Book excerpt: In this dissertation, the impact of program generosity on per enrollee Medicaid spending by state is examined through a combination of descriptive and econometric analyses. The dissertation employs a unique dataset that compiles a variety of Medicaid program data, including per enrollee spending and Medicaid program characteristics, covering the period 2002-2004. Generosity indicators are constructed to quantitatively measure differences in program generosity across three dimensions: optional benefits, eligibility rules, and physician fees. These indicators are the variables of interest in the econometric analysis of per enrollee Medicaid spending, while additionally controlling for relevant economic, demographic, health supply, ideology, and policy variables.

Book excerpt: Although economic theory suggests that the federal government can influence spending by states through subsidies to programs that states operate, no recent work has quantified the magnitude of this effect for Medicaid, the largest program of this type in the U.S. We find that Medicaid spending per enrollee responds to the magnitude of the federal subsidy. The Affordable Care Act (ACA) and its subsequent interpretation by the Supreme Court gave states the option to expand eligibility for their Medicaid programs in exchange for increases in the generosity of the federal subsidy. States that exercised this option increased Medicaid spending per enrollee on enrollees who were eligible even before the ACA by approximately 15 percent. Depending on the specification, this translates into an elasticity of Medicaid spending per enrollee with respect to the after-subsidy price of Medicaid to a state of -0.494 to -0.579.

With the expansion of Medicaid under the Patient Protection and Affordable Care Act and rising health care costs, the Congressional Budget Office projects that such expenditures will grow to about $576 billion by fiscal year 2024. GAO was asked to examine variation among states in Medicaid spending and factors that influence such spending, and state approaches to setting rates per enrollee for Medicaid managed care plans. This report examines (1) Medicaid spending per enrollee by state; (2) selected factors that influence Medicaid spending per enrollee, by state; and (3) how states account for factors that influence expected per-enrollee spending when setting rates for Medicaid managed care plans. GAO analyzed Medicaid enrollment and combined federal and state expenditure data provided by CMS for fiscal year 2008, and examined illustrative examples of factors that have been reported to influence health care spending, such as age, disability status, and scope of benefits received. GAO used 2008 data because they allowed a comparable view of spending per enrollee and factors that influence spending per enrollee-as 2008 was the most recent year for which certain data on factors influencing spending were readily available.
