En adoptant une approche basée sur les facteurs de risque communs, le dépistage et le traitement de la parodontite peuvent avoir un impact favorable sur la prévention et l'évolution de certaines maladies systémiques. Principal coordinateur dans le parcours de soins des patients atteints de maladies chroniques, le médecin généraliste doit être conscient de l'interrelation entre maladies parodontales et maladies chroniques afin de fournir des informations et des soins appropriés aux patients. Notre enquête par questionnaire auprès d'un échantillon de médecins généralistes vise à apprécier leurs connaissances sur les maladies parodontales, les facteurs de risque de ces maladies et leur relation avec les maladies systémiques non transmissibles et à connaître leurs pratiques professionnelles dans la prise en charge des maladies parodontales chez les patients atteints de maladies chroniques. Il s'agit aussi de caractériser les obstacles et solutions pour l'intégration de la prise en charge des maladies parodontales dans la prise en charge des maladies chroniques. Les résultats de notre enquête révèlent une connaissance moyenne des médecins généralistes sur les maladies parodontales et leur association avec certaines maladies chroniques ainsi que des écarts dans leurs pratiques cliniques selon le secteur d'exercice. Toutefois, la quasi-totalité des médecins (>90%) sont favorables à faire évoluer leur pratique clinique dans la prise en charge des patients atteints de maladies chroniques. Le dépistage des maladies parodontales pourrait être intégré utilement au bilan médical. Pour cela, il parait nécessaire de réaliser à la fois des actions de formation initiale et continue auprès des médecins généralistes, des actions d'information auprès des patients et de prévoir des dispositifs facilitateurs à la collaboration entre médecins généralistes et chirurgiens-dentistes. Ainsi, la prise en charge des patients atteints de maladies chroniques pourrait être améliorée, permettant d'éviter une perte de chance en matière de rétablissement ou de maintien d'une bonne santé bucco-dentaire et générale.

Méthodes : Une revue systématique de la littérature a été conduite dans PUBMED et COCHRANE, visant à déterminer l'impact de l'ETP et de la maintenance parodontale sur l'évolution de la maladie. Puis, un questionnaire anonyme a permis d'évaluer si les patients étaient suffisamment informés sur les moyens de prévention et les recommandations. Résultats : Sur 4566 articles, 22 ont été retenus. Neuf évaluaient l'effet d'un programme d'ETP, qui semble améliorer les principaux paramètres cliniques (indice de plaque, indice de saignement et gingival). Treize études évaluaient la maintenance, et soulignaient l'importance d'une bonne observance, puisque la perte dentaire moyenne était significativement moins importante chez les patients observants que chez les patients non compliants (-1,15 IDC 95 % [-2,18 ; -0.11], p=0,006). Le questionnaire auprès de 278 patients a montré que 69 % ignoraient ce qu'est une parodontite, 62 % n'ont jamais bénéficié d'une séance dédiée à l'hygiène buccodentaire, et qu'environ 50 % ne pratiquaient pas un brossage adapté. Conclusion : L'ETP influence positivement l'évolution de parodontopathies, mais est insuffisamment appliquée en pratique, possiblement à cause des contraintes de gestion du cabinet. Le développement de la profession d'hygiéniste dentaire ou d'approches comme l'entretien motivationnel pourraient pallier ces limites.
