Book excerpt: The purpose of this investigation was to evaluate the effects of air-powder polishing on shear bond strength in vitro for two metal bracket adhesive systems currently in use for direct orthodontic bonding procedures. Ninety-six third molar teeth with flattened enamel surfaces were randomly assigned to be bonded with metal brackets using either a two paste or a no-mix orthodontic adhesive resin. Thwelve samples in each treatment group were air-powder polished for 0, 15, 30, or 60 seconds. Each bracket adhesive bond was then tested for shear strength using the Instron Universal Testing Device. Scanning electron microscopy determined the fracture pattern of representative samples from each treatment group. Mean shear bond strength values for each treatment group were calculated and expressed in magapascals (MPa). Values from baseline to 60 seconds varied from 22.9 ± 1.9 MPa to 18.2 ± 4.1 MPa for the two paste system and from 15.5 ± 2.1 MPa to 14.6 ± 1.9 MPa for the no-mix adhesive. Differences between the adhesive systems at all air-powder polishing times were statistically significant (p d".05). Statistical analysis by a two-way ANOVA in combination with the Newman-Keuls Multiple Range Test for pairwise mean comparisons determined a differential effect of air-powder polishing on the bond strength of the two adhesive systems. After 60 seconds of air-powder polishing, results showed a significant decrease (p d".05) in the mean bond strength of the two paste adhesive resin when compared to baseline, 15, and 30 second treatments. No significant time effect of air-powder polishing was found for the no-mix adhesive system. SEM analysis of the bond failure sites showed tooth-adhesive, within adhesive and bracket-adhesive fracture patterns for the viewed samples of each adhesive type and air-powder polishing treatment time. Fractures within tooth structure were also noted for the two paste system, indicating strong enamel-adhesive-bracket bonds. Differences in the fracture pattern of the two paste 60 second air-powder polishing group may account for the differential effect of treatment. Although in vitro results showed decreased bond strength for the two paste system, use of air-powder polishing for the routine maintenance of the orthodontic patient should not affect clinical performance of this adhesive.

Book excerpt: Objective: To compare the mean bond strengths and mode of bond failure, in vitro, of five bonding systems (MIP 1, Plastic Conditioner 2, Assure 2, Scotchbond 3, and Transbond XT 1), when bonding an orthodontic bracket to an artificially-aged composite resin restoration, with and without mechanical surface preparation with a diamond bur. Methods: Class V buccal composite resin restorations were prepared in 240 upper right central incisor dentoform teeth. The restorations were artificially age d for 35 days, bonded with metal brackets, stored in distilled water at 37°C for 30 days, thermocycled for 500 cycles, and subsequently debonded with an Instron universal testing machine. Results: The mean bond strengths for Transbond, MIP, Plastic Conditioner, Assure, and Scotchbond groups were 12.1, 12.3, 13.3, 17.2, and 17.7 MPa respectively. The mean bond strengths for Transbond+Diamond, MIP+Diamond, Plastic Conditioner+Diamond, Assure+Diamond, and Scotchbond+Diamond groups were 18.5, 16.4, 19.1, 19.5, and 20.7 MPa respectively. ANOVA revealed a statistically significant diffe rence (P d"0.05) among the groups. Conclusions: Mechanically roughening the surface of a composite resin restoration with a diamond bur, provided significantly greater bond strengths, regardless of the bonding resin used. However, Assure and Scotchbond, without diamond bur preparation, provided similar bond strengths to Transbond, MIP and Plastic Conditioner, with diamond bur preparation. 1. 3M Unitek, Monrovia CA 2. Their main advantage is that they reduce the number of steps required in the bonding of orthodontic brackets. However, their low ph was a cause of concern. This book describes a study undertaken to see the effects of ph of self-etchant on shear bond strength of orthodontic brackets. These shear bond strengths were also compared to those obtained with a conventional 3-step bonding system. It was found that self-etching primers not only reduce the chair time in an orthodontist's clinic but also reduce the amount of enamel lost during etching. The bond strength achieved was less than that of the conventional bonding system but was acceptable for orthodontic purposes. Thus self-etching primers are an important and useful invention for simplifying the most critical step in orthodontic clinic- bonding.

Book excerpt: The purpose of this study was to compare the bond strengths of three adhesive systems (two-paste, no-mix, and one light-activated) when used alone and combined with a porcelain priming agent to bond ceramic brackets to porcelain surfaces. Sixty porcelain specimens were randomly assigned to the six different treatment groups. One half of the groups were bonded with the porcelain priming agent and one of the adhesive systems and the other half with one of the adhesive systems without the priming agent. The shear bond strengths, expressed in megapascals (MPa), of all specimens were tested, using an Instron testing machine, 10 minutes after being bonded. The surface of the porcelain and the bracket base were examined, using the scanning electron microscope (SEM), to determine the bond failure patterns and check the prcelain surface for the presence of cracks and fractures. Across the three different adhesive systems, mean shear bond strength measurements ranged from 6.3-7.5 MPa when the porcelain primer was used and from 1.8-3.8 when the porcelain primer was not used. There was a statistically significant difference within each adhesive between those samples with the priming agent (mean shear bond strength of 6.5 ± 0.8, 7.5 ± 1.0, and 6.3 ± 1.2) and those without the priming agent (mean shear bond strength of 3.8 ± 0.4, 1.8 ± 1.8, nad 2.6 ± 1.3) (p ≤ 0.5). Differences between the three adhesives were not statistically significant (p ≤ .05). Evaluation of the bond failure sites, using the SEM, revealed failures at the porcelain-adhesive interface, within the adhesive and at the bracket-adhseive interface for the porcelain primer samples. The samples that were bonded with the different adhesives only displayed a failure exclusively at the porcelain-adhesive interface. The failure patterns were verified by qualitative energy dispersive x-ray analysis (EDS). It can be concluded from this study that using any of the three adhesive systems combined with the porcelain priming agent will provide sufficient bond strength to withstand normal orthodontic forces and not damage the surface of the porcelain when debonded.

Book excerpt: The evolution of orthodontic materials in the past 5 decades has been a unique progress curve characterized by periods of intense activity with many developments followed by long quiescent intervals. An example of the delay in adopting the advances in dentistry might well be illustrated by the acid-etching technique, which was introduced in the mid 1950s1 and became universally accepted in the 1960s. It took more than a decade for this practice to become standard procedure in orthodontics on a large scale. Infact, orthodontic bonding has advanced more in last 10 years than previous 25. In the past decade, we have witnessed unprecedented progress in bonding to enamel with various materials techniques and auxiliaries involving glass ionomer bonding, self-etching primers, and new, powerful light-curing sources, along with self-ligating brackets and a different treatment concept, the Invisalign technique.

Book excerpt: Introduction: In an attempt to save chair time during bonding, self-etching primers are used along with metal brackets that are precoated with adhesive material. Fluoride releasing sealants has also been used commonly to prevent decalcification during orthodontic treatment. The purpose of this study was to compare the shear bond strength (SBS) of APC precoated brackets bonded with two different self-etching primers: Transbond Plus self-etch primer (TPSEP) and Reliance self-etch primer. This study also evaluated the effect of combining TPSEP or the Reliance SEP with LED Pro Seal, a fluoride releasing sealant for the enamel surface. Each of the self-etching primers (SEPs) and the combination of SEP combined LED Pro Seal were evaluated for shear bond strength after 24 hours of storage and thermocycling with 1 week of storage, in order to simulate some of the temperature changes in the oral cavity.^Materials and Methods: One hundred and twelve teeth were randomly divided into eight groups as follows: Group 1: Transbond Plus self-etch primer (TPSEP), Group 2: Reliance self-etch primer (Reliance), Group 3: TPSEP and LED Pro Seal, Group 4: Reliance and LED Pro Seal, Group 5: TPSEP with thermocycling, Group 6: Reliance with thermocycling, Group 7: TPSEP and LED Pro Seal with thermocycling, Group 8: Reliance and LED Pro Seal with thermocycling. An occlusogingival load was applied to the brackets producing a shear force at the bracket-tooth interface utilizing a Universal Testing Machine. Results: Significant differences in shear bond strengths among the groups containing the TPSEP or Reliance SEP, were detected by two-way ANOVA (p = 0.018). The results of the Newman-Keul's test that compared the test groups at 24 hours show that the shear bond strength of Reliance SEP (3.98 MPa " 1.30) was not statistically different from TPSEP (4.25 MPa " 1.16).^When each of the SEP is combined with the Pro Seal, a statistical difference in shear bond strength is detected between TPSEP (6.01 MPa " 3.05) and Reliance SEP (3.76 MPa " 1.52). Reliance SEP used alone vs. Reliance SEP combined with Pro Seal did not show any statistical difference in shear bond strength. However, when TPSEP was compared to TPSEP combined with Pro Seal, it showed a statistical difference in shear bond strength. Nonetheless, a two-way ANOVA did not detect any statistically significant interaction between the SEPs and the Pro Seal (p = 0.058). None of the groups showed statistically significant differences in shear bond strength when compared at the one week group. Conclusion: The newly introduced Reliance SEP provides similar shear bond strengths to Transbond Plus SEP. Reliance SEP with LED Pro Seal and Transbond Plus SEP with LED Pro Seal both provide adequate shear bond strengths.^Both SEPs used alone or with LED Pro Seal maintained its bond strength after thermocycling. LED Pro Seal is demonstrated to have good bond strength to enamel and can withstand the oral environment.

Book excerpt: Background: Light cured composite resin can be time consuming for bonding orthodontic brackets. A new Blue light diode laser may reduce curing times for composite resins. Hypothesis: The laser will achieve similar bond strengths in half the time and at greater distances as the LED. Methods: 90 extracted premolars will be bonded and cured using Laser and LED curing units at varying times, another 120 extracted premolars will be bonded and cured at varying distances. An Instron machine will be used to test shear bond strength. Results: The laser was not able to achieve similar bond strengths as the LED curing light in half of the curing time. At 5 seconds, the laser had 35% lower bond strengths but was still in the clinically acceptable range. The laser achieved similar bond strengths when the curing tip was at greater distances as when it was close to the bracket-adhesive-tooth interface.
