Submitted by Kwok Hei MOK for the degree of Doctor of Philosophy at The University of Hong Kong in May 2005 Glaucoma is a complex multifactor disorder known to cause progressive loss of retinal ganglion cells and their axons, leading to a reduction in the thickness of the Retinal Nerve Fiber Layer (RNFL). There remains considerable disagreement within the glaucoma community as to the possible morphologic damage pattern differences in the primary chronic open angle glaucoma patients with elevated intra-ocular pressure (HTG) and those with normal intra-ocular pressure (NTG). It would be intriguing to determine if there is any morphological difference in RNFL between HTG and NTG patients since this may provide clues as to possible different factors responsible for the damage to their eyes. iiOptical coherence tomography (OCT, Humphrey systems Inc., Dublin, CA, USA) is a noninvasive technology that allows high resolution (approximately 10μm) in vivo cross-sectional imaging of the retinal structures based on the principle of Michelson interferometry. It allows a direct and objective estimation of RNFL thickness. One major benefit of RNFL assessment using OCT is that twelve clock hour thickness values along a circular path around the optic disc are displayed. This regional data analysis makes it more sensitive to detect localized changes. However, reproducibility of regional data was reported to be unsatisfactory. We found that increasing the number of scans to five scans per examination significantly improves reproducibility of regional values in both healthy and glaucomatous eyes. The peri-papillary RNFL under investigation exhibited double-hump patterns. RNFL was thickest in the supero- and infero-temporal regions (double humps) and thinnest in the temporal and nasal regions (double troughs). We observed that the RNFL in 129 eyes of 129 Hong Kong-Chinese subjects appeared to significantly decrease both over time cross-sectionally (3.3μm per decade, pTo best of my knowledge, there has been no study specifically investigating the profile of RNFL injury at different stages of glaucoma. Specific glaucomatous RNFL profiles injury may not only serve as a useful basis for diagnosing and monitoring disease progression, but also provide insights into the mechanism and pattern of ganglion cell atrophy in glaucomatous optic neuropathy. Glaucomatous RNFL loss occurs with regional preferences at different stages of the disease. We first reported that only infero-and supero-temporal regional values were found to decrease continuously, and that a localized defect was observed first in the infero-temporal region in pre-perimetric and early stages. This finding of preferential damage in the inferior as opposed to superior fibers in the very early stage of glaucoma damage is intriguing, and may shed light into the mechanism of the damage.. Retinal nerve fiber layer thickness (RNFLT), phase retardation, and birefringence measured by Polarization Sensitive Optical Coherence Tomography (PS-OCT) have been used for glaucoma diagnosis. We first investigated two different image registration algorithms, a mutual information (MI) based algorithm and a log-polar transform cross-correlation (LPCC) based algorithm, on both human and non-human primate models. We evaluated the effects of image registration on longitudinal analysis of RNFLT in non-human primates using PS-OCT. Then, we investigated thickness, phase retardation, birefringence, and reflectance of the retinal nerve fiber layer as measured by PS-OCT in normal and glaucomatous non-human primates in a longitudinal study. We defined a new Reflectance Index (RI) and demonstrated that it might be an earlier indicator of glaucoma onset than RNFLT, phase retardation, or birefringence. Finally, we validated this finding on cross-sectional clinical study on human eyes measured by PS-OCT and RTVue OCT. For the data measured by PS-OCT, we showed that for distinguishing between glaucomatous and healthy eyes, as well as for distinguishing between glaucoma suspect and healthy eyes, our new normalized RNFL reflectance index (NRRI) performs significantly better than phase retardation and birefringence. The performances of NRRI and RNFL thickness in both conditions were statistically indistinguishable in this study, which is likely due to the limited sample size. For the data measured by RTVue OCT, the performances of NRRI and RNFL thickness were statistically indistinguishable for distinguishing between glaucomatous and healthy eyes. NRRI performs significantly better than RNFL thickness for distinguishing between glaucoma suspect and healthy eyes. NRRI also performs significantly better than temporal, superior, nasal, inferior and temporal (TSNIT) average and nerve fiber indicator (NFI) from GDx VCC for distinguishing between glaucoma suspect and healthy eyes. NRRI is a promising parameter for distinguishing glaucoma suspect and healthy eyes and may indicate disease in the pre-perimetric stage.

Book excerpt: Glaucoma is a group of progressive optic neuropathies that can lead to irreversible vision loss if left untreated. Spectral (SD-OCT) is an important imaging tool used in the diagnosis and management of glaucoma. It provides thickness measurements of the retinal nerve fiber layer (RNFL), a useful index for monitoring progressive thinning that occurs from damage. However, there exists high between-subject variability in RNFL thickness in people free of eye disease. Another approach is RNFL reflectance, proposed in 1921 when reduction in reflectance was observed in damaged RNFL. Despite numerous reports from the past century supporting this, clinically applied quantification of RNFL reflectance using OCT is possible but lacking.First, I assessed healthy and glaucomatous eyes for the presence of presumed activated astrocytes and Muller cells (ARAM), a superficial retinal gliosis that could complicate reflectance-based measurements of the RNFL and reportedly only occurs in glaucomatous eyes with higher frequency in females. Recent work by our lab found ARAM in healthy eyes, and I confirmed this finding in a large sample. I found that ARAM occurred at similar frequencies in control and patient groups and was correlated to age-related posterior vitreous detachment but not to glaucomatous disease or to sex.Second, I assessed the feasibility of using attenuation coefficient (AC), a relative measurement of RNFL reflectance, to differentiate control from glaucomatous eyes. This method was successful in identifying most of the patients when assessing depths between 24 and 60 microns below the internal limiting membrane, which avoided superficial ARAM and captured deeper RNFL defects.Third, I investigated the normal variability of AC in younger and older healthy eyes and found a small age effect. Effects of axial length or sex were insignificant. Methods to reduce within-subject and between-subject variability are needed before normative reference ranges can be developed.Lastly, the group results on AC mentioned above used a method of manual data extraction, and these results were confirmed with an independent method for automated data extraction.
