Book excerpt: Objectives: To compare the effects of manual therapy, exercise, and cervical traction to manual therapy, exercise, and sham traction on pain, function and disability in patients with cervical radiculopathy (CR). Summary of Background Data: To date, optimal strategies for the management of patients with CR remain elusive. Preliminary evidence suggests that a multi-modal program consisting of manual therapy, exercise and cervical traction, may result in positive outcomes for patients with CR. However, there exists limited evidence to support the use of mechanical cervical traction in patients with CR. Methods: Patients with CR were randomized into one of two groups: Group MTEXTraction (manual therapy, exercise and intermittent cervical traction) and Group MTEX (manual therapy, exercise and sham intermittent cervical traction). Patients were treaated on average 2 times per week for an average of 4.2 weeks. Outcomes measures collected at baseline, 2 weeks, and 4 weeks included the numeric pain rating scale (NPRS), patient specific functional scale (PSFS), and neck disability index (NDI). Results: Subjects in both groups experienced significant improvements in pain, function disbility at the 2 and 4 week follow-up. There were no significant differences between the groups for any outcome measure at 2 weeks or 4 weeks. Conclusions: The addition of intermittent cervical traction to manual therapy and exercise did not provide additional improvements in pain, function, and disability. Patients receiving either management strategy demonstrated improvements in pain, function and disability over the course of a 4 week treatment program.

Book excerpt: The major part of the thesis involved in a randomized controlled clinical trial undertaken to investigate the effects of cervical manual therapy (mobilization and manipulation) on patients with chronic mechanical neck pain. One hundred and forty-seven patients were recruited from the Physiotherapy Department, Prince of Wales Hospital and randomized into a manual therapy (MT) group and a control (CON) group. The age range of the MT group was from 19 to 66 (mean age 46.6℗ł10.0) and that of the CON group was from 23 to 63 (mean age 45.5℗ł9.5). The patients in the MT group received manual therapy, 15-minutes of irradiation therapy (IRR) together with neck exercises listed in a neck exercise pamphlet, whereas those in the CON group only received IRR and neck exercises. All patients received 8 sessions of treatment over a 4-week interval. The outcome measures included subjective pain intensity (NPRS), neck disability (NPQ), cervical ROM, HRA and postural stability (BBS). Assessments were made by a blinded assessor at baseline, immediately post-intervention (completion of 8 sessions of treatment), 3 months and 6 months after completion of treatment. No statistically significant differences were found between the two groups in any of the outcome measures in the baseline comparison. After 8 sessions of intervention over a 4-week period, patients in the MT group had realized greater improvement than those in the CON group in cervical ROM (12.85% vs. 1.11%), HRA (25% vs. 3.32%), postural stability (BBS) (38.3% vs. 17.19%) and NPRS (38.9% vs. 10.2%) (p

Background: Manual therapy and spinal manipulation are commonly used as an integral part of the treatment by a physical therapist for the treatment of neck pain. The use of Translatoric Spinal Manipulation, or TSM, will provide a safe alternative to non-specific multi-segment rotational cervical manipulation. Methods: 3 participants were randomly assigned for each treatment group (HV and LV), with a primary complaint of neck pain. Each participant received 6 treatments in a 2 week period. The subject's cervical mobility and pain was recorded before and after each intervention. The NDI was completed prior to the first and following the last intervention. Results: Visual interpretation shows improvement in ROM for both groups. The LV group reports twice as much significant decrease in self-reported level of pain. All participants reported a significant functional improvement on the NDI scale. No adverse events were reported. Conclusion: The results from this study support the hypothesis that cervical traction manipulation of the cervical spine may immediately improve ROM, pain and function. The completion of the RCT with a larger sample size is needed to evaluate the interaction between the velocity and the effectiveness of the intervention.

Traumatically compressed medullary tissue is invariably attenu ated and weakened. On application of skull traction, the weakened section of the compressed cervical cord is the part that is most overstretched. This inevitably results in increased neurological def icit. On scrutiny of the clinical records of 100 tetraplegic patients treated by skull traction at the National Spinal Injuries Centre at the Stoke Mandeville Hospital, Aylesbury, England, from 1971 to 1982, we found that therapeutic skull traction had been followed by an immediate increase in neurological deficit in 12% of the patients - a relatively high figure for the type of case in which beneficial effects oftraction were habitually anticipated. Moreover, in studies on cadavers, artificial defects in fresh human cervical cord in situ showed typical deformation following application of traction, confirming the basic deleterious effects of therapeutic skull traction on the injured cervical cord.

Book excerpt: Neck pain, also known as cervical pain is one of the most common pain a person would encounter one way or another. It may be due to stress, mechanical faults, posture, etc. It is usually accompanied by stiffness and other maladies, which cause disruption in functions. Neck pain is less studied, unlike to low back pain. However, a survey on Canada and Finland revealed that neck pain is a problem commonly reportyed from 70 percent of the population at some time of their lives (Bogduk, 2003). In the Philippines, there are no studies published yet that extensively investigate neck pain. A neck pain, which is diagnosed to have no specific identifiable cause in 12 weeks or more is commonly called chronic mechanical cervical pain (CMCP). Clinically, patients will complain form pain with or without range of motion limitations, but often associated with feeling of heaviness and discomfort. Functions might have been affected by the presence of discomfort, not because of the limitation of motion. There are nmo specific evidence-based clinical guidelines for the treatment of chronic cervical pain the Philippines. AIM OF THE STUDY: The study aimed to compare the effectiveness of specific cervical mobilization (SCM) to physical therapy (PT) modalities and exercies on chronic mechanical cervical pain (CMCP). This effectiveness would be based on biopsychosocial model of disease, wherein the bases of improvement is the patients' perception of improvement in terms of pain, functions and psychoemotional perceptions. METHODOLOGY: There are twenty patients that initally answered the BNQ, were divided into two groups: ten patients for the SCM group and ten for the PT group. The PT groups had undergone PT using the hot moist pack, ultrasound and exercises thrice a week. While the SCM group had two sessions per week of specific cervical mobilization of either C4-C5 or C5-C6. After six weeks, the BNQ was administered again and the statistics was calculated. RESULTS: Using the Wilcoxon Signed Rank Test, both groups have siginifcant difference from their pre and post scores on the BNQ scores. Meaning, SCM and PT were both effective in managing CMCP. To test if there is significant difference between the two groups, Mann-Whiteney Test. In this study, there are no significant difference with SCM and PT. This might mean that either PT or SCM could me effective in managing CMCP.
