Les infections du site opératoire (ISO) sont fréquentes (prévalence : 4,5%), graves (létalité : 2,5% à 5%), coûteuses (environ 1 semaine d'hospitalisation supplémentaire, environ 3 000 dollars) et en partie évitables (Senic Project). La législation française, l'évolution jurisprudentielle des responsabilités, l'accréditation et les contraintes budgétaires font de la surveillance épidémiologique des ISO un enjeu de management des risques nosocomiaux. Dans un établissement de santé, l'organisation de la surveillance nécessite une bonne définition préalable des objectifs, des moyens nécessaires et des méthodes envisagées pour les atteindre. L'efficience de la méthode de surveillance choisie dépend en partie du suivi des patients après leur sortie de l'hôpital. La seconde partie de ce travail est une étude descriptive des IS0 diagnostiquées lors de ce suivi. Matériels et méthodes : Nous avons utilisé les données recueillies par INCISO, réseau de surveillance des ISO coordonné par le C-CLIN Paris Nord. Résultats : 15 085 interventions de chirurgie digestive ont été étudiées. La durée moyenne de séjour était de 9,5 jours. 759 ISO ont été diagnostiquées, 35,4% après la sortie de l'hôpital. 57% des ISO à NNIS 0 l'ont été après la sortie. La proportion d'ISO après sortie était inversement proportionnelle à la durée d'hospitalisation et le délai médian de diagnostic était de 12,5 jours. La plupart des facteurs de risque classiques d'ISO étaient associés après analyses unifactorielles, au risque d'ISO après sortie (NNIS index, classe de contamination, score ASA, durée d'intervention, expérience du chirurgien, urgence, chirurgie coelioscopique). Conclusions : Le taux d'incidence des ISO à NNIS 0 est un indicateur de qualité des soins chirurgicaux. Le suivi après sortie des patients opérés est le gage d'une surveillance épidémiologique de qualité procurant des taux d'incidence d'IS0 à NNIS 0 fiable.

Dans les pays à ressources limitées, des obstacles à son utilisation ont été mis en évidence. Nous avons estimé l’incidence des ISO dans un hôpital cambodgien en utilisant différentes définitions et modes de collecte de l’information. Chaque opéré à l’hôpital Preah Kossamak était invité à donner son consentement pour être suivi 30 jours. Les critères de survenue d’ISO étaient évalués tous les deux jours au lit du patient. À la sortie d’hospitalisation, une carte de suivi était remise et il était demandé au patient de la présenter à chaque consultation ambulatoire. Les patients étaient ensuite appelés par téléphone 15 et 30 jours après l’intervention. Entre avril et juin 2011, sur 167 patients inclus, les critères du CDC ont mis en évidence 24 ISO (14 ISO / 100 opérés), contre 20 ISO en considérant la survenue d’un écoulement purulent. Les données de suivi ambulatoire ont été collectées pour 121 des 139 patients sortis sans ISO. Parmi eux, 9 patients supplémentaires ont présenté du pus sur la cicatrice opératoire d’après un professionnel de santé (20 ISO / 100 opérés) et 16 patients supplémentaires ont déclaré un écoulement de liquide blanc (29 ISO / 100 opérés). Cette première étude de surveillance des ISO au Cambodge a présenté un taux d’incidence élevé. L’utilisation d’un écoulement purulent comme critère diagnostique et le suivi ambulatoire des patients par une carte et des appels téléphoniques se sont révélés faisables et utiles pour la surveillance active des ISO dans un pays à ressources limitées.
