Book excerpt: Clinical Ladder programs have documented benefits for both participants and healthcare organizations. These benefits include: (a) retention and recruitment, (b) increased job satisfaction, (c) autonomy, and (d) positive impact on quality of patient care. The purpose of this quantitative descriptive study was to gain a better understanding of factors that influence decision making of nurses to participate or not participate in a Clinical Ladder program. Expectancy theory is a process motivational theory, which was used as the conceptual framework to help identify factors influencing decision-making. In this quantitative study, the researcher used a Likert scale survey tool to measure (a) demographic information, (b) perceived barriers, and (c) internal and external motivational factors. Two different survey tools specific to whether nurses have participated or not in the Clinical Ladder program were used to gather data. The surveys were distributed to a convenience sample of nurses throughout a 650 bed midwestern hospital with an existing Clinical Ladder program. The results demonstrated that the subjects who had never participated in the Clinical Ladder program were motivated by extrinsic factors, specifically monetary compensation. Clinical Ladder participants were motivated more by intrinsic motivational factors. Overall, Clinical Ladder programs demonstrate positive benefits, in not only professional growth, autonomy, positively impacting practice and job satisfaction. It is necessary for nursing leaders to support Clinical Ladder programs to achieve maximum enrollment in the program. These study results have potential to achieve increased Clinical Ladder program participation.

Data were collected via an online survey and variables of interest were explored using descriptive statistics. When demographic variables were tested, there were no statistically significant differences found between those who participated and those who did not participate in the clinical ladder program. When professional attributes were tested, statistically significant differences were found in clinical competence, clinical performance, rewards and benefits, and job satisfaction with participants showing more of these attributes than nonparticipants.
