Book excerpt: Noise exposure is the second leading cause of acquired sensorineural hearing loss and is one of the most common occupational and environmental hazards. Examining changes in behavioral thresholds has long been the standard for detection and monitoring of noise-induced hearing loss (NIHL). It has been suggested that electrocochleography (ECochG) could be an additional tool for assessing NIHL. Otoacoustic emissions (OAEs) provide objective information on the integrity of outer hair cell (OHC) function and have also been suggested for evaluating damage due to noise overexposure. The broad experimental question was: What is the effect of short-term narrowband noise exposure, as a function of ear and gender, on cochlear function as measured with behavioral, ECochG, and OAE indices? In Experiment 1, it was of interest to first examine the reliability of ECochG electrode type (i.e., Lilly-TM Wick vs. TIPtrode[TM]) at two stimulus rates (i.e., 7.7/s vs. 77.7/s). Electrode and rate were statistically significant (p [less than] .001) predictors of SP and AP responses (i.e., responses were more apt to be present when recorded with the wick electrode at the slow rate). Test-retest reliability was examined with correlation coefficients, linear mixed model analyses of variance, test-retest differences, and Bland-Altman plots. There were statistically significant correlations (p [less than] .05) between initial test and retest for all ECochG indices (i.e., summating potential [SP] amplitude, action potential [AP] latency, AP amplitude, SP/AP amplitude ratio, and SP/AP area ratio) when using a Lilly TM-Wick electrode and for all ECochG indices except SP amplitude when testing with a TIPtrode[TM]. Amplitude measures were significantly (p [less than] .01) larger when recording with the wick electrode. SP amplitudes were significantly (p [less than] .05) larger for the faster rate. AP latency was significantly (p [less than] .001) longer for the fast rate. AP amplitudes were significantly (p [less than] .05) larger for the slower rate. Both SP/AP amplitude ratio and SP/AP area ratio were significantly (p [less than] .001) larger for the fast rate. There was no statistically significant effect of test on any ECochG indices (p [less than] .05). In Experiment 2, auditory threshold differences were examined as a function of ear, gender, and frequency. Significantly (p [less than] .0001) larger auditory threshold differences were observed for left ears and for 3000 Hz than 2000 Hz, 4000 Hz or 6000 Hz. Additionally, statistically significant correlations between right ear auditory threshold differences at 3000 Hz and right 2000 Hz pure tone acoustic reflex thresholds (p = .04) as well as between left ear auditory threshold differences at 3000 Hz and left 2000 Hz pure tone acoustic reflex thresholds (p = .03) and 2000 Hz narrowband noise acoustic reflex thresholds (p = .01) were found. Statistically significant main effects of level (p [less than] .0001) and frequency (p [less than] .0001) were observed for DPOAE I/O functions in Experiment 3. DPOAE absolute amplitude differences were largest for the L1, L2 level of 55, 40 dB SPL and smallest for the L1, L2 level of 65, 65 dB SPL. DPOAE absolute amplitude differences were also smallest for the f2 frequency of 2051 Hz. A statistically significant gender by frequency interaction (p [less than] .05) was also identified. Females generally had larger DPOAE absolute amplitude differences than males except at the f2 frequency of 4980 Hz. Finally, Experiment 4 revealed a statistically significant interaction of ear and gender (p [less than] .05) for SP amplitude. SP amplitudes were significantly increased for female left ears following noise exposure while female right ears showed essentially no change. Additionally, left ear SP/AP amplitude ratios and SP/AP area ratios were significantly (p [less than] .05) increased following noise exposure. In summary, due to the excellent test-retest reliability and easier identification of ECochG wave components, Lilly-TM Wick electrodes were deemed superior for recording ECochG. Experiments 2, 3, and 4 revealed that behavioral thresholds, DPOAE I/O functions, and ECochG showed measureable changes following a 2000 Hz narrowband noise exposure.

Book excerpt: In order to identify persons with early noise-induced cochlear damage and to predict susceptibility to future damage it is desirable to develop a test that is sensitive to effects of noise exposure on the auditory system. Traditionally, hearing has been monitored with behavioral methods requiring a threshold 10-15-dB shift to be considered significant. More recently otoacoustic emissions have been studied as a method, which may show earlier effects of noise exposure, and thus useful for hearing conservation programs. The aim of this project was to develop a test method applicable to the military population. Following a brief, intense noise, distortion product otoacoustic emissions (DPOAEs) were measured in normal hearing young adults for several conditions. Behavioral thresholds were measured using a Bekesy procedure. Experiment 1 involved a DPgram paradigm. F2 ranged from 1.0 to 4.0 kHz, with F2/F1=1.22. Stimulus levels were either L1=70 dB SPL, L2=60 dB SPL or L1=65 dB SPL, L2=45 dB SPL. The exposure was a 100 dB SPL, 1/3 octave band of noise centered at 1.41 kHz. Results showed a consistent decrease in behavioral thresholds at 2.0 kHz immediately following exposure. Only one of ten subjects showed a consistent decrease in DPOAE amplitude for any F2 frequency. In a second series of experiments, DFOAE input-output functions were obtained for F2-4.0 kHz, with F2/F1=1.22 and L1=L2+15 dB, before and after exposure to a 100 dB SPL, 1/6-octave band noise centered 2.28 kHz. Pre-exposure I/O functions were very stable. In response to low to moderate level stimuli, DFOAE amplitude tended to decrease immediately post-exposure, increase and then decrease again (i.e., bounce). While for both DP paradigms the time course of the DFOAE amplitude shifts roughly paralleled threshold shifts, there was significantly inter-subject and intra-subject variability.

Book excerpt: In 1966, sympathetic fibers were identified in the cochlea, terminating on the cochlear artery and near the auditory nerve fibers at the level of the habenula perforata (Spoendlin and Lichtensteiger, 1966). The cochlear sympathetic fibers originate in the stellate ganglion (SG) and the superior cervical ganglion (SCG). Both sets of fibers have a demonstrated effect on cochlear blood flow (CBF) (Laurikainen et al., 1993, 1994, 1997; Ren et al., 1993). The SCG fibers have been implicated in modulation of susceptibility to noise-induced hearing loss (NIHL) (Borg, 1982; Hildesheimer et al., 1991; Horner et al., 2001; Hildesheimer et al., 2002). Ablation of the SCG generally leads to protection from NIHL, but the previous studies do not separate the effects of SCG ablation on temporary and permanent threshold shift (TTS and PTS), and there is also some question about whether the effect is unilateral or bilateral. In the current study, we explore the effects of unilateral and bilateral SCG ablation on threshold shift from noise exposure, as measured with inferior colliculus (IC) evoked potentials, distortion product otoacoustic emissions (DPOAE), and outer hair cell (OHC) loss. SCGs were isolated at the level of the bifurcation of the carotid artery and removed unilaterally in 15 chinchillas. Another 8 chinchillas underwent bilateral ablation. 20 animals were employed as sham controls. Each animal was implanted with IC electrodes and exposed to an octave band noise centered at 4 kHz for one hour at 110 dB SPL. Hearing thresholds and DPOAE input/output functions were measured at six hours, one day, three days, one week, and three weeks after the noise. After the three-week test, animals were sacrificed and OHCs were counted for cochleograms. Results showed improved recovery of DPOAE amplitudes after noise exposure in ears that underwent SCGectomy, as well as lower evoked potential threshold shifts relative to sham controls. Effects of SCGectomy on OHC loss were small. Results of the study suggest that sympathetic fibers do exert some influence on susceptibility to noise, but the influence may not be a major one.

These programmes rely on monitoring hearing using pure tone audiometry. However, audiometry is not very sensitive and only detects a permanent hearing loss after significant injury has occurred. Distortion product otoacoustic emissions (DPOAEs) are generated by cochlear non-linear physiological processes associated with the outer hairs cells that underpin detection of sound, thus DPOAEs have promise as a tool to detect and monitor cochlear hair cell damage in NIHL. The DPOAE has two components, one generated at the point of interaction between the primaries (generator), the second reflected from the characteristic place of the DPOAE frequency (reflected). Some of the variability in DPOAE is believed to result from the interaction of the components, termed the DPOAE fine structure, and extracting the components may reduce the variability and improve the sensitivity of the DPOAE to injury. This study investigated the feasibility and reliability of a new approach to separating the DPOAE components, and their relationship to hearing measures in people with and without exposure to noise. Methods: Logarithmically swept primaries with a least-squares-fit (LSF) analysis were used to separate the DPOAE components, and the approach was evaluated and optimised in normally hearing subjects. Two studies followed that investigated the components of the DPOAE and how they correlate with behavioural measures of hearing. Firstly, in subjects (n=16) with 'normal' hearing (20 dB HL), both behavioural measures of the auditory filter (notched-noise maskers) and threshold were compared to the components of the DPOAE. In the second study subjects (108 men) with a mixture of thresholds, noise exposure and age were recruited and then divided into two equal sized and age matched groups (noise exposed and non-noise exposed) based on a detailed history of self-reported noise exposure. The DPOAE components were compared to audiometric threshold and behavioural measures of compressive nonlinearity using Schroeder-phase masked thresholds. Results: Overall, psychoacoustic measures of auditory filter and cochlear non-linearity were influenced by audiometric threshold, and exhibited no relationship with DPOAE level in subjects with 'normal' hearing (

Recovery from a 110 dB SPL 7.8 kHz pure tone presented for 20 minutes was measured using auditory brainstem response thresholds and distortion product otoacoustic emissions immediately after the traumatic exposure and 10 to 12 days later. Auditory toughening did not have an effect on hearing and outer hair cell function immediately following a traumatic sound exposure. The toughened groups showed, however, a greater recovery than the control group, which was proportionally dependent on the toughening sound intensity, 10 to 12 days after the traumatic sound exposure.
