Chez l'enfant cette douleur peut être le premier signe de pathologies d'origines diverses, digestives ou extra digestives, conduisant souvent à la prescription d'examens complémentaires. Le but de ce travail est de décrire les pratiques aux urgences pédiatriques de Bordeaux. Il s'agit d'une étude descriptive et rétrospective d'une série de 264 cas venus aux urgences pédiatriques de Bordeaux entre janvier et juillet 2003. La douleur abdominale aigue représente 5 % des consultations réparties de façon égale entre la journée et la garde. Elle touche autant les filles que les garçons. 75 % des enfants sont âgés de 6 et 15 ans et la majorité d'entre eux n'ont pas d'antécédent digestif. La majorité des enfants qui se présentent aux urgences n'ont pas vu de médecin préalablement et en général, une consultation simple est suffisante pour aboutir au diagnostic. Au cours de la consultation, 80,8 % des enfants ont eu un examen complémentaire biologique ou radiologique. Les diagnostics retenus sont d'origine médicale dans la moitié des cas, indéterminés dans 36,5 % et 8,9 % des enfants viennent pour urgences chirurgicales. Au vu de cette étude, l'échographie est l'examen complémentaire de pratique courante qui est le plus contributif au diagnostic. C'est également le seul examen courant pour lequel les indications sont systématiquement re-discutées. La prescription d'examens complémentaires la plus abusive concerne les radiographies d'abdomens sans préparation pour lesquelles seulement 18,5 % ont eu un intérêt dans la recherche du diagnostic. Le respect des recommandations permettrait probablement de diminuer ces prescriptions abusives et par là même le taux d'irradiation et le coût induit. Une des solutions pour limiter les prescriptions inutiles serait peut-être de rediscuter la demande des examens complémentaires en fonction des recommandations adaptées aux pratiques des différents services de chirurgie et de médecine de l'hôpital des enfants du CHU de Bordeaux et de joindre ces recommandations à la feuille d'observation des urgences.

La démarche diagnostique est guidée par l'âge de l'enfant, l'interrogatoire et l'examen clinique. L'échographie est actuellement un examen de choix en pédiatrie. L'objectif de cette étude est de décrire ses indications et sa place dans la prise en charge diagnostique des pathologies digestives, au POSU, hors contexte traumatique. Matériel et Méthodes : Ce travail est une étude rétrospective incluant des patients ayant bénéficié d'une échographie abdominale lors de leur passage au POSU du CHRU de Nancy, entre juillet et septembre 2014, et entre janvier et mars 2015. Nous avons relevé les diagnostics suspectés avant l'échographie, le diagnostic échographique et le diagnostic final retenu. Résultats : Nous avons inclus 221 patients (103 filles, 118 garçons), âgés de 3 mois à 18 ans. Le premier motif de consultation est la douleur abdominale aigüe. Le principal diagnostic suspecté est l'appendicite aigüe. 67% des diagnostics suspectés avant échographie sont des urgences chirurgicales. Dans 22% des cas, aucun diagnostic n'est clairement suspecté avant la réalisation de l'examen. Presque la moitié des échographies (47%) sont sans particularité. 23.8% des diagnostics suspectés sont confirmés. Conclusion : Devant une urgence abdominale, l'échographie est un élément clé du diagnostic final en confirmant ou infirmant une urgence chirurgicale. Les indications non étayées par la clinique n'aboutissent pas à un diagnostic échographique. Des moyens pour améliorer les indications de prescription de ces échographies sont possibles.

Nous avons réalisé une étude rétrospective du 1er janvier au 31 décembre 2000 au sein de l'unité d'accueil et d'urgences pédiatrique du CHU de Grenoble afin d'évaluer les modalités de prise en charge des douleurs abdominales aiguës chez l'enfant de plus de 2 ans . Cette étude a inclus 718 enfants avec un taux de recouvrement de 96.6% et une prévalence retrouvée à 14.2%. 46.7% des enfants arrivent aux urgences sans consultation médicale préalable avec un recrutement plutôt urbain. L'âge moyen est de 8 ans et 1 mois. La recherche d'une toux en cas de douleurs abdominales aiguës et d'une fièvre supérieure à 38ʻaugmente la valeur prédictive positive pour le diagnostic de pneumopathie de 5.1 à 75 %. L'association clinique la plus pertinente pour le diagnostic d'appendicite aiguë est " douleur aiguë décrite en fosse iliaque droite + Température>37.3ʻC + Faciès particulier + Défense en fosse iliaque droite " donnant un valeur prédictive positive à 99.5%.". En rajoutant les nausées, la valeur prédictive négative passe à 100%. Un bilan biologique est réalisé 1 fois sur 2 en présence d'une douleur abdominale. Il n'existe pas de lien entre la valeur de la CRP et le diagnostic d'appendicite aiguë. Une hyperleucocytose > à 12 G/l oriente vers une pathologie organique en particulier pour le diagnostic d'appendicite aiguë. On s'aperçoit en pratique que le diagnostic de constipation est porté dans 73.7% des cas sur les critères radiologiques de l'ASP. Près de 30% des enfants qui se présentent pour douleurs abdominales aiguës ont une échographie abdominale. La présence de signes appendiculaires à l'ASP a la même spécificité que ceux retrouvés à l'échographie pour le diagnostic d'appendicite aiguë. Il convient donc de redéfinir l'indication de l'échographie dans la prise en charge des douleurs abdominales aiguës de l'enfant.
