Available in PDF, EPUB and Kindle. Book excerpt: Significant demographic changes are altering the structure of the American population. Larger numbers of immigrants are entering the work force, will become part of our aging population, and increasingly, are providing care for the elderly. Family structures and communities are evolving as marriage, childbearing, divorce, and cohabitation trends are changing. The working population that supports the elderly, physically and economically, is also changing and will most likely become smaller and less able to support this growing population. What does this mean for the well-being of our aging population and our efforts to ensure the quality of life for our elderly now and that we will want to enjoy ourselves as we become part of this older population? In this volume Drs. Schaie and Uhlenberg and a host of leading scholars look at the current structure of the American population in an effort to determine the impact it will have on the lives of the elderly and those growing older with disabilities and chronic illness. They examine the effects of the aging baby boomers on health care, migration and immigration and how it can support or tax health care networks, cultural issues regarding access to health care, and changing cultural attitudes towards marriage and family that are affecting the relationships between the elderly and their communities.

Book excerpt: The Medicare Prescription Drug, Improvement, & Modernization Act of 2003 (MMA) directed a study of the compensation of physicians in traditional fee-for-service (FFS) Medicare. This study explored linking physician compensation to efficiency -- defined as providing & ordering a level of services that is sufficient to meet a patient¿s health care needs but not excessive, given the patient¿s health status. This report: (1) estimates the prevalence in Medicare of physicians who are likely to practice inefficiently; (2) examines physician-focused strategies used by health care purchasers to encourage efficiency; & (3) examines the potential for CMS to profile physicians for efficiency & use the results. Includes recommendations. Charts & tables.

Available in PDF, EPUB and Kindle. Book excerpt: The federal budget impacts American policies both at home and abroad, and recent concern over the exploding budgetary deficit has experts calling our nation's policies "unsustainable" and "system-dooming." As the deficit continues to grow, will America be fully able to fund its priorities, such as an effective military and looking after its aging population? In this third edition of his classic book The Federal Budget, Allen Schick examines how surpluses projected during the final years of the Clinton presidency turned into oversized deficits under George W. Bush. In his detailed analysis of the politics and practices surrounding the federal budget, Schick addresses issues such as the collapse of the congressional budgetary process and the threat posed by the termination of discretionary spending caps. This edition updates and expands his assessment of the long-term budgetary outlook, and it concludes with a look at how the nation's deficit will affect America now and in the future. "A clear explanation of the federal budget... [Allen Schick] has captured the politics of federal budgeting from the original lofty goals to the stark realities of today."—Pete V. Domenici, U.S. Senate

Available in PDF, EPUB and Kindle. Book excerpt: Calling for a fundamental change in the focus of public policy in America, this book paints a vivid portrait of the nation's social health. Miringoff and Opdycke clearly show that social progress has stalled and the country's energies need to be directed at critical domestic issues in the years ahead. The authors propose a new agenda for monitoring America's social well-being built around sixteen key indicators of American life, such as infant mortality, teenage suicide, health insurance coverage, and affordable housing. They maintain that social conditions, like economic conditions, must be constantly monitored in order to have a clear sense of "how we are doing" as a society. The book builds on the work of the Institute for Innovation in Social Policy and argues that there needs to be a greater visibility for social issues - and a closer link between social reporting and public action - to better address the nation's social problems. It considers the critical role of the media in advancing public understanding of social issues, and examines important advances in the community indicators movement and international social reporting. Eye-opening and compelling, the book is a provocative centerpiece for policy debates and national initiatives on today's crucial domestic concerns.

Available in PDF, EPUB and Kindle. Book excerpt: Everyone agrees on the need to reform Medicare but not on how to do it. Some argue the program is too comprehensive, others that it is not comprehensive enough. Some suggest it pays too much for health care, others, too little. Meanwhile, the financial stakes continue to mount. Medicare spending exceeded $400 billion in 2007, making it more expensive than the entire health systems of most other nations, as well as the largest national public program other than Social Security and national defense. In R eforming Medicare, Henry J. Aaron and Jeanne M. Lambrew deftly guide readers through this complex debate. They identify and analyze the three leading approaches to reform. Updated social insurance would retain the current system while rationalizing coverage and reducing bureaucracy. Premium support would replace the current system with a capped, per-person payment that beneficiaries could use to buy health insurance. Consumer-directed Medicare would have beneficiaries pay for care up to a high deductible from government- supported savings accounts and offer premium-support coverage above the deductible. In addition to rating each option on its ability to promote access to health care, improve the quality of care, and control costs, the authors evaluate each reform's political strengths and weaknesses. Given the heat generated by the Medicare debate, it is unlikely that any single approach will be implemented in full. Consequently, Aaron and Lambrew describe incremental strategies that blend elements of each plan. Their analysis provides essential insight into the types of hybrid policies that Congress will consider in coming years.
