Book excerpt: Excerpt from Cerebro-Spinal Fever This book has for its aim an attempt to bring together and correlate the clinical and pathological facts which we were enabled to accumulate during the epidemic of 1915. In January of that year, some of the first cases to occur in the Eastern Command were brought to the 1st Eastern General Hospital for treatment, and came under our care. The earlier cases were admitted in the first instance to different wards and were therefore under the charge of various physicians. Our thanks are due to Major Wright and Captains Curl and Haynes for allowing us every facility for studying the cases which had been admitted under their care. To Captain Curl we are especially indebted for much valuable counsel and help in dealing with these earlier cases. At the end of February the War Office appointed one of us bacteriologist to deal with the outbreak in the Western part of the Eastern Command. At the same time Colonel Griffiths arranged that a ward should be set apart for the treatment of all cases that arose. We were appointed to have charge of the cases admitted. A laboratory, which had been equipped by the Insurance Act Committee for purposes of research at the 1st Eastern General Hospital, was given for the investigation. In addition to providing the laboratory, the Committee assisted our investigations by appointing Mr H. W. C. Vines to study special problems as they arose. We wish to express our great indebtedness to the Insurance Act Committee for the equipment so generously given. To Mr Vines our thanks are specially due as much of his work has been incorporated in the present volume. Major Hele also rendered valuable assistance when the pressure of work was extremely great. In addition to laboratory work, it was the duty of the bacteriologist to visit the place of origin of every case, and investigate the hygienic conditions in which it arose. At this visit all contacts were examined to discover carriers. Ah proved carriers were at once brought into the special Cerebro-Spinal Fever ward, where they were kept under observation until two consecutive throat swabs had proved negative. Every case was therefore fully investigated by us from its commencement to the termination of the illness. Since then a variety of techniques for obtaining cerebrospinal fluid from various sites have developed, and the relationship of cerebrospinal fluid to a variety of disease processes has received considerable study. In this volume, we have attempted to assemble reviews of some of these important developments during the past century. Chapter 1 contains a historical review of key investigations that helped shape our concepts of cerebrospinal fluid from the earliest recorded description by the author of the Edwin Smith Surgical Papyrus in the seventeenth century B. C. to contemporaries such as Kabat, Glusman, and Knaub, who introduced quantitative determinations of cerebrospinal fluid gamma globulin levels. In Chapter 2 there is a description of our current understanding of the anatomical compartment for the cerebrospinal fluid and the physiology of its formation, circulation, and absorption. Dr. Richard Leech in Chapter 3 contributes a comprehensive description of the disorders associated with excessive quanti ties of cerebrospinal fluid-commonly referred to as hydrocephalus. A variety of techniques for properly collecting and evaluating cerebrospinal fluid are detailed in Chapter 4.

Book excerpt: Excerpt from Epidemic Cerebrospinal Meningitis My studies in the Research Laboratory of New York and during the Epidemic of Texas in 1912, afforded me a rare opportunity to apply the Laboratory studies of meningitis at the bedside of the patient and to demonstrate the importance of utilizing both laboratory and clinical observations for systematic control of an epidemic and for properly treating the disease. The studies in Dallas in the municipal control of the epidemic and the coincident studies in the Dallas Meningitis Hospital, enabled me to apply in connected, systematic order many of the excellent observations which bad been recorded by different workers. Many of the experiences in the Laboratory and at the bedside so obtained arc recorded in these pages. Particular attention has been given to a description of methods employed in scientific municipal control of epidemics, especially with regard to careful, scientific quarantine and the application of specific preventive measures. Many observations on the blood-pressure in meningitis are noted and the method of controlling the administration of the antimeningitis serum by changes in blood-pressure is carefully described. Toward the end of the Texas epidemic, experimental studies on prophylactic vaccination against meningitis were carried out. I hope that this study will stimulate further research which will help to establish this measure as a dependable preventive against the disease. The purpose of the work throughout is to convey a thorough yet simple description of the clinical and laboratory findings in the disease and to so interpret the laboratory descriptions as to familiarize the reader with their application in treatment and in clinical analysis of the disease. I take pleasure in expressing my thanks to Dr. William H. Park for his encouragement and assistance to me in my work in the Research Laboratory and to Dr. Simon Flexner for his interest and valuable suggestions during the Texas meningitis epidemic.
