Book excerpt: Context: Adolescent female athletes are at a 4-6 times greater risk for an anterior cruciate ligament (ACL) injury than male adolescent athletes. Most ACL injuries occur during a non-contact mechanism while decelerating and/or changing direction. Previous research uses unanticipated cutting tasks to recreate realistic scenarios that show an increased load on the ACL when the task is unplanned. Biomechanical and neuromuscular factors can be modified for the athlete to be better prepared for the high demands placed on the knee. Identifying deficits in female athletes during unanticipated cutting maneuvers can contribute to improving mechanics at the knee and work to prevent ACL injuries. Objective: To determine differences in knee biomechanics during an anticipated and unanticipated sidestep cutting task through trunk, hip, knee, and ankle kinematics and kinetics. Study Design: Cross-over study. Setting: Laboratory. Participants: 10 healthy high school female athletes. Inclusion: Healthy with no lower extremity injury in the past six months or cleared by physician. Exclusion: History of ACL injury or repair, previous injury to ankle, hip, or knee in past 6 months, and any vestibular or balance disorders. Intervention: Participant will run 6 m at a speed of 3.5-5.0 m/s toward a force plate to perform either an anticipated or unanticipated sidestep cut or crossover cut. Main Outcome Measures: Kinematic data was assessed using a 12 camera 3D analysis. Kinetic data was assessed using an in-ground force plate. A time series analysis was conducted on the data. Results: No significant differences were found for the frontal plane, sagittal plane, or ground reaction forces for kinematics or kinetics. Conclusion: The data shows a trend that the female athletes were in a more at risk position of sustaining an ACL injury during the unanticipated task compared to the anticipated task.

Book excerpt: An athlete is required to adapt to different situations throughout participation such as not knowing which direction they will be running, judging the actions of their opponents, catching or throwing a ball, and listening to the people around them. We investigated the effects of anticipation and dual tasking on trunk and lower extremity kinematics during a side step cutting maneuver in 32 healthy active males. Participants were directed with a green light to cut to the right or left either before they started running, anticipated cut, or right before they made the cut, unanticipated cut. While making the cut the participant also had to focus on possibly catching a ball meaning the ball was either faked or thrown to the participant. Our results did not indicate there was a significant effect of anticipation on a task. However, when the ball was either faked or thrown regardless of anticipation there was a decrease in lateral trunk flexion towards the stance leg at peak knee flexion and toe off. There was also an increase in trunk rotation at initial contact, peak knee flexion, and toe off when the ball was thrown. Overall, anticipation or dual tasking did not affect lower extremity kinematics.

Objectives: 1a) To compare entry speed and ECA between sex and leg preference during unanticipated sidestep cutting in soccer players. 1b) To determine the relationships between ECA with peak hip and knee joint angles during unanticipated sidestep cutting in soccer players. 2) To determine if an interaction effect exists between sex and leg preference on peak hip and knee angles during unanticipated sidestep cutting in soccer players.Methods: 21 adolescent and adult male and female soccer players completed unanticipated running sidestep cuts to a prescribed 45 ̊ in both directions. Lower limb kinematics were attained via 3D motion capture. Hip and knee joint angles were calculated using a joint coordinate system, and peak angles were identified during the stance phase of cutting. Book excerpt: Noncontact anterior cruciate ligament (ACL) injury is becoming more common among sport-related injuries. Several “high-risk” movements such as landings and cutting have been associated with ACL injury along with several biomechanical risk factors. These risk factors, specifically at the hip, knee, and neuromuscular abnormalities (unbalanced quadriceps-hamstrings activation ratios) have also been identified as possible causes that lead to ACL injury. Females are nearly 2.5 times more likely to experience an ACL injury while playing volleyball when compared to their male counterparts. While research continues to investigate the reasoning for this injury, the results may not be accurate to what athlete’s experience while playing in a real competition. The purpose of this study was to determine the effects of a physical overhead goal and a subsequent unanticipated lateral cut on the kinematics, kinetics, and muscle activation of the hip and knee of the dominant leg during the landing of a stop vertical jump task (volleyball block). Participants completed four testing conditions – with a target and an anticipated cut following the landing (WTA), with a target and an unanticipated secondary cut (WTUA), and then both anticipated and unanticipated secondary movements with no overhead target (NTA, NTUA). Results indicated that when a target was added to the testing environment, there was a significant decrease in the initial contact (IC) knee flexion angle and an increase in the peak knee extension moment. When comparing anticipated and unanticipated tasks, there were decreases in the IC knee abduction angles and peak knee extension moments in the unanticipated trials compared to the anticipated trials. Significant interactions were also seen in the peak knee abduction moments and the IC hip flexion angles specifically when the target is present. These findings suggest that the more realistic testing environment altered hip and knee landing mechanics, which in turn can be used to help better evaluate the risk factors for ACL injury.

Sex and age are factors that have an impact on these injuries. One possible reason is that females have harmful movement patterns compared to males during activities such as cutting while running. The literature still lacks studies examining the influence of age on these movements. Therefore, research is needed to determine if sex and age have an impact on the movement patterns of soccer players. The primary objective of this study was to examine if sex and age impact the joint angles and inter-joint coordination during an unanticipated cutting task in soccer players. A secondary objective compared the strength, jump performance and dynamic balance of male and female soccer players of different age groups. The study recruited healthy soccer players of different age groups. This included 11 adolescents (five males, six females) and 11 adults (seven males, four females). A 12-camera three-dimensional motion capture system recorded joint angles while participants performed an unanticipated side-step cutting task. A hand-held dynamometer measured muscle torques. Hop tests measured jump performance and the modified Star Excursion Balance Test measured dynamic balance. Principal component analysis (PCA) identified differences in joint angles. Adult males presented greater changes in hip flexion at pre-contact compared to stance/take-off than adolescent males and adult females had lower changes than adolescent females. Females had lower changes in hip flexion, and lower hip abduction and ankle inversion angles. Adolescents had greater hip internal rotation and knee flexion angles, but lower changes in knee flexion angles at pre-contact compared to stance/take-off. For inter-joint coordination, females had a more out-of-phase pattern than males in the foot/shank segment in sagittal plane. In regard to clinical measures we observed a few differences in knee extensor torque and balance, with adolescents exhibiting better results. Clinicians and coaches should be aware that these characteristics might impair performance and increase injury risk"--

Gender differences occurred at the hip as females used less hip abduction to perform the movements. Less hip abduction for females is closer to the "position of no return" seen in ACL injuries. Although our results confirmed published gender differences, this may have been due a combination of the difficulty of the tasks and the relatively low skill of the participants. Direction differences occurred as the crossover cut was performed with knee abduction and foot pronation while the opposite was true for the side cut. Furthermore, the crossover cut seemed to be performed by utilizing a preceding side cut. Therefore, the crossover cut seems to be a more dangerous movement. Although accommodation for unplanned movements may occur in the sagittal plane, unanticipated cutting movements are performed differently than anticipated cutting movements. Since anticipation can affect the performance of cutting movements, incorporation of unanticipated maneuvers should be included in training prevention programs. Also, increased development of methods for preventing anticipation is required to more closely simulate competition in the lab setting.

Injury can hamper this component reducing the quality of life. The time needed to recover from injury can have economic, psychological, and emotional impacts. Therefore it is important to determine ways to prevent injuries whenever possible. One common injury occurring to many active people is an inversion ankle sprain. Unfortunately after such an injury the likelihood it may occur again increases due to the decreased ankle stability. To help prevent ankle sprains, individuals can wear external ankle supports such as ankle tape or ankle braces. The latter of the two is reusable, does not degrade with activity, and reduces the risk of ankle injuries. However, with the reduction of the ankle’s range of motion there may be an alteration in the kinetic chain dynamics of the lower extremities, which may have consequences including risk of injury at other joints, including the knee and hip. Females have a four to six fold incidence rate of ACL injuries when compared to men. Additionally 60% of ACL injuries occur during noncontact situations. This study was designed to examine whether the kinetics of the knee and hip, and the angular kinematics of the knee are affected in a female population when wearing an ankle brace during a sidestep cutting maneuver. Sixteen healthy and recreationally active females between the ages of 18-40 performed a number of dynamic movement tasks using of their preferred leg. A total of 24 experimental trials were completed over two sessions (12 in session one and 12 in session two) with half of each sessions trials being straight run throughs and the other half being sidestep cutting maneuvers. Half of the participants wore a brace during their first session (no brace for their second) and the other half did not wear a brace during their first session (brace for their second). The results showed no difference in almost all dependent variables except for knee varus moment between sessions one and two with a p-value less than 0.05. These findings suggest a learning effect occurred and the participants altered their valgus moments without altering knee angular kinematics. Wearing an ankle brace did not increase the injury risk factors for the knee in this task.

Further, there are biomechanical differences of lower extremity joints in various tasks with both genders, which apparently means that the task utilized plays a crucial influence on joints dynamics. In soccer, the heading motion is one of the most common activities, which is more likely associated with landing. The purpose of this study was to investigate the effect of forward heading motion by comparing the kinetics and kinematics of lower extremity between genders during 1) a stop-jump task and 2) a jump-heading task. 10 male and 10 female soccer players performed 5 stop-jumps and 5 jumps with heading a soccer ball. Findings displayed a combination of reduced initial knee flexion, greater initial hip flexion, greater peak vertical GRF, and greater peak knee extension moments with both genders during the jump-heading task. Additionally, greater peak knee abduction angles were observed among female players during both tasks. These findings suggest that a higher risk of non-contact ACL injuries might occur during the jump-heading task compared with the stop-jump task, and this risk could be higher among female players than male players. These findings might lead soccer trainers to consider different strategies and techniques of landing after heading motions in game-like conditions. Also, PTs and rehabilitation designers who work with ACL-injured patients might consider the outcome of this study. Future studies should consider to examine ankle dynamics in addition to knee and hip in game-like conditions.
