Why haven't these marvels excited me before? I believe that the use of schemata and drawings or photographs of animal embryos, commonly used in textbooks of embryology, simply did not appeal to me as a clinician. Only actual photo graphs of human embryos can establish the bond necessary for interaction. Just imagine the excitement when you find out how many struc tures you can recognize in a 5-week-old embryo, barely measuring 1 cm in length. But our fascination does not stop here. The pro gression of changes taking place during the next 3 weeks is so rapid that at the time when the embryo measures 3 cm, all structures familiar to us are not only easily recognizable, but also already in their anatomical position. How can we hide our amazement when we realize that such a state of perfection is present in an embryo a bit longer than the distal phalanx of our little finger? At 8 weeks the embryonic period ends and the fetal one starts. Although the shape and the relative size of bones, joints, muscles, nerves, and vessels will undergo changes, the basic elements are all in place. This implies that major malformations must develop during the embryonic period.

Book excerpt: Our common interest in surgery of the vertebral artery was born in 1976, when as residents in the same hospital, we attended an attempt by two senior surgeons to treat an aneurysm of the vertebral artery at the C 3 level. Long discussions had preceded this unsuccessful trial, to decide if surgery was indicated and to choose the surgical route. Finally a direct lateral approach was performed, but access was difficult and correct treatment was impossible, resulting in only partial reduction of the aneurysmal pouch. Following this experience, we decided to seek a regular and well defined approach for exposition of the vertebral artery. Review of the literature indicated some surgical attempts, but the descriptions did not give the impression of safety and reproducibility. No landmark on the described surgical route appeared sufficiently reliable. Henry's anatomical work (1917) gave the only accurate description on vertebral artery anatomy, and it became the basis for our work. When the same patient was referred again one year later, after a new stroke in the vertebro-basilar system, we had behind us repetitive experience on cadavers of an original approach to the distal vertebral artery.

In recent years, comprehension of the molecular genetics of blood vessel formation has progressed enormously and studies in vertebrate model systems, especially the mouse and the zebrafish, have identified a common set of molecules and processes that are conserved throughout vertebrate embryogenesis while, in addition, highlighting aspects that may differ between different animal groups. The discovery in the past decade of the crucial role of new blood vessel formation for the development of cancers has generated great interest in angiogenesis (the formation of new blood vessels from pre-existing ones), with its major implications for potential cancer-control strategies. In addition, there are numerous situations where therapeutic treatments either require or would be assisted by vasculogenesis (the de novo formation of blood vessels). In particular, post-stroke therapies could include treatments that stimulate neovascularization of the affected tissues. The development of such treatments, however, requires thoroughly understanding the developmental properties of endothelial cells and the basic biology of blood vessel formation. While there are many books on angiogenesis, this unique book focuses on exactly this basic biology and explores blood vessel formation in connection with tissue development in a range of animal models. It includes detailed discussions of relevant cell biology, genetics and embryogenesis of blood vessel formation and presents insights into the cross-talk between developing blood vessels and other tissues.
