The current studies based on molecular and cellular biology while ignoring pathophysiology and pharmacology may be leading the development of antitumor drugs in the wrong direction and wasting a lot of money. Although there have been numerous reports of genetic and phenotypic changes in tumors, a large body of pathological and clinical evidence supports the conclusion that there are no pivotal changes in tumor cells that distinguish them consistently and reliably from normal dividing cells. Unlike using antibiotics against bacterial infection, therefore, anticancer agents (ACAs) need to be delivered selectively to tumor tissues and should be kept there long enough to reproduce the concentrations they reach in the Petri dish, which is a closed space where the cytocidal effects of any anticancer agents (ACAs) including molecular targeting agents are very strong. In the body, however, administered ACAs are cleared with the passage of time. Furthermore, most human cancers possess abundant stroma that hinders the penetration of drugs into the tumor microenvironment. Therefore, to overcome these difficulties, novel drug delivery systems have been designed, such as nanoparticles and ACA conjugated antibodies to stromal components and to cancer cell surface antigens. These advances are described in this book after the first section, which describes core features of the pathophysiology of the cancer microenvironment, on which these new developments are based.

The nanoparticle acts as a water-soluble and bio-compatible platform that allows the delivery of hydrophobic drugs to the site of therapy. Due to the favorable surface area, hundreds of molecules can be attached to a 5 nm gold nanoparticle. More importantly, the versatile nanoparticle surface plays a vital role in targeted drug delivery. In this dissertation, we focus on efficient drug vectors for cancer therapy by synthesizing PEGylated gold nanoparticle-phthalocyanine conjugates. Both covalent and non-covalent drug binding can be achieved by designing the functionality of the nanoparticle surface. Due to the design of the gold nanoparticle carrier, using amphiphilic polymers, the drug delivery can be facilitated based on its size and the enhanced permeability and retention in the tumor tissue. Compared to the covalent approach, the non-covalent delivery approach of the hydrophobic photodynamic therapy drug Pc 4 through gold nanoparticles has provided rapid release and enhanced photocytotoxicity in cancer cells. In vivo studies in cancer bearing mice have shown an efficient deep penetration of the drug into the target tumors by the nanoparticle carriers within hours. In addition, controlled drug delivery system using visible-NIR light is achieved by a masked Pc 4-gold nanoparticle conjugate system. Targeted drug delivery can be achieved by modification of the carrier surface with active targeting ligands. After modification of the gold nanoparticle-Pc 4 conjugates with an EGF peptide, drug uptake into the tumor cells is dramatically improved due to the combination of active and passive targeting compared to untargeted conjugates. A 10-fold increase of the drug accumulation in a brain tumor was observed for the targeted drug delivery system. This shows that the designed gold nanoparticle-based drug delivery vector is able to cross the blood brain tumor barrier to address the brain tumor owning to the EGF targeted effect. The biodistribution of the drug and gold nanoparticles in vivo was investigated over a time frame of seven days. The biodistribution of the drug was studied by monitoring its intrinsic fluorescence in the organs and the gold nanoparticle concentrations in the corresponding organs have been quantified as well. Both gold nanoparticles and the drug can be removed from the body through renal clearance and the hepatobiliary system within days after drug administration.

Book excerpt: Although tremendous efforts have been made on targeted drug delivery systems, current therapy outcomes still suffer from low circulating time and limited targeting efficiency. The integration of cell-mediated drug delivery and theranostic nanomedicine can potentially improve cancer management in both therapeutic and diagnostic aspects. Herein, by taking advantage of innate immune cell's ability to target tumor cells, we developed a novel drug delivery system by using macrophages as both nanoparticle-carriers and navigators to achieve cancer-specific drug delivery. Firstly, we fabricated theranostic nanoparticles from a unique biodegradable photoluminescent poly (lactic acid) (BPLP-PLA), which inherently possesses strong tunable fluorescence, biodegradability, and cytocompatibility. In order to minimize the damage of cancer drugs to carrier immune cells and other healthy cells, a BRAF V600E mutant melanoma specific drug (PLX4032) was loaded into BPLP-PLA nanoparticles. Muramyl tripeptide (MTP) was also conjugated onto the nanoparticles in order to improve the nanoparticle loading efficiency. The resulting nanoparticles were internalized within macrophages, which were tracked via the intrinsic fluorescence of BPLP-PLA to demonstrate potential in imaging-based diagnosis and therapy. Nanoparticle-carrying macrophages were able to bind with and deliver drugs to melanoma cells under both static incubation and dynamic shear-flow conditions. Finally, pharmacological studies indicated that the delivered PLX4032 loaded nanoparticles did not show cytotoxicity against macrophages, while effectively killing melanoma cells. Our "smart" immune cell-mediated drug delivery system demonstrates the advancement of intrinsically traceable and targeted cancer nanomedicine.

It allows targeted administration to specific sites and sustained drug delivery. Therefore, this regimen greatly increases the therapeutic index and drug concentration in tumor tissues. Among all types, biomaterials-based nanocarriers, owing to their biocompatible nature, offers a potentially safer strategy to improve the survival of cancer patients. This thesis investigated the therapeutic activity of copolymeric nanoparticulate systems encapsulated with oleanolic acid (OA). Four types of biodegradable copolymers, namely, mPEG-P(D, L)LA, mPEG-P(L)LA, mPEG-P(D, L)LGA and mPEG-P(L)LGA, were employed to fabricate OA-loaded nanoparticles (OA-NPs) by nanoprecipitation. Physicochemical characterizations were performed using dynamic light scattering (DLS), transmission electron microscope(TEM)and high performance liquid chromatography (HPLC). Cytotoxicity of the formulations was evaluated on A549 (human alveolar carcinoma), HepG2 (human liver carcinoma) and BEAS-2B (normal bronchial epithelial) cells via 3-(4, 5-dimethylthiazolyl-2)-2, 5- diphenyltetrazolium bromide (MTT) assay. The induced cell death pathway was confirmed by fluorescence-activated cellsorting (FACS) flow cytometry. The synthesized NPs were spherical, with size dimensions of 40-70nm. The particle size increased to 230-260nm after the incorporation of OA. Polydispersity indices of less than 0.3 indicated the narrow size distribution of the negatively charged NPs. OA-NPs in suspension form were stable over 7 days of storage atboth in 4Cand 25C. Significant inhibitory effect was demonstrated by the OA-NPs on A549 and HepG2 cell lines through apoptosis (p mPEG-P(L)LGA >mPEG-P(D, L)LA > mPEG-P(L)LA NPs in decreasing order. The cytotoxic effect of OA-NPs on BEAS-2B cells was less than that on the two other cancer cell lines, while blank NPs (without OA incorporated) exhibited insignificant toxicity on all cell lines. This reflectedthe cytotoxic effect was ascribed to the encapsulated OA rather than the copolymeric NPs. Overall, our studies clearly revealed the specific anti-tumor capability of the developed OA-loaded nanoparticulate systems.
