Pregnancy may be one of the causes of this, with 56% of women of childbearing age being overweight or obese. Excessive weight gain during pregnancy and postpartum weight retention may increase a woman's risk of obesity and chronic disease later in life. Moderate calorie restriction and exercise interventions have been shown to reduce body weight and improve body composition during the postpartum period. While weight loss interventions have been successful, high attrition rates limit the widespread effectiveness of these interventions. Furthermore, there is a lack of research examining the effects of a weight loss program on cardiometabolic risk factors in this population. Therefore, the primary aim of the studies in this dissertation were: 1) To determine the effect of a diet and exercise intervention on weight, body composition, and cardiometabolic risk factors, 2) to improve lifestyle behaviors through improved diet quality and cardiovascular fitness, and 3) to describe the relationship between chronic inflammation and bone mineral density (BMD) in overweight and obese postpartum women. The first study concluded that a home-based diet and exercise program resulted in greater reductions in weight, waist circumference, sagittal diameter, and abdominal fat mass compared to a control group. Additionally, the intervention resulted in a significant improvement in cardiovascular fitness. The second study concluded that the intervention improved triglyceride concentrations, decreased insulin resistance, and improved metabolic syndrome risk factors. Finally, the third study concluded that the intervention resulted in less loss of BMD at the total body and hip and less loss of BMC at the hip, lumbar spine, and femoral neck after controlling for lactation status and weeks postpartum at baseline. Change in cardiovascular fitness was a predictor of change in total hip and femoral neck BMD, while change in inflammation was a predictor of change in total body BMD. These results suggest that a home-based, diet and exercise intervention is effective in improving body composition, cardiovascular fitness, and some cardiometabolic risk factors in overweight and obese postpartum women. Exercise, combined with a modest restriction in energy intake, has been shown in non-lactating women to improve body composition and protect bone during periods of weight loss. However, there is a paucity of research on exercise and dietary interventions in lactating women aimed at promotion of bone health and weight loss. Therefore, the primary objectives of the two studies in this dissertation were: 1) to describe the changes in dietary intake, bone turnover markers, and hormones related to lactation and exercise, as well as predict changes in bone mineral density (BMD) during lactation in women whom had undertaken resistance exercise, and 2) to promote lifestyle changes through a home-based nutrition and exercise intervention targeting overweight lactating postpartum women. The first study concluded exercise and a higher dietary calcium intake were inversely related to bone loss during lactation, when controlling for parity. Additionally, an increase in bone formation markers predicted an increase of lumbar spine BMD, whereas increased levels of estradiol and IGF-1 were positively related to total body BMD. The second study results suggest moderate energy restriction and resistance training are safe methods for weight loss in overweight fully breastfeeding women with no adverse affects on BMD and infant growth. Additionally, the use of MyPyramid Menu Planner appeared to be a novel tool to aid in weekly individual dietary counseling and weight loss. This dissertation research was the first to examine the effects of an exercise and weight loss intervention on attenuation of lactation-induced bone loss in overweight women. A total of 10 overweight/obese women participated in an employee wellness weight loss program two times per week for eight weeks. At baseline and last week of the program all participants underwent measurements for physical activity, body mass index (BMI), body fat, circumferences, and VO2max. There were no significant changes in BMI, body composition and VO2max following this 8-week program. It is possible that this is due to either a decrease in spontaneous daily physical activity or increased caloric intake as a result of participation in this program.

It is not well established whether antenatal exercise can improve these outcomes. Hypothesis We hypothesized that antenatal exercise in overweight and obese women could improve offspring and maternal health and metabolic milieu, and protect offspring from adverse programming effects. Methods/Design: A parallel two-arm randomised controlled trial was conducted in healthy non-smoking overweight and obese women (BMI≥25kg/m2) with a singleton pregnancy. Participants were randomised at 20 weeks of gestation to an intervention group (who underwent a home-based, moderate-intensity stationary cycling programme) or a control group (no exercise intervention). The primary trial outcome was birth weight and secondary outcomes included perinatal complications, neonatal body composition, maternal physical health (weight gain, fitness and body composition), quality of life, and pregnancy and delivery complications. Maternal and offspring (cord) blood metabolic markers were also measured. Primary analysis was based on intention-to-treat, using analysis of covariance regression models to evaluate differences between intervention and control groups, adjusting for pre-specified covariates. Results Seventy-five enrolled participants were randomised to intervention (n=38) or control (n=37) groups. Offspring birth weight was similar between groups (adjusted mean difference 104 grams, 95% CI -116 to 324, p=0.35). Maternal aerobic fitness improved following the intervention but there were no differences in maternal weight gain, quality of life, pregnancy outcomes, or postnatal maternal body composition between groups. Exercise compliance ranged from 0-85 % (mean 33%). Offspring neonatal adiposity and perinatal outcomes were similar. There was an increase in bone mineral content in offspring in the intervention group, and male offspring had increased adiposity compared to gender-matched controls. There were no differences in maternal metabolic markers between groups. Offspring in the intervention group had lower cord blood interleukin-6 levels, and male offspring also had reduced insulin-like growth factor binding protein-1 levels, while other markers were similar between groups. Conclusions: These study finding suggest that non-weight-bearing antenatal exercise does not appear to improve short term maternal and offspring outcomes. However, improvement in maternal fitness and differences in offspring body composition and metabolic markers indicate the potential for long-term health effects. This needs to be established by further research.

Resistance training interventions have been conducted with normal weight pregnant women and they have been deemed safe and beneficial, therefore this intervention should be applied to the overweight/obese population. The purpose of this study is to determine the effect of resistance exercises during pregnancy on infant body composition. METHODS: Participants were randomized into two groups, resistance and non-exercising control, and trained from 16 weeks gestational age until delivery. The resistance group participated in three 50-55-minute sessions per week targeting all major muscle groups. After birth, one-month body composition measures consisted of BMI, skinfold and circumference measurements, and magnetic resonance imaging (MRI). Independent t-test were performed to determine differences between groups and like-genders. Correlation looked for relationships between infant variables and average METS, gestational weight gain (GWG), and pre-pregnancy BMI. Multiple linear regression was performed controlling for maternal BMI and infant gender; another linear regression was performed controlling for GWG, infant gender, and average METs. RESULTS: We analyzed data from women randomized to resistance exercise (n=7) or usual care controls (n=7). There was a 3.6% increase in lean mass volume for the infants of resistance exercisers compared to controls. Trends of lower subcutaneous fat volume, visceral/retro fat volume, and total fat volume were found in infants of resistance exercisers versus controls. Correlation measures conveyed a moderate positive relationship between GWG and one-month weight (p=0.09). The multiple linear regression measure determined infant gender is a significant predictor for lean mass volume (adj. R2=0.21; p=0.03) and one-month weight (adj. R2= 0.18; 0.04). CONCLUSION: The trends of higher lean mass volume with lower subcutaneous fat volume, visceral fat volume, and total fat volume in infants of resistance exercisers compared to infants of controls is promising for future investigators. Determining the direct response of resistance exercise on infant body composition from only overweight/obese mothers should be conducted to help provide more answers.

Because exercise increases mobilization and utilization of fatty acids, maternal activity may also influence the amount of LCPUFA in breast milk. In addition, exercise has been shown to improve bone mineral density (BMD) in postmenopausal women. During lactation, bone turnover is elevated to provide calcium for breast milk. This results in decreased BMD. Therefore, the purpose of these studies was to: 1) investigate the effects of diet and exercise on fatty acid concentrations in maternal plasma and breast milk, and 2) determine the effects of a specific resistance and aerobic training program and dietary intake on BMD and bone turnover during lactation. In the first study, LCPUFA in plasma and breast milk were measured at 12 weeks postpartum in exercising and sedentary women. Dietary intake was recorded for three days. A subsample of women participated in exercise and rest sessions to examine the acute effects of exercise on breast milk LCPUFA. In the second study, whole body, lumbar spine (LS) and hip BMD and bone mineral content (BMC), and bone turnover markers were measured before and after a 16-wk intervention in breastfeeding women who either participated in a specific resistance and aerobic training program or were sedentary. Dietary intake and body composition were also assessed in all women. In the first study, there were no differences in essential fatty acid intake between the two groups.
