Book excerpt: Foundational Knowledge for the Practice of Family Medicine in West Africa is a selection of introductory concepts from literature using literature search and review; a collection of lectures from eminent family physicians from South Africa, Canada, United Kingdom, and United States; lecture notes; seminal essays; and journal articles that guided the author through the transition from general practice to family medicine in the 1980s and 1990s. The concept of family medicine in West Africa was a paradigm shift from a specialty conception of a conglomeration of independent specialties in general practice to a unique specialty with its own distinct identity with a fundamental principle based on the biopsychosocial model. The specialty draws a distinctive identity from its approach to care by placing emphasis on the contextual setting of the patient, starting with the family and extending to the sociocultural and economic environment. This composite approach to health care was new and confusing to the uninitiated medical graduate undergoing a residency training taught in the traditional biomedical reductionist model and to other medical specialists who asked what new knowledge we were bringing to the table that other specialists were not already teaching. As for those teachers of family medicine in the subregion, it was quite clear as to the need for this conceptual framework in family medicine as opposed to the limitation of general practice. This book documents the struggle to position family medicine as a separate specialty of medicine in West Africa.

Available in PDF, EPUB and Kindle. Book excerpt: Long Road from Quito presents a fascinating portrait of David Gaus, an unlikely trailblazer with deep ties to the University of Notre Dame and an even more compelling postgraduate life. Gaus is co-founder, with his mentor Rev. Theodore M. Hesburgh, C.S.C., of Andean Health and Development (AHD), an organization dedicated to supporting health initiatives in South America. Tony Hiss traces the trajectory of Gaus's life from an accounting undergraduate to a medical doctor committed to bringing modern medicine to poor, rural communities in Ecuador. When he began his medical practice in 1996, the best strategy in these areas consisted of providing preventive measures combined with rudimentary clinical services. Gaus, however, realized he had to take on a much more sweeping approach to best serve sick people in the countryside, who would have to take a five-hour truck ride to Quito and the nearest hospital. He decided to bring the hospital to the patients. He has now done so twice, building two top-of-the-line hospitals in Pedro Vicente Maldonado and Santo Domingo, Ecuador. The hospitals, staffed only by Ecuadorians, train local doctors through a Family Medicine residency program, and are financially self-sustaining. His work with AHD is recognized as a model for the rest of Latin America, and AHD has grown into a major player in global health, frequently partnering with the World Health Organization and other international agencies. With a charming, conversational style that is a pleasure to read, Hiss shows how Gaus's vision and determination led to these accomplishments, in a story with equal parts interest for Notre Dame readers, health practitioners, medical anthropologists, Latin American students and scholars, and the general public.

Based on his blog A Country Doctor Writes, these vignettes by Swedish born physician Hans Duvefelt range from Alexithymia to what doctors call Zebras, exotic conditions they always look for but usually never encounter. From delivering babies to attending timely and untimely deaths, they touch on every stage of life. Some pieces describe overlooked diseases and disease mechanisms and some describe heart rending life circumstances caused by both rare and common diseases. EARLY PRAISE"Hans is a wonderful storyteller. As a primary care physician myself, I look up to the wisdom, insight, and inspiration that resonate from his stories." (Kevin Pho, MD, Founder, KevinMD, Keynote physician speaker)"Whether you are a college student or a medical student considering a career as a Family Physician or if you are a resident looking to learn from a master clinician or someone who enjoys stories from the world of practice, this book is for you." Available in PDF, EPUB and Kindle. Book excerpt: From a childhood spent playing marbles and climbing trees in Ceylon to a medical career in bustling Singapore, Dr K. Puvanendran’s experiences have been rich and varied. A leading neurologist, he counts kings and presidents among his former patients. This charmingly written autobiography traces the trajectory of his life against the changing landscapes of two vastly different countries. As a boy in Jaffna, Dr Puvanendran found imaginative ways to fill his time. He recounts with great relish the carefree pranks, adventures and school experiences. Woven into the evocation of these simple pleasures are also sobering glimpses of the darker periods in Ceylon’s history. After attending medical college in Colombo, Dr Puvanendran stayed in Ceylon to work before accepting a position at Outram Road General Hospital, now Singapore General Hospital, in 1971. As the narrative unfolds, we read about the roots of his interest in neurology, the highs and lows of his career, the doctors who inspired him and the most memorable medical cases from his fifty years of practice. Some of these intriguing cases include sleep-related crimes, for which he has testified in court as a local pioneer in sleep medicine. Dr Puvanendran’s story takes us through the old world of Ceylon and into the heady post-independence days of Majulah Singapura (“Onward Singapore”, as the national anthem proclaims), offering along the way a successful doctor’s take on the study and practice of medicine.

Available in PDF, EPUB and Kindle. Book excerpt: This book considers the family doctor relationship and the process of ending that relationship. What happens when a family doctor or someone like them, deeply committed to long-term relationships, decides to end those commitments? What’s involved? What are the embodied experiences for doctor and patient, for doctor and staff, for physician leader and others? What comes next? This book invites the reader to immerse in personal stories and reflections of family physicians who choose to retire from practice, depart long-standing leadership roles, or shift from one place of deep relational commitments to something else. These stories concern the particulars of family medicine and general practice, but they share much with any vocation rooted in the duties, challenges, and rewards of relationships bound by covenant and not transaction. This book is relevant to all professionals involved in healing relationships.
