The biological fixation between the dental implant surfaces and jaw bones should be considered a prerequisite for the long-term success of implant-supported prostheses.Aim/Hypothesis : The aim of this case is to compare the effectiveness of immediate single implants with different surface in the aesthetic zone.Materials and Methods : Patients requiring a single tooth implant-supported restoration in the maxillary esthetic zone received the implant with rough surface and combine surface (rough surface + machined surface) placed in healed sites. The combine implant surfaces is also called hybrid implant. Implants that achieved an insertion torque of at least 35 Ncm were immediately non-occlusally loaded. The screw-retained provisional crown was delivered within 7 hours following the surgery and non functionally loaded. The definitive prosthesis was delivered within 2 months after implant insertion. Two months later the osteointegration progress was evaluated in all installed implants using: Dental X-rays and Osstellu2122 system.Results : After two months period of healing, high level of integration was observed. The stability of these implants was measured using Osstellu2122 system. These implants showed higher values ; ISQ 75 (hybrid) and 76 (rough surface), respectively. It also observed a good soft tissue adaptation clinically. No complications were observed. Radiographic bone changes were similar in both dental implants.Conclusions and Clinical implications :Both implants is a valuable treatment option for immediate implant placement and loading of single crowns in the maxillary esthetic zone.

So, dental implants have several interfaces and there is a challenge between bacterial colonization and cell functional integration of the dental implant surfaces. Various surface modifying methods such as blasting,etching, anodizing or coating with biomaterials are introduced. Surface modifications of dental implants is one of the most widely researched fields of implant dentistry. These surface modifications improve the adsorbtion of protein, cells and so result in faster osseointegration. However, no reports have demonstrated the superiority of one surface treatment over the other. Aim/Hypothesis: The purpose of this systematic review was to evaluate the clinical success of different implant surface properties and acquire actual results.Materials and Methods: An electronic search was conducted in MEDLINE (PubMed search form). The study is based on the Cochrane Review Methods. The main question is u2018Which implant surface modification technique is best in terms of clinical outcomes?u2019 In last decades (from January 2008 to December 2018), published clinical studies on implants with different surface properties were independently evaluated two reviewers (K.D. and M.H.A.) based on the inclusion criteria. In vitro studies, animal studies, retrospective studies, non-comparative studies and case reports were excluded. (1) Comparative clinical studies of different implant surfaces reporting one year or longer follow-up period, (2) more than ten patients including studies and (3) articles to consider mean marginal bone loss result were included.Results: A total of 2604 articles were evaluated from database. 110 articles is repeated, 2357 articles are animal studies and cell studies, 137 articles not relevant outcomes, 21 clinical studies were evaluated. 3 studies excluded because follow-up smaller than 1 year. Remaining 18 studies met the inclusion criteria and included to the review (Table 1). The modification methods were classified as physically, chemically or biochemically treated. Generally implant failure was related with smoking habit. Because of the low number of studies assesing each surface modification for failure, annual failure rates were not measured by surface modification technique. u0130mplant success were favorable for all modification techniques, but chemically and biochemically modification procedures can improve osseointegration at initial stage.Conclusions and Clinical u0130mplications: Within the limitation of this systematic review, it can be concluded that biochemical and chemical techniques can accelerate osseointegration process and may be prefered for early loading planning. However, the studies have different methods and it is difficult to figure out scientifically that superiority of one surface modification technique over another. Further comparative and long term clinical studies are required to identify exact results about the surface modification techniques.

An increasing number of clinical publications report on immediate implant restoration in the extraction. The advantages of immediate implantation after tooth extraction are shorter treatment time, fewer surgical procedures, and ensuring optimal tooth tissue aesthetics. Frequently, however, compromised teeth that are indicated for surgery are involved with acute and chronic infectious conditions which traditionally contraindicate their immediate restoration with dental implants. A review of current literature suggests that immediate implantation in infected sites should be avoided. Recent experimental studies have corroborated this clinical experience and shown that professional alveolar socket debridement and broad-spectrum antibiotics generate adequate conditions for the bone remodelling process around immediate dental implants placed into infected sites.Background: The immediate placement of dental implants into fresh non-infected sockets is an accepted successful technique, while placement of these implants into pathological Dento-alveolar sockets is still a controversial issue.Aim/Hypothesis: The aim of this clinical case-serious study was to report, retrospectively, the survival rate of immediate dental implants placed immediately after extraction of teeth with infected Dento-alveolar sockets in the maxilla and mandible at the osseointegration period (3 months).Materials and Methods: A retrospective analysis was made of the clinical and radiographic findings corresponding to 15 consecutive patients (7 women and 8 men, between the ages 21and 59 years) with a mean age of 44.14 years (SD= 9.28). A total of 36 locking-taper Bicon dental implants (hydroxyapatite, HA-coated) were immediately placed into infected fresh extraction- sockets included radicular cysts, chronic peri-apical lesions and sub-acute periodontal and endodontic infections using the flap or flapless surgery. Retrospective data on implant position, implant size, pathological condition, and baseline radiographs were obtained from patient records. Clinical and radiographic analyses were obtained to evaluate the treatment-outcome. Data were analyzed using descriptive statistics.Results: All the implants (N=36) except 2 achieved osseointegration in 3 months follow up. This reflects a 94.44% success rate within this small study group and 5.55% failure rate. Complications were related to the reasons for tooth loss.Conclusions: This retrospective clinical case-serious study showed the possibility thatimmediate dental implant placement might be successful in infected extraction sockets.

All rights not granted by the above license are retained by the author. Abstract: Radiotherapy is a common treatment for head and neck cancers. However, it compromises bone healing. Titanium implanthas been shown to be a predictable method for replacing missing teeth. Clinical studies revealed that implant failure rate in irradiated regionwas high. Many studies showed that modifications of implant surface could enhance implant osseointegration by improving cell attachment, cell growth and bone formation. Nevertheless, there were few studies investigating the effect of implant surface modification on osseointegration in irradiated bone. In the first experiment, the effect of fluoride-modified (FM) titanium surface on irradiated osteoblast attachment was assessed. The morphology and chemical composition of FM surface was assessed by SEM, AFM and XPS. Osteoblasts received 0Gy, 2Gy, 4Gy, 6Gy, 8Gy, 10Gy radiation. Cell number, fluorescence intensity and cell area of irradiated osteoblasts were assessed. The number of osteoblasts onFM surface was fewer than those on NF surface after 0Gy, 2Gy, 8Gy and 10Gy radiation. Cell area of osteoblasts on FM surface was less at 2Gy radiation but larger at 6Gy radiation than on NF surface. The fluorescence intensity of osteoblasts was also higher on NF surface than on FM surface after receiving 0Gy, 2Gy, 4Gy, 10Gy radiation. In the second experiment, an animal model was established to study the effect of radiation on osseointegration. Rabbits were divided into 15Gy and 30Gy radiation groups. Only the left leg was exposed to radiation, and the right leg was protected from radiation. Totally, 24 implants were inserted. Implant stability quotient (ISQ), bone volume to total volume (BV/TV), bone-to-implant contact (BIC), and bone growth rate were measured. After 15Gy and 30Gy of radiation, ISQ and BV/TV were significantly reduced. At week 3, 15Gy radiation group displayed slower bone growth rate comparing with the control side. Fluorochrome results showed that the 30Gy radiation side had a significantly slower apposition of new bone.In addition, BIC on30Gy radiation side was notablypoorer than that on 15Gy radiation side and on 30Gy control side. Based on the animal model, the third experiment investigated effects of calcium phosphate nanocrystals on implant osseointegration in irradiated bone. Titanium implants treated with nano-scale calcium phosphate (CaP) crystals served as the test group while ones with dual acid-etching only served as the control group. The left leg of rabbits received 15Gy radiation and implants were placed in the irradiated leg. Significant higher ISQ was detected in the nano-CaP group at week 12. The bone growth rate in nano-CaP group was more than doubled than the control group at both week 6 and week 9. The fourth experiment evaluated artifacts on micro-CT images caused by titanium dental implant. Implants were assigned into four groups: (1) implant only; (2) implant with covering screw; (3) implant with resin embedding; and (4) implant with covering screw and resin embedding. Each implant was scanned by micro-CT at 3 angulations. Implant angulation was the most determining factor followed by resin embedding. Data were gathered from treatment involving the placement of single implants in the esthetic zone. Initial bone heights vs. progressive bone heights over a five-year period were evaluated. Soft-tissue heights surrounding the implants were also evaluated. Analysis of the data should assist the dentist in the treatment and early detection of complications, timing patterns after treatment and accurate expectations, which can be relayed to the patient prior to treatment. Methods: This retrospective analysis of a subset of implants from two large ongoing clinical trials involved 27 single-tooth Rough-to-the-top (RTTT) implants (Omniloc, Zimmer Dental, Carlsbad, CA) and 34 single-tooth Smooth-collar (SC) implants (Biomet 3i, West Palm Beach, FL) in a total of 41 patients. Patients included in this study were those who had implants restored in the maxillary esthetic zone and whose radiographs at both time intervals were acceptable for analysis. Implants were placed and restored using a standard protocol. Standardized pen-apical radiographs were taken at baseline and at a five-year follow-up period using a customized jig. Bone levels were measured from the standardized radiographs at baseline and at the five-year time period using digital and standard methods. The radiographs were scanned at 4800 x 9600 dpi using a digital scanner (HP LaserJet 4800 series scanner, Palo Alto, CA) and then imported to a laptop computer (Dell Inspiron E1705) for further analysis. The images were then analyzed using the ImageJ 1 .34S software program (National Institute of Health). The digital method converted the radiographs into three-dimensional models based on radiodensity so that bone levels were more easily discernible and measured. Clinical attachment level (CAL) and implant mobility measurements were collected at baseline, six-months, one-year, three-years, and five-year time periods. Bone levels were calculated to the nearest 0.01 mm and CAL levels were recorded to the nearest 0.5 mm. Bone level changes, CAL and mobility data were analyzed using a repeated- measures ANOVA test. Results: The bone levels were calculated from the implant-abutment (IA) interface for both groups at the five-year period. The bone position from the crestal bone level to the IA interface for the rough-implant group was -0.61 ±0.08 mm. The bone position from the crestal bone level to the IA interface for the smooth-implant group was -1.55 ±0.10 mm (-1.96 ±0.16 mm for the stepped SC group and -0.85 ±0.19mm for the straight SC group). The total mean bone level change over the five-year period for the rough-implant group was -0.19 ±0.09 mm. The total mean bone level change over the five-year period for the smooth-implant group was -0.36 ±0.06 mm. The total mean bone level change for the mesial surface of both groups was -0.33 ±0.07 mm and -0.22 ±0.03 mm on the distal surface. According to the original osseointegration concept, an implant surgery under sterile conditions was advocated. However, all surfaces in the oral cavity are moisturised by saliva, which itself contain approximately 110 bacteria per millilitre. Consequently, there is a risk that implant and bone are contaminated during incision, osteotomy, implant insertion and bone augmentation procedures.Aim/Hypothesis The aim of this in vivo study is to investigate whether osseointegration is affected by saliva contamination during dental implant placement in an augmented site. It was hypothesized that saliva contamination during implant insertion in augmented area has a negative effect on osseointegration.Materials and Methods (984/1000 characters max)Six sheep were used in the present study. In the calvaria bone of each sheep, six bone defects, 8 mm in diameter of hemispheric shape were created and filled with three different bone graft materials. Autogenous bone, bovine bone mixed with autogenous bone (50:50), and biphasic ceramic bone substitute were used respectively. After 5 weeks of healing, 36 dental implants (Nobel Biocare Mark III, machined surface, 3.75 X 7mm), including 18 contaminated implants with fresh human saliva (Group SC) and 18 implants with no contamination (Group NC) were randomized installed in the centre of the augmented areas. After a healing period of 5 weeks, bone blocks containing implants were retrieved, and undecalcified ground sections were fabricated. For histomorphometric analysis, bone to implant contact (BIC), bone area fraction occupancy (BAFO), bone and material area (BMA) and bone area (BA) were measured. The statistical analyses were performed at a statistical difference of 0.05.Results All groups showed no inflammation signs around the implants and osseointegration in residual bone area. The overall test revealed a significantly lower amount of BIC in Group SC compared to Group NC (P=0.036) in the augmented area, however no significant difference in the area of the pre-existing bone (P=0.429). For BAFO, BMA and BA, there were no significant differences between Group SC and Group NC.Conclusion and Clinical implication Within the limitations of the present in vivo model, it was shown that saliva contamination during dental implant placement in augmented area, significantly affected the bone formation on the implant surface (BIC), however presented less effect on bone formation in areas more distant from the implant surface. The results indicate that it might be crucial to pay attention to saliva contamination during implant placement in an augmented area.
