Book excerpt: Osteoporosis is a condition involving decreased bone mineral density (BMD) and increased fragility of the skeletal system. Osteoporosis affects ^75 million individuals in the United States, Europe, and Japan. In the United States alone, hip fractures affect 500,000 individuals per year, and annual healthcare costs for osteoporotic fractures are approximately $l4 billion. A high peak BMD can prevent or delay the onset of osteoporosis and its complications. Exercise and diet may affect peak BMD by as much as 20 to 40% each and have been identified as the two most important controllable factors determining BMD. The current study investigated the effect of a calcium, vitamin D, and vitamin K supplement combination during initial military cadet exercise training on: BMD, stress fracture occurrence, hormones associated with BMD, and biochemical markers of bone turnover. Significant changes in BMD, either between the supplemented group or the unsupplemented group or across time for both groups were not found. The majority of participants (n = 22) had unexpectedly high levels of physical activity prior to enrollment, and the initial military exercise training program included only moderate levels of activity. Book excerpt: Current options for maintaining or slowing aging-related bone mineral density (BMD) loss in postmenopausal women primarily include pharmaceutical agents. More recently, physical activity and exercise have been suggested as highly effective, low cost alternatives. Weighted aerobic exercise, utilizing load carriage systems (LCS), is known to increase the gravitational forces impacting bone, creating a higher osteogenic stimulus than standard aerobic exercise. In response to the positive research on aerobic exercise with well-designed LCS, Dr. Lawrence Petrakis, MD, developed a unique 5.44 kg uniformly weighted exercise suit. This study aimed to examine the effects of the uniformly weighted exercise suit on serum biochemical markers of bone formation (Amino- Propeptide of Type 1 Collagen [P1NP]; Carboxy-Terminal Propeptide of Type 1 Collagen [P1CP] and resorption (Carboxy-Terminal Telopeptide of Type 1 Collagen [CTX]) in response to submaximal aerobic exercise in postmenopausal women with low bone density. Nine volunteer, sedentary to lightly active, healthy postmenopausal women (Age: 58.7±1.1 years, BMI: 28.2±1.0, BMD T-score: -1.2±0.5) participated in this within-subjects study, wherein each participant exercised under two counterbalanced conditions (aerobic exercise with [ES] or without [NS] the exercise suit). During each condition, participants walked on a treadmill at 65%-75% of their age-predicted maximum heart rate until they reached their goal caloric expenditure (400kcal). There was a seven-day washout period between sessions. Serum was processed using ELISA protocols to investigate the change in biomarker at 24 and 72 hours post exercise, relative to baseline. The results indicated, when compared to the NS condition, the ES condition elicited a greater positive change in P1CP at 24 hours (Phours following exercise (P0.05). There was no effect of condition on P1NP at any time point (P>0.05). In sum, submaximal aerobic exercise while wearing the uniformly weighted exercise suit elicited an antiresorptive effect on bone collagen resorption with a simultaneous increase in bone collagen formation 24 hours post exercise.

Book excerpt: The Female Athlete Triad (Triad), a syndrome of low energy availability, menstrual dysfunction, and low bone mineral density (BMD), affects up to 60% of exercising girls and women. As such, understanding the physiology underlying the Triad is imperative in order to identify therapeutic targets and develop effective treatment strategies to prevent long-term health detriments in afflicted women. Bone health is a major clinical outcome of concern in female athletes and exercising women with the Triad, as bone loss or suboptimal peak bone mass development may predispose women to osteoporosis and related fractures later in life. To garner a better understanding of factors contributing to bone health and to inform guidelines for treatment targets in female athletes and exercising women with the Triad, this purpose of this dissertation was 1) to assess the unique and combined effects of energy status and estrogen status on volumetric BMD (vBMD), bone geometry, and estimated bone strength in exercising women, 2) to assess the unique and combined effects of energy status and estrogen status on bone turnover in exercising women, 3) to evaluate predictors of change in bone turnover dynamics across a 12-month nutritional intervention in women with exercise-associated menstrual disturbances, and 4) to determine if bone turnover dynamics at baseline impact BMD outcomes in women with exercise-associated menstrual disturbances participating in a nutritional intervention. Key findings of the studies that made up this dissertation implicate that energy status is a main driving factor dictating vBMD, bone geometry, estimated bone strength, and bone metabolism in exercising women. The results indicate that improving energetic status will not only improve bone turnover at the cellular level, but that bone turnover favoring bone formation over bone resorption will also predict improved BMD in the following year. The findings support the guidelines put forth by the Female Athlete Triad Coalition that endorse nutritional therapy as the first line of treatment for the Triad, as low energy availability is the root cause of the clinical sequelae, including menstrual dysfunction and compromised bone health.

Book excerpt: Weight-bearing exercise positively affects bone mineral density (BMD) and bone strength, presumably by altering the balance between bone formation and resorption. The purpose of the present study was to determine the acute response of markers of bone turnover and hormones to a single-bout of resistance-training or plyometrics. Twelve recreationally active males, aged 24-63 years, participated in this partially randomized cross-over study, which included five trials: PLY (fed/fasted), RT (fed/fasted), and a no-exercise, fed control trial (CON) (n=6). Blood was drawn immediately prior to exercise (PRE), immediately following exercise (POST), and 15, 30, 60, 120 min, and 24 hr following PRE. Total testosterone (T), intact parathyroid hormone (PTH), and cortisol (COR) concentrations in serum were determined. Serum bone-specific alkaline phosphatase (BAP) (bone formation marker) and tartrate-resistant acid phosphatase, isoform 5b (TRAP5b) (bone resorption marker) were measured. The results of the present study suggest favorable changes in bone formation and bone resorption as assessed by bone turnover markers (BAP and TRAP5b) following a single-bout of RT or PLY. The decrease in PTH following plyometrics and resistance-training positively correlated with a decrease in TRAP5b, suggesting that PTH may mediate the exercise-induced changes in osteoclast activity.

Book excerpt: "Introduction: Young male jockeys compromise bone health by engaging in caloric restriction and high volumes of physical activity during periods of musculoskeletal growth and development. Failure to attain peak bone mass (PBM) during growth can have adverse short and long term musculoskeletal effects, with numerous studies demonstrating inferior bone health in jockey populations. However, no study to date has addressed counteracting the deleterious effects that participation in this sport has on bone health. The purpose of this six-month double-blind randomised placebo controlled trial was to examine the efficacy of 800mg calcium and 400 IU vitamin D daily supplementation on improving bone mineral properties at the tibia (weight-bearing) and radius (non-weight-bearing) using peripheral Quantitative Computed Tomography (pQCT) and blood-borne markers of bone turnover.Three inter-related studies were designed following the findings of a systematic review and meta-analysis examining the effects of calcium and vitamin D supplementation on bone mineral density in healthy males. Study one examined the effect that the protocol had on markers of bone metabolism and bone properties of the radius. Study two was designed to investigate the impact of six months supplementation on weight-bearing bone while study three further explored alterations to radial and polar cortical bone properties at the tibial mid-shaft following the clinical trial.Methods: Twenty-nine male jockeys (age=20.18 ± 3.23yrs) were originally recruited to the study with 17 completing the intervention. Bone properties at the ultra-distal (4%) and proximal (66%) radius and tibia using pQCT and serum vitamin D, Procollagen type 1 N propeptide (P1NP) and C-terminal telopeptide of type I collagen (CTx) were assessed at baseline and six months. Bone properties at the 66% tibial site were further analysed using BoneJ pQCT distribution plug-in. Polar and radial volumetric bone mineral density (vBMD) was measured in 36, ten degree cortical sectors (polar) and three concentric cortical divisions (radial). Polar distribution was further consolidated into four, 90 degree quadrants aligned to anatomical planes. Cortical mineral mass, endocortical and pericortical radii were also analysed.Results: After co-varying for height, body mass and baseline bone measurements, the analysis of co-variance (ANCOVA) results of these combined studies demonstrate that six months calcium and vitamin D supplementation stimulated a reduction in bone resorption together with significant improvements to bone material properties at the proximal tibia in the supplemented group. ANCOVA serum analysis indicated significantly higher vitamin D levels (18.1%, p=0.014) and lower CTx (ng/L) (-24.8%, p=0.011) in the supplemented group with P1NP unchanged.
