for drug addiction. This volume reviews studies on drug abuse prevention and treatment strategies under five main areas: 1. Reducing supply - strategies to control the flow of drugs from production to retail distribution; 2. Reducing demand - prevention of drug use at all stages of involvement and consumption levels; 3. Reducing harm - promoting situational risk reduction practices for regular users, addicts and recreational users; 4. Reducing addiction - drug treatment options for various groups in various settings; and 5. Drug policies and prescriptions - focused on debates about prohibition and legalization.

This publication examines the realities faced by urban populations around the world, focusing on the impact of globalisation and the way cities are governed and planned, on the make-up and density of their population, and on their cultures and economies. Issues considered include: the impact of globalisation on urban culture; urban renewal and cultural strategies; the concept of metropolitanization; socio-economic and cultural impacts of international migration; urban poverty and homelessness, social inequality and exclusion; urban governance, safety and crime trends; contemporary planning strategies and the role of civil society; progress towards attainment of the Millennium Development Goals targets for sanitation and housing. The report highlights the need for a new culture of planning to establish multicultural and inclusive cities, involving civil society as well as public authorities.
