Book excerpt: The world continues to lose more than a million lives each year to the HIV epidemic, and nearly two million individuals were infected with HIV in 2017 alone. The new Sustainable Development Goals, adopted by countries of the United Nations in September 2015, include a commitment to end the AIDS epidemic by 2030. Considerable emphasis on prevention of new infections and treatment of those living with HIV will be needed to make this goal achievable. With nearly 37 million people now living with HIV, it is a communicable disease that behaves like a noncommunicable disease. Nutritional management is integral to comprehensive HIV care and treatment. Improved nutritional status and weight gain can increase recovery and strength of individuals living with HIV/AIDS, improve dietary diversity and caloric intake, and improve quality of life. This book highlights evidence-based research linking nutrition and HIV and identifies research gaps to inform the development of guidelines and policies for the United Nations’ Sustainable Development Goals. A comprehensive approach that includes nutritional interventions is likely to maximize the benefit of antiretroviral therapy in preventing HIV disease progression and other adverse outcomes in HIV-infected men and women. Modification of nutritional status has been shown to enhance the quality of life of those suffering HIV/AIDS, both physically in terms of improved body mass index and immunological markers, and psychologically, by improving symptoms of depression. While the primary focus for those infected should remain on antiretroviral treatment and increasing its availability and coverage, improvement of nutritional status plays a complementary role in the management of HIV infection.

Book excerpt: WHO and UNICEF jointly developed this global strategy to focus world attention on the impact that feeding practices have on the nutritional status, growth and development, health, and thus the very survival of infants and young children. The strategy is the result of a comprehensive two-year participatory process. It is based on the evidence of nutrition's significance in the early months and years of life, and of the crucial role that appropriate feeding practices play in achieving optimal health outcomes. The strategy is intended as a guide for action; it identifies interventions with a proven positive impact; it emphasizes providing mothers and families the support they need to carry out their crucial roles, and it explicitly defines the obligations and responsibilities in this regards of governments, international organizations, and other concerned parties.

Book excerpt: A comprehensive review of the uses and interpretation of anthropometric references undertaken by WHO in the early 1990s concluded that new growth curves were needed to replace the existing international reference. To develop new standards, a multi-country study was carried out to collect primary growth data and related information from 8440 healthy breastfed infants and young children from diverse ethnic backgrounds and cultural settings (Brazil, Ghana, India, Norway, Oman, and the USA). The first set of growth standards (length/height-for-age, weight-for-age, weight-for-length, weight-for-height and body mass index-for-age) was published in April 2006. This report presents the second set of growth standards (head circumference-for-age, arm circumference-for-age, triceps skinfold-for-age, and subscapular skinfold-for-age). The standards depict normal early childhood growth under optimal environmental conditions and can be used to assess children everywhere, regardless of ethnicity socioeconomic status and type of feeding. This report will be useful to pediatricians and other healthcare providers, nutritionists, policy makers, researchers, national institutes of health, schools of medicine, and professional associations. Available in PDF, EPUB and Kindle. Book excerpt: In the early 1980s it was discovered that HIV, the virus that causes AIDS, could be passed through a mother's milk to her baby. Almost overnight in the industrialised countries, and later in the African countries most ravaged by HIV, breastfeeding became an endangered practice. But in the rush to reduce transmission of HIV, everything we already knew about breastfeeding's life-saving effects was overlooked, with devastating consequences for mothers and babies. In HIV and Breastfeeding: the untold story, former IBCLC Pamela Morrison, an acknowledged authority on HIV and breastfeeding, reveals how women in the world's most poverty-stricken areas were persuaded to abandon breastfeeding as part of a short-sighted and deadly policy that led to an humanitarian disaster. The dilemma that breastfeeding, an act of nurturing which confers food, comfort and love, could be at once life-saving yet lethal, has been called 'the ultimate paradox'. This critical account reveals how vital breastfeeding is, even in the most difficult of circumstances, and examines the lessons that can be learned from the mistakes of the past - which is particularly relevant as we deal with the consequences for mothers and babies of another global pandemic, Covid-19. With detailed information for HIV-positive mothers and their caregivers, and success stories from mothers themselves, this book is essential reading for anyone involved in protecting and supporting breastfeeding, or with a need for evidence-based information about breastfeeding and HIV.

Available in PDF, EPUB and Kindle. Book excerpt: This manual provides expert practical guidelines for the management of severely malnourished children. Addressed to doctors and other senior health workers, the manual explains exactly what must be done to save lives, achieve successful management and rehabilitation, prevent relapse, and thus give these children the greatest chance of full recovery. Throughout, the importance of treating severe malnutrition as both a medical and a social disorder is repeatedly emphasized. As successful management does not require sophisticated facilities and equipment or highly qualified personnel, the manual also performs a persuasive function, encouraging health professionals to do all they can to save these children and meet their great need for care and affection. Recommended procedures draw on extensive practical experience as well as several recent therapeutic advances. These include improved solutions of oral rehydration salts for the treatment of dehydration, better understanding of the role of micronutrients in dietary management, and growing evidence that physical and psychological stimulation can help prevent long-term consequences of impaired growth and psychological development. Noting that the physiology of malnourished children is seriously abnormal, the manual gives particular attention to aspects of management - whether involving the interpretation of symptoms or the use of specific interventions - that differ considerably from standard procedures for well-nourished children. Details range from the reasons why IV infusion easily causes overhydration and heart failure, through a list of treatments that have no value and should never be used, to the simple reminder that underarm temperature is not a reliable guide to body temperature in a malnourished child during rewarming. Further practical guidance is provided in eight appendices, which use numerous tables, charts, sample recording forms, instructions for preparing feeds, and examples of easily constructed toys to help ensure that management is thorough, safe, and in line with the latest knowledge.

Available in PDF, EPUB and Kindle. Book excerpt: Thousands of HIV-positive women give birth every year. Further, because many pregnant women are not tested for HIV and therefore do not receive treatment, the number of children born with HIV is still unacceptably high. What can we do to eliminate this tragic and costly inheritance? In response to a congressional request, this book evaluates the extent to which state efforts have been effective in reducing the perinatal transmission of HIV. The committee recommends that testing HIV be a routine part of prenatal care, and that health care providers notify women that HIV testing is part of the usual array of prenatal tests and that they have an opportunity to refuse the HIV test. This approach could help both reduce the number of pediatric AIDS cases and improve treatment for mothers with AIDS. Reducing the Odds will be of special interest to federal, state, and local health policymakers, prenatal care providers, maternal and child health specialists, public health practitioners, and advocates for HIV/AIDS patients. Book excerpt: Breastfeeding is an intimate and deeply rooted bodily practice, as well as a highly controversial sociocultural process which invokes strong reactions from advocates and opponents. Touching on a wide range of issues such as reproduction, sexuality, power and resources, and maternal and infant health, the controversies and cultural complexities underlying breastfeeding are immense.Ethnographies of Breastfeeding features the latest research on the topic. Some of the leading scholars in the field explore variations in breastfeeding practices from around the world. Based on empirical work in areas such as Brazil, West Africa, Darfur, Ireland, Italy, France, the UK and the US, they examine the cross-cultural challenges facing mothers feeding their infants.Reframing the traditional nature/culture debate, the book moves beyond existing approaches to consider themes such as surrogacy, the risk of milk banks, mother-to-mother sharing networks facilitated by social media, and the increasing bio-medicalization of breast milk, which is leading its transformation from process to product. A highly important contribution to global debates on breast milk and breastfeeding.

Book excerpt: The spread of the AIDS virus has introduced a new element into the formula-versus-breastfeeding controversy. Mothers, particularly those in developing nations, have been urged to breastfeed in order to better nourish their infants and protect them from disease or contaminants in the water used to prepare formula. Now, however, mothers and healthcare workers must consider the danger of transmitting AIDS via breastfeeding. When HIV-infected women nurse their children, they significantly increase the risk of transmitting the virus. The issue is further complicated in countries where the alternatives are not very promising and in cultures that stigmatize women for even undergoing AIDS testing. This informative analysis includes the development of research into HIV and breastfeeding, the medical and political questions surrounding the controversy, and options and solutions for women to consider in feeding their infants. Fully indexed, this book is an important contribution to the social and medical studies of one of the most tragic facets of the HIV/AIDS epidemic.
