To aid in developing strategies for prevention and remediation of overweight in military personnel, the U.S. Army Medical Research and Materiel Command requested the Committee on Military Nutrition Research to review the scientific evidence for: factors that influence body weight, optimal components of a weight loss and weight maintenance program, and the role of gender, age, and ethnicity in weight management.

Scientists have established that caloric intake, macronutrient composition of the diet, basal and resting metabolic rate, thermic response to food, energy expenditure associated with movement and physical activity, and preferential storage of the surplus of calories as fat or lean tissues are critical determinants of energy balance and body weight. While much has been learned, the field is poised for major advances with the advent of a variety of imaging techniques, progress in quantitative and molecular genetics, use of transgenic rodent models and of breeding experiments with informative inbred strains, availability of stable isotopes for metabolic and behavioral studies, and a growing number of useful experimental animal and human models. This volume takes an integrative approach to obesity. It is structured around four major topics: (1) the animal and human models currently available for the study of body weight regulation with their strengths and limitations, (2) the molecular and genetic basis of the regulation of body weight, (3) the metabolic and physiological mechanisms involved, and (4) the behavioral and social determinants. The 13 background papers provide a critical overview of the present knowledge base while the group reports summarize the extensive deliberations of 38 international experts. Particular emphasis has been given to promising research areas and on the advances needed to ensure a better understanding of the biological and behavioral mechanisms of the regulation of body weight, with a particular emphasis on overweight and obesity.

Two stimuli can be discerned that give a sense of urgency to this research. The first is the data, from many sources, confirming an inexorable upward trend in the prevalence of overweight and obesity in developed countries. The picture in the emerging nations is unclear because of both a lack of appropriate survey data and the continued scourge of under nourishment among their poor. It is likely, however, that, throughout the world, wherever disposable income and food availability are high, obesity and overweight will be a continuing and increasing problem. The second driving force among researchers is the realization that, to date, there has been little success in either stemming the tide of individuals experiencing excessive adiposity or enabling them to recover a more desirable body weight and composition. Such are the problems. But significant progress in research into the origins and treatment of this condition is being made, and in recent years has been brisk. Technical advances (such as the ability to measure total energy expenditure in free-living individuals with good reliability), new and imaginative thinking and a determination not to be satisfied with hypotheses until they have been thoroughly challenged by experiment have yielded significant advances.

Book excerpt: Background: Poor diet choice and lack of physical activity has led to high rates of obesity in the United States. Despite this, physicians are reluctant to communicate the risk factors of obesity with their patients. The Affordable Care Act is encouraging obesity prevention measures by rewarding physicians who incorporate weight management behavioral counseling in their practice. Prochaska's Transtheoretical Model has been used in prior studies as a weight management counseling method. However, the attrition rate in weight management programs is still high. Objectives: To implement a pilot study for weight management that increases participant retention rate as well as participant weight loss utilizing the Transtheoretical Model and by maintaining weekly contact with participants. Methods: Nineteen obese adult patients from Jefferson's Family Medicine were enrolled in a weight management pilot study and followed for 3 months. At baseline participant's social demographic information was documented and health records were added. In addition, a formal assessment of each participant's stage of change with respect to physical activity, fruit and vegetable intake, and fat intake was initiated and a personal action plan was created. A four week action plan was developed by the participant. The counselor made weekly contact through participant's choice of telephone calls, text messages, or email to reinforce and support each participant's efforts. At three months the participant met with the counselor for a follow-up visit to reassess goals and document physical changes. Results: There were 19/62 (32%) referred patients who were enrolled in the study. At 3 months 16/19 (84%) remained in the program (95% CI: 0.604-0.966, p=.0001). Patients lost on average 2 kg and 2.2% of their body weight. All patients remained obese except one patient who dropped to 29.7 kg/m2. Conclusion: The pilot project was successful based on the retention rate and weight loss.
