The purpose of the current research is to determine the PEAK Relational Training System's potential applicability in assessing language and cognition functioning in adults with brain injury. 20 adults with brain injuries served as the participants for this study. Using a Pearson Correlation, the relationship between performance on the PEAK assessment and the Ross Information Processing Assessment (RIPA-2), pre-morbid education levels, Glasgow Coma Scale scores, and the location of injury in the brain was analyzed. Significant relationships between PEAK and RIPA-2 scores and PEAK and number of years since injury were shown. Furthermore, implementation of the PEAK was shown to be highly reliable. The current research has shown the effectiveness of the PEAK Relational Training System Direct Training Assessment for adults with brain injury.

Book excerpt: This study examines the validity of using a combination of two psychometric measures, an emotional adjustment measure, and functional assessment measures to predict vocational outcome in a traumatically brain injured population. Patients included 33 males and 11 females, with an average age of 32.3 years, and a stable work history over the past three years prior to injury. All had sustained a traumatic brain injury in the 12 months prior to initial testing, with a mean of 3.8 months since injury. Levels of severity of injury included 24 patients with severe injury, 12 patients with moderate injury, and 8 patients with mild injury. Patients completed the Logical Memory subtest (LM) of the Wechsler Memory Scale-Revised, Paced Auditory Serial Addition Test (PASAT), Beck Depression Inventory (BDI), and Personal Capacities Questionnaire (PCQ). A clinician working closely with the patient also completed the Functional Assessment Inventory (FAI) and the Behavior Checklist (BC) at the time of initial testing. Follow-up testing on available patients (n=16) was completed approximately six months after initial testing. Comparison of the functional assessment measures demonstrated that patients exhibited a decreased awareness of functional limitations relative to clinician's ratings, but identified an increased number of personal strengths. The present study demonstrates the first comparison of FAI and PCQ ratings in a TBI population, as well as the first available field research using the PCQ. Results also indicated that the only significant predictor on earned income after six months was the overall functional limitations score on the PCQ. The only significant difference in patients' test performance at six months with scores at initial testing was seen on the PASAT, which suggested that patients had a significant improvement in their speed of information processing after six months. In addition, comparison of patients from Canadian and American rehabilitation agencies, respectively, revealed no significant differences between patients at either initial testing or at follow-up.

Available in PDF, EPUB and Kindle. Book excerpt: Adults with mild traumatic brain injury (mTBI) are at risk for developing persistent cognitive, physical, and psychological symptoms. Adults with mTBI often report difficulties meeting the demands of everyday communication. Studies which characterize mTBI-related communication functions are needed in this growing area of rehabilitation and research. Speed of information processing is expected to affect communication because of the speeded nature of conversation. In this dissertation, we report data from three studies designed to characterize language performance after mTBI. For two studies, we tested a speed-based hypothesis based on the evidence that speed of information processing is an area of cognitive impairment for many people after mTBI. We compared performance to an OI group to discriminate the effects of brain versus general trauma in the subacute stage of recovery from mTBI. Results indicated that on experimental naming and comprehension tasks, individuals with mTBI performed in a comparable way to OI controls. The effect of a condition of speed was statistically significant for the group as a whole, but differences could not be attributed to mTBI specifically. In addition, accuracy and reaction time were correlated with injury and symptom variables such as sleep quality and fatigue. Our third study aimed to characterize communication in adults with mTBI by measuring self-perception of communication problems and comparing these responses to a group of adults with moderate to severe TBI and a community based comparison (CC) group. The mTBI group identified fewer areas of deficit in communication competence than any of the study groups and there were only significant differences between groups at the opposite ends of the injury spectrum. The study findings suggest that perhaps a combination of a demanding linguistic tasks combined with strict temporal response windows can be a potential method for further study. These studies provide evidence that although injuries of this nature are considered "mild," the issues surrounding appropriate assessment, treatment, and recovery are complex and should be regarded in future research. The Wechsler Preschool and Primary Scale of Intelligence (WPPSI-IV) for Children intelligence assessment was assessed to the participant prior to baseline and after training, and programs adapted from the PEAK-Relational Training System were used for relational training procedures. A multiple-baseline design across skills (relational frames) design was used in the present study. Test relations (Y-Z) were probed during training after 3 training trial blocks and novel stimuli were used. The results indicate that the relational procedures were effective in increasing IQ on the block design subtest. The implications of the current study are discussed.

Book excerpt: Social cognition focuses on how people process, perceive, and apply information in social context in order to understand and infer their own behaviors and that of others. Impaired social cognition is a common, and often chronic, outcome of many neurological and psychiatric disorders, including traumatic brain injury (TBI). By lacking these social skills, there is a major disadvantage for social integration with others. In order to assess social cognition deficits in adult TBI populations, Part 1 and 3 of The Awareness of Social Inference Test (TASIT) were administered to examine social cognition impairments. A sample of 58 participants (28 TBI, 30 healthy control) were compared on their ability to accurately recognize emotions and interpret social situations. Mixed analyses of variance (MANOVA) revealed that those diagnosed with mild and severe traumatic brain injury may have difficulties with both basic aspects of social perception and more complex higher-order cognitive abilities when compared to healthy controls, showing less accuracy for both Part 1 and 3 of TASIT. These findings support previous literature that state how social perception deficits affect TBI populations, which has implications for both remediation of such impairments and for educating those who interact with people diagnosed with TBI.

They include difficulties in speaking, understanding, reading, writing, conversation skills and participation. These deficits result from underlying cognitive impairments in attention, memory, executive functions, linguistic skills and information processing. Not rarely, persistent communication difficulties may greatly affect the everyday life after a TBI. Language comprehension plays an essential role in functional communication. The aim of our current research is to find out what kind of cognitive-communication disorders patients with TBI have and how these disorders are connected with diffuse tensor imaging (DTI) measures of white matter tracts. All participants will be evaluated by a speech-language pathologist, psychologist, and neuroradiologist (DTI-imaging) during one month. We are currently collecting data of 50 cases with moderate u2013 severe diffuse TBI and 30 healthy controls. ObjectivesWe studied the differences between TBI and healthy participants groups in seven different language comprehension subtests with performance and performance time. We will present a subgroup where language comprehension data is already available. MethodLanguage comprehension data of 38 participants (12 male and 26 female) with TBI, aged 19-53 (M = 35) years was used. The control group consisted of 24 healthy participant matched by age, education and gender. All participants were administered a battery of subtests of language comprehension. The groups were compared with nonparametric tests (Mann-Whitney U-test) across the seven different language subtests. We analyzed both performance and performance time.Results & ConclusionLanguage comprehension performance was reduced in five of seven subtests for the TBI participants compared to the control group. Speed of performance was slower in two of seven subtests for TBI participants. Patients with TBI seem to have difficulties in multiple areas of language comprehension. To what extent non-linguistic cognitive deficits explain these findings and how they correlate with injuries in white matter tracts of the TBI subjects will be analyzed next.

The goal of our study was to determine how performance on measures of auditory comprehension, verbal reasoning, oral expression and reading comprehension obtained during acute care hospital admission in patients with complicated mTBI contribute to acute care discharge outcomes. Method: Speech-language pathologists administered the Boston Naming Test (short form), the conversational discourse subtest of the Montreal Protocol for the Evaluation of Communication, verbal fluency tasks, the verbal absurdity subtest of the Detroit Test of Learning Aptitude, the complex ideational material subtest of the Boston Diagnostic Aphasia Examination and the reading comprehension subtest of the Canadian Adult Achievement Test to 128 patients with a diagnosis of complicated mTBI admitted to the TBI program of the McGill University Health Center-Montreal General Hospital, a supra-regional Level I trauma hospital. Medical charts of participants were retrospectively reviewed to gather demographical data, language test results and accident-related information. Outcome measures obtained from the TBI program database included length of stay (LOS), discharge destination and scores on the extended Glasgow Outcome Scale (GOSE). Results: Over half of the patients with complicated mTBI (56.3%) required rehabilitation services post discharge from acute care. Of this group, 39.1% were transferred to an in-patient rehabilitation center whereas 17.2% were discharged home and obtained services in an out-patient facility.
