Book excerpt: Cataract surgery is one of the most frequently performed procedure in the United States, and cataracts are a leading cause of visual impairment in the world. Glaucoma is also a very common eye disease with an expected 3.3 million Americans afflicted with primary open angle glaucoma by 2020. The coexistence of these two diseases is not uncommon and management of a cataract can have significant impact on the glaucoma status of a patient. Cataract surgery textbooks may have one chapter dedicated to the management of cataract with coexistent glaucoma. Likewise, a typical glaucoma surgical text dedicates one chapter to the management of these two diseases concurrently. This book will focus on the treatment of cataract in the setting of glaucoma, using an evidence-based medicine approach. Managing cataracts to the best advantage of the glaucoma should result in the best visual outcomes for patients. It should be a wonderful resource for general ophthalmologists and residents as well as glaucoma fellows.

Book excerpt: Glaucoma surgery was the topic of the eleventh World Glaucoma Association Consensus meeting. Glaucoma surgery in primary open-angle glaucoma was also the topic of the second Consensus meeting held in 2005. For the current version, angle closure and angle-closure glaucoma, as well as the secondary glaucomas, were also included. But the expansion of the subject matter is not the only reason for the timing of this new Consensus. Perhaps even more important is the explosion of new surgical ideas and techniques, which are revolutionizing the surgical approach to the glaucoma patient. The idea of targeting anatomic outflow and inflow pathways and individualizing care has opened so many new possibilities for improving surgical outcomes. Furthermore, outcomes are no longer only focused on lowering intraocular pressure, but also on enhancing health- and vision-related quality of life. As with prior meetings, it was a daunting task to seek and obtain consensus on a broad subject matter for which there are so many different opinions. As it is unclear how each of us decides how we practice and the evidence to guide us often is sparse, this consensus — as well as the others — is based not only on the published literature, but also on expert opinion. Although consensus does not replace and is not a surrogate for scientific investigation, it does provide considerable value, especially when the desired evidence is lacking. The goal of this consensus is to provide a foundation for glaucoma surgery and how it can be best done in clinical practice. Identification of those areas for which we have little evidence, and therefore the need for additional research, always is a high priority. We hope that this consensus report will serve as a benchmark of our understanding of what is known and what we would like to know. However, this Consensus Report is expected to be just a beginning that will be revised and improved with the emergence of new evidence, and new and improved surgical procedures.

A wide range of settings are considered, including cataract surgery in patients with uveitis, traumatic cataract, dislocation of an intraocular lens, intumescent cataract, brunescent cataract, combined cataract and corneal disease, combined cataract and glaucoma, pediatric cataract, and refractive cataract surgery. In addition, preoperative factors conducive to a favorable surgical result are identified and appropriate management of postoperative complications, described. As longevity increases, cataract is becoming an even greater public health issue worldwide. The use of cataract surgery, already one of the most frequently performed surgeries, is consequently increasing still further – a trend reinforced by developments in instrumentation and technology and improvements in access to surgery. However, comorbidities and challenges during surgery are very common. Challenges in Cataract Surgery will be a valuable reference for all residents, fellows, and practicing ophthalmologists who wish to improve their surgical techniques and outcomes.

Available in PDF, EPUB and Kindle. Book excerpt: For thousands of years, cataract surgery continues to be the "pearl" of ophthalmic surgery. It is explained by the fact that cataract removal always leads to an increase in visual acuity of patients. A new technology of cataract removal, phacoemulsification, was introduced by Charles Kelman in 1967. Despite this, it was difficult for the surgeons to transform from extracapsular cataract extraction to the new method - phacoemulsification. Recently, in the ophthalmological practice, the method of femtolaser cataract surgery (FLACS) has been introduced. This method allows you to automatically perform the basic stages of cataract surgery - corneal incisions, capsulorhexis, nucleus fracture. At the same time, to apply the various techniques of cataract surgery, the surgeon must possess virtually all the necessary techniques. This is dictated by the fact that, as a rule, cataract itself is rarely isolated - senile. Knowledge of the basic rules of cataract surgery, the use of pupillary rings and iris retractors in narrow pupils, the use of capsular rings in subluxations of the lens, the use of special modes of phaco in severe late cataracts, and a comprehensive clinical way of thinking can undoubtedly reduce the complications of cataract surgery.

Book excerpt: With a revolution in the surgical management of mild to moderate glaucoma, new procedures allow for the use of microinvasive techniques that avoid disruption of the conjunctiva and formation of a bleb. These approaches range from laser therapies targeting the outflow system of the eye to the implantation of miniature devices in Schlemm’s canal or the suprachoroidal space. The emergence of this new branch of glaucoma surgery, often coupled with micro-incisional cataract surgery, requires the practicing surgeon to learn about anatomical nuances to ensure success. MIGS: Advances in Glaucoma Surgery will aid the novice resident as well as the seasoned ophthalmic surgeon facing these new issues in glaucoma treatment. The book addresses preoperative patient selection advice, intraoperative pearls for success, and post operative management skills, which are some of the important aspects of the process towards the adoption of MIGS. Why you will want to read MIGS: Advances in Glaucoma Surgery: • Forward thinking contributions from seasoned MIGS experts who have intimate knowledge of the devices benefits and potential pitfalls in their use • Basic knowledge is provided that will allow each surgeon to tailor their treatment plan while leveling the learning curve • Assists in indentifying patients with a milder form of the disease and higher target pressures who may benefit from the MIGS devices MIGS: Advances in Glaucoma Surgery is the first book of its kind to cover the range of devices in this arena using expert contributors and offering real world pearls for practice that can be used in the operating room. Practicing cataract and glaucoma physicians, residents, and fellows will benefit from this timely resource as the thought process for how to treat glaucoma has advanced.
