Book excerpt: Effects of music therapy under general anesthesia in patients undergoing abdominal surgeryNakhli M.S., Kahloul M., Chakroun A., Harrathi M.A., Mhamdi S.Sahloul Hospital, Dept of Anaesthesiology & Intensive Care, Sousse, TunisiaBackground and Goal of Study: Music therapy, an innovative approach that has proven effectiveness in many medical conditions, seems beneficial also in managing surgical patients. The aim of this study is to evaluate its effects, under general anesthesia, on perioperative patient satisfaction, stress, pain, and awareness. Materials and Methods: This is a prospective, randomized, double-blind study conducted in the operating theatre of visceral surgery at Sahloul Teaching Hospital over a period of 4 months. Patients aged more than 18 undergoing a scheduled surgery under general anesthesia were included. Patients undergoing urgent surgery or presenting hearing or cognitive disorders were excluded. Before induction, patients wore headphones linked to an MP3 player. They were randomly allocated into 2 groups: Group M (with music during surgery) and group C (without music). Hemodynamic parameters, quality of arousal, pain experienced, patient's satisfaction, and awareness incidence during anesthesia were recorded. Results and Discussion: One hundred and fourty patients were included and allocated into 2 groups that were comparable in demographic characteristics, surgical intervention type and anesthesia duration. Comparison of these two groups regarding the hemodynamic profile found more stability in group M for systolic arterial blood pressure. A calm recovery was more often noted in group M (77.1% versus 44%, p

Book excerpt: Background and Goal of the Study: Awake ophthalmologic surgery is particularly stressful for patients. Music has long been known to reduce anxiety, minimize the need for sedatives, and make patients feel more at ease. The purpose of this pilot study was to evaluate the effect of music on anxiety in outpatients undergoing elective eye surgery under topical anaesthesia. Materials and Methods: Sixty-two patients were prospectively and randomly assigned to hear relaxing music or no music via headphones just before surgery. Surgical Fear Questionnaire (SFQ) and Patient Satisfaction Questionnaire (PSQ) were used to assess anxiety before and after music session and overall patient satisfaction postoperatively. The proportion of patients receiving midazolam during surgery was also recorded. Comparisons were made using Chi-Square, or ANOVA, where appropriate. Results and Discussion: According to SFQ scores, anxiety was significantly reduced among the music group after listening to music (23 [0-45] vs 65 [10-88] p

Book excerpt: Background: This systematic review was designed to evaluate the analgesic effects of patient's "favorite music." The music was chosen by the elective cesarean section patient undergoing general anesthesia and it was played during the perioperative period. The review was designed to evaluate the evidence using GRADE and determine the effectiveness of music in reducing the overall pain and opioid usage in postoperative cesarean section patients. Method: The medical literature was exhaustively searched using the keywords listed below. The search was limited to randomized controlled trials on humans in the English language published between 2000-2011. MEDLINE, Web of Science, Cochrane Systematic Reviews and CINAHL were searched and three relevant studies performed in Turkey or Iran since 2008 were isolated. Results: Three RCTs were reviewed with a total of 250 patients. Half listened to favorite music prior to, or immediately after, undergoing cesarean section under general anesthesia. The control groups listened to no music. Overall opioid usage was significantly lower in the music group. The music group also had consistently lower pain scores. GRADE ratings for both outcomes of pain levels and opioid usage were considered High and suggested a strong recommendation for favorite music as adjunct therapy for pain relief. Conclusion: Favorite music, played either before or after c-section surgery to a conscious, uncomplicated patient undergoing elective cesarean section under general anesthesia, reduces postoperative pain and the use of opioid analgesic medication administered by PCA. The music intervention was determined to be risk free.

Book excerpt: Music therapy is a process based intervention combining the art of music and the interpersonal interaction of therapy as verified by science. Music therapy can reduce psycho-physiologic stress, pain, anxiety, and isolation while undergoing a major surgical procedure. This study determined the effect of music therapy in anesthesia recovery period among post surgical patient at the post anesthesia care unit of Tondo Medical Center. It utilized Pre-Test and Post test design for selected post surgical patients. The statistical measures used were mean, standard deviation and t-test for the pre-test/post-test design. Findings showed that there were significant differences in the physiologic responses after the intervention. Differences between the control and experimental groups were significantly noted. there were significant differences in the experimental group rather than the control group. Music therapy interventions would be an effective complimentary treatment to improve physiologic responses during the anesthesia recovery period.

Book excerpt: Numerous studies have examined music therapy and its effect on relaxation in the operating room, post-anesthesia units, and critical care units, but studies on music therapy in endoscopy are limited. This study examined the effect of soft music during a colonoscopy and its' relationship to the amount of conscious sedation required by the patient. A Convenience sample of thirty-eight patients undergoing a colonoscopy in the Ambulatory / Endoscopy unit were examined. Nineteen patients who received the soft music were compared to a group of nineteen patients who did not receive the music therapy. A medication log sheet was utilized to record the amount of conscious sedation that each patient received. Each patient that agreed to participate in the study signed a consent form. The t-test was utilized to test the level of significance for this study. Each of the three medications used was examined individually. The first medication, Demerol, revealed a t-value of 1.9 which was not significant at p=.06. The second medication, Versed, resulted in a t-value of 2.6, p=.01 which was statistically significant. Recommendations for further study include a larger sample size, a reduction in visual distractions, limiting study to a more narrow age group, and examining gender differences.

It can be performed with no preoperative testing, no intravenous insertion, and no monitoring. Like a dental procedure, the patient simply gets up and goes home after the procedure. Presented in an easy-to-read, bullet-point format, Wide Awake Hand Surgery guides surgeons through all aspects of WALANT. The book covers a wide variety of topics including minimal pain injection of local anesthesia, nerve and tendon decompression, wrist surgery, repair of lacerated tendons, tendon transfers, finger fractures, lacerated nerves, metacarpal fractures, arthritis surgery and complex reconstructions in hand surgery. The book includes more than 150 step-by-step surgical and instructional videos as well as numerous color clinical photographs. Color drawings clearly guide the surgeon to the correct anatomic locations for anesthetic injections, and the book includes an atlas of tumescent local anesthesia distribution anatomy. Available in PDF, EPUB and Kindle. Book excerpt: Despite changes in treatment modalities, pain and anxiety are both natural and highly anticipated patient outcomes related to a surgical procedure. Patients experience anxiety related to the uncertainty of any surgical procedure. Increased levels of pain and anxiety can lead to negative effects both physiologically and psychologically. Narcotics and sedatives are most commonly administered to alleviate both of the conditions. When given in large amounts and coupled with anesthetic drugs, adverse effects such as over-sedation can occur. This is an adverse effect that can be problematic and costly for not only the patient, but the nurse and facility as well. Respiratory depression and over sedation go hand-in-hand. When over sedation occurs, a patients respiratory effort can be greatly impacted. Over-sedation and other adverse effects can prolong not only the recovery period, but ultimately delay patient discharge. By adding a non-pharmacologic intervention such as music therapy to the treatment plan, the nurse will be able to reduce pain and anxiety, thus reducing the amount of narcotics and sedatives required. This can be used as a nursing driven intervention to be implemented in the pre-operative phase and follow the patient through the post-anesthesia phase. With the appropriate implementation plan, stakeholder acceptance, and resources, music therapy can be a beneficial influence on physiological parameters and psychological state. Music therapy can provide relaxation, lessen muscle tension, and provide a healing influence for patients. Music therapy can have a great impact on not only patient satisfaction, but anesthesia outcomes as well. Literature has shown that music therapy used as a complementary therapy has been proven as an inexpensive way to reduce perioperative pain and anxiety.
