Book excerpt: The present volume contains a large variety of treatment approaches to the long-term rehabilitation of the alcoholic, ranging from the biological to the physiological to the psychological to the social. The multiplicity of proposed therapies, each of which has its strong proponents, suggests that alcoholism is either a complex medical-social disease syndrome requiring a multipronged treatment approach or a very simple illness for which we have not yet dis covered the remedy. The latter may, indeed, be true, but we cannot use what we do not know and must use what we do know. We do, however, have the obligation to be responsible in our treatment, to provide the best that is known at this time, and to be discriminating in our prescription of appropriate treat ment for individual patients. If there is one conclusion we would like to offer in our preface, it is that alcoholics constitute a markedly heterogeneous popula tion with widely disparate needs, for whom, at least at our present level of knowledge, a broad spectrum of treatment modalities is necessary. If this is true, then probably most of this book has validity. With this volume on the treatment and rehabilitation of the chronic alco holic, we bring to completion our five-volume series, The Biology of Alcoholism. As the title of the present volume indicates, we have departed from our original intention to deal solely with biological aspects of the syndrome and have attempted rather to produce a more comprehensive work.

An alcoholic is a victim of social circumstance and genetic destiny. Although one might imagine that this dual approach is a reflection of today's enlightened and sympathetic society, historian Sarah Tracy discovers that efforts to medicalize alcoholism are anything but new. Alcoholism in America tells the story of physicians, politicians, court officials, and families struggling to address the danger of excessive alcohol consumption at the turn of the century. Beginning with the formation of the American Association for the Cure of Inebriates in 1870 and concluding with the enactment of Prohibition in 1920, this study examines the effect of the disease concept on individual drinkers and their families and friends, as well as the ongoing battle between policymakers and the professional medical community for jurisdiction over alcohol problems. Tracy captures the complexity of the political, professional, and social negotiations that have characterized the alcoholism field both yesterday and today. Tracy weaves American medical history, social history, and the sociology of knowledge into a narrative that probes the connections among reform movements, social welfare policy, the specialization of medicine, and the social construction of disease. Book excerpt: This book reports the findings of a study of the treatment of alcoholism in the out-patient clinics and the related in-patient facilities of state-supported alcoholism programmes in the United States. The authors compared a number of clinics simultaneously, and were thus able to investigate the influence of a variety of treatment programmes on a variety of patients. They show that clinics play a valuable role in assisting patients who have retained social stability despite their problem by maintaining contact with such patients, but that they are rarely useful for modifying either drinking habits or other aspects of malfunctioning in the case of patients whose social stability has crumbled. The study further shows that improvement in drinking habits (either by abstinence or by controlled drinking) is related to what the clinic does and to changes in the patient's social and interpersonal environment outside the clinic.
