The goal was to gain insight from the staff nurses via a qualitative survey to provide data for nurse leaders to examine common perception themes associated with implementation of bedside shift report with a standardized safety assessment. Themes derived from data analysis indicated that nurses perceived bedside shift report to increase patient safety but felt that some nurses were not willingly participating in bedside shift report. Staff nurses indicated that nurse leaders should consistently remind them about utilizing key words and evaluate their performance of the bedside shift report with safety assessment during observations. Implications for nurse leaders based on findings from the research suggested the importance of consistency and clear expectations of what is expected.

The purpose of this study is to show bedside shift report increases patient safety as well as staff satisfaction. This study found bedside report lead to less falls and medication errors. Communication breakdowns are the cause of a significant number of sentinel events. Staff was satisfied with shorter report times and improved communication between shifts. Nurses were better prepared and were able to better assess patient needs after implementation of bedside shift report. Nurses were held more accountable and patients became more involved in their care. This new process gave the oncoming nurse the opportunity to visualize the patient and ask questions. Finding of this report confirmed how important having a standardized report system is in reducing errors. Accurate and consistent communication was improved after the implementation of bedside reporting. Effective communication at shift change is a fundamental component of nursing and the culture of safety. This study was conducted within a large hospital on medical and surgical units. A survey was obtained before the implementation of bedside report and several times over the first year post. Along with the use of surveys visual observations were done as well as in person interviews. Results of this study show that bedside report increased nurse's satisfaction, helped in the prioritization of patients which increased work flow, and decreased the amount of time for handoff.

Book excerpt: Background: The Joint Commission (TJC) evaluated sentinel events in healthcare and reported communication failure of care providers during patient handoffs as the most frequently cited root-cause. To improve patient outcomes and to avoid costly mistakes, it is recommended that the change of shift report should take place at the patient's bedside. Foreground: Studies regarding nursing shift reports indicate that information given is inconsistent among nurses. Despite attempts at creating standardized reporting sheets and implementing reporting methods to guide nurses, a lack of consistency remains, which can be a detriment to patient safety. Bedside reporting is recommended by TJC, but is not currently part of the nurse to nurse shift handoff report at a local, rural hospital. Framework: The Johns Hopkins Nursing Evidence-Based Practice Model was utilized as a guide to find the best evidence to implement and in translating the studies into practice. Method: Pre and post surveys were used to collect data on nursing staff perceptions on bedside reporting. After completing a pre survey, an education module on bedside reporting was shared with participants. Survey results obtained from nursing staff after participating in education were compared to the survey results of nursing perceptions before completing the education module. Descriptive data was obtained by comparing perceptions before and after education. Results: The findings show a positive change in nursing stadd perceptions on bedside reporting as evidenced by an improvement in pretest to posttest survey scores. Post education, there was an increase from 45% to 68% of the participants perceiving bedside reporting as more comprehensive than current reports held in a break-room. One-hundred percent of the participants perceive that bedside reporting was a beneficial form of communication, compared to eighty percent pre education. Conclusion: Educating the nursing staff on bedside reporting has led to an increase in buy-in. This can result in the success and sustainability of bedside reporting on the nursing unit, after full implementation. Tools, such as the ready-to-use Studer Group Bedside Reporting Tool Kit, can provide nurse managers with a valuable resource that may foster the support and sustainability of bedside reporting in their departments.

Recent research has shown an increase in patient safety, nurse satisfaction and patient satisfaction when nurse-to-nurse handoff occurs at bedside and includes the patient. Bedside report has been shown to increase the perception of safety in the patient, identify potential medication errors before they occur, and influence a sense of satisfaction among nurses. The nurses that practice this style of reporting are facilitating a connection between the patient and the nursing staff. A review of the literature seeking studies reflective of a change to bedside report has been conducted. With the use of Lewin09́s Change Theory an implementation, evaluation and dissemination plan has been established to support an Adult Medical Unit09́s change to bedside shift report. Each phase of the plan utilized evidence based best practice to support the change to bedside report. In implementing a standardized report process, this unit09́s goal was to improve patient safety, patient satisfaction, and nurse satisfaction. Surveys were conducted before and after implementation of the change along with real time audits to provide feedback to staff. Additional tools were provided to staff to further support their transition to bedside report. Through the literature review conducted, other near miss situations were noted that further supported the implementation of bedside shift report. These included medication errors, infiltrated intravenous sites, and incorrect laterality of scheduled surgeries. It is also noted that bedside report is supported by the Institute of Medicine and The Joint Commission who aim to support health care to be provided in the safest manner.

The goal of this report is to provide information in a standardized format that allows accurate data to be transferred to the oncoming nurse in a clear, concise manner. Literature reveals this report occurring at the patients is the best nursing practice; however, it does come with obstacles, competing priorities, and distractions. A breakdown in communication can cause disruptions to care, inaccurate information, or even harm and death to the patient in certain circumstances. Identifying a standardized approach to bedside report is a priority for patient safety and the hospital’s safety culture. The Nurse Knowledge Exchange Plus (NKE plus) created by Kaiser Permanente was designed with a multifaceted team and front-line nursing staff to engage the team in helping break down the obstacles and distractions. NKE plus was chosen as the standardized tool for beside reporting for this project because of the engagement of staff, ability to work through barriers and distractions, and provide a comprehensive tool that uses the electronic health record, safety checks, and inclusion of the patient and family (Lin, Heisler, Fahey, McGinnis, & Whiffen, 2015). This evidence-based quality improvement project looked at the nurse’s perceptions of bedside reports and Press Ganey’s communication with nurse's scores for improvements.The results of this Doctorate of Nursing Practice (DNP) project did not support the evidence that NKE plus improved nurses’ perception of bedside reports or improved communication with nurses’ scores. However, several areas did show statistical significance. Those areas included: (a) the nurse I follow from a previous shift completes job responsibilities; (b) interpersonal relationships between shifts are good; (c) the patient’s condition matches what I get in the report; and (d) the current system of report fosters a partnership between the nurse, patient, and families. The results were skewed due to a lack of surveys for both the nursing questionnaire and patient surveys. Further dissemination of this project includes completing NKE plus throughout the other three units within the hospital because of the evidence supporting including patient and family during bedside report.

Some of these errors are accounted for through miscommunication during the change of the nurse shift report. One of the Joint Commission’s 2019 national patient safety goals for hospital programs was to improve communication effectiveness among caregivers since mistakes in a caregiver’s communication accounted for 37% of severe patient injuries. Safety is threatened by patients not being included. Therefore, adopting the bedside shift report allows patients to be actively engaged, reinforcing nurse-patient communication. The bedside shift report enables nurses to provide safe, high-quality, patient-centered care at the bedside. At the time of this project, a 24- cardiac bed unit at a community hospital in Louisiana lacked a bedside shift report. Surveillance of the nurse shift report at the nurses’ station unveiled countless communication gaps. The Hospital Consumer Assessment of Healthcare Providers and Systems survey measures patient perception of their hospital experience. The scores from April 2019 revealed a need for improvement in nurse-patient communication at the unit level. The data were compared for the communication with nurse dimension of the Hospital Consumer Assessment of Healthcare Providers and Systems scores preimplementation of the bedside shift report from April 2020 (July 2018–June 2019 discharges), with survey scores postimplementation of the bedside shift report from April 2021 (July 2019–June 2020 discharges), using the longitudinal data analysis. The four specific questions in the Hospital Consumer Assessment of Healthcare Providers and Systems survey related to communication with the nurse and comparing data preimplementation and postimplementation of the bedside shift report were examined. This quality improvement research project aimed to assess the bedside shift report on patient satisfaction in care delivery for the communication with nurse dimension in the Hospital Consumer Assessment of Healthcare Providers and Systems survey. Ineffective communication during shift-to-shift reports can contribute to gaps in patient care and breaches in patient safety, including medication errors, falls, and sentinel events. The greatest risk of communication breakdown is during transitions in care. CONTEXT: St. Louise Regional Hospital, a small community hospital, consisting of an eight-bed medical-surgical intensive care unit lacked structure in how handoff should occur and had variances in shift handoffs. Observation of the shift handoff at the nurses' station revealed many communication gaps that have shown negative impacts on patient safety and outcomes. The improvement project described in this paper focused on evidence-based practices that support the benefits of bedside shift and implementations of standardized handoff tools. INTERVENTION: Intervention included an education in-service for unit managers, educational coordinators, and staff nurses to emphasize the important benefits of bedside shift report. MEASURES: The three components for evaluating improvement include outcome, process, and balancing measures to determine whether the improvement project has had the desired impact. The outcome measure expected to yield the following: direct observation of nurses during bedside report to calculate how often is being done, monitor for decrease in overtime due to more efficient shift-to-shift bedside reports, and a review of risk management reports observing for effect on the number and severity of medication errors. RESULTS: The outcome measure is to increase the nurses' compliance with bedside shift report in the intensive care unit to at least 80% within six months of implementation. CONCLUSION: With the proposed change, bedside shift report will addresses all the safety hazards by reducing adverse events, such as medical errors, patient falls at shift change, and sentinel events. Bedside shift report improves patient safety, enhances the quality of care, improves patient and nurse satisfaction, decrease unnecessary healthcare expenditure, and saves time.

Without accurate shift report nurses may miss valuable information leading to adverse effects. With the implementation of bedside shift report it has been shown to improve how patients perceive their quality of care. In one study patient and family members felt they were listened too, were taken into account, were better informed, concerns were dealt with immediately, and thus patients felt an increase in sense of safety (Wildner and Ferri, 2012). The implementation process for incorporating bedside shift report will be a conjoined effort. In order to ensure bedside shift report does indeed improve how patients perceive their quality of care an evaluation plan must occur. The method used in this study is a patient survey (Appendix A) and to view the improvement in patients' quality of care through hospital consumer assessment of healthcare providers and system (CAHPS) hospital survey scores (Appendix B). Once these methods are viewed at the three month mark and then again at the six month mark dissemination of results can take place with upper management and staff.

Author: Gilbert Young, RN, BSN Setting: Unit 1 North is a 32-bed cardiovascular specialty unit that specializes in the care of post-cardiac surgery and complex telemetry patients. The typical length of stay is between 5-7 days. Patients are admitted to the unit from the ICU, ED, or other medical facility for cardiac surgery. Rationale: The quality gaps are nurse communication HCAHPS scores not being consistent with performance goals, the unit had 16 documented patient falls in 2016, and patient surveys and staff handoff observations supported the need for this project. The project aim is to (1) improve nurse communication, (2) increase the nurses' consistency and satisfaction with handoff (3) implement the clinical report tool, and (4) reduce patient falls by 25% on cardiovascular specialty unit by August 2017. Return on Investment: Reducing four patient falls or 25% of last year's total will be an annual cost avoidance of $120,000 once this performance improvement plan is implemented. The initial annual savings is estimated at $110,300 Literature Review: The studies assert the importance of bedside report as a means to include the patient, eliminate information gaps, visualize the patient and surroundings for patient safety, and improve team communication. Additional studies examined the use of a standardized nursing handoff tool and saw improvement in patient satisfaction and a reduction in nursing errors. Methodology: Institute of Healthcare Improvement model for improvement and Lewin's change management model. Collaborated with unit council to develop clinical handoff tool. Educational plan includes PowerPoint presentation and simulation sessions. Project implemented in February 2017 and guided by PDSA (plan-do-study-act) cycle. Nurse leaders to observe use of clinical handoff tool during shift handoff. Results: Since implementation in February, average daily census of 32 patients and averaged 66 survey responses each month. Nurse communication scores have been consistently 4 Stars since implementation of this QI project. Current HCAHPS scores up to May 2017. There was a 12.5% reduction in patient falls compared to the same time period last year. Nurse leader observation of NKE using the handoff competency checklist demonstrated successful use of the handoff tool by all nurses, including remediation for some staff. Patient and staff surveys revealed positive improvements in seeking patient input and staff experiencing more satisfaction with the quality if information they receive at shift handoff.

The proposed change for this project is to implement bedside shift report on each nursing unit with a standardized approach to the process and format of the handoff report after an extensive literature review of evidence based practice (EBP) articles was performed. EBP proves bedside shift report reduces adverse events, increases patient and nurse satisfaction, and reduces nurse overtime by improving and standardizing communication while at the bedside. Following Lewin's change theory stages to unfreeze, move, and refreeze as a guide will be beneficial in making this proposed change into nursing practice. The continuity of care document (CCD) within the electronic medical record (EMR) system will be developed and utilized into a standard SBAR (situation, background, assessment, recommendations) format for the bedside report. Leadership and staff nurses are the key stakeholders involved in the process and their buy-in is vital for the success of this project. Implementation will occur on the pilot unit and slowly rolled out to other units every two weeks. Several methods will be used to evaluate the effectiveness of bedside report including nursing and patient surveys and Press Ganey results for long term monitoring to name a few. The results will be disseminated to nursing staff on a regular basis and to nurse leadership monthly.

Direct observation and surveys are the evaluation methods used, and a dissemination plan for findings will be discussed. Bedside shift report can be an effective strategy to increase safety for best patient outcomes.
