In many countries, the older population now faces the prospect of spending a quarter of their lives aged over 65, and a significant proportion will have to cope with cognitive decline associated with normal ageing or with dementia disorders. Given that these fundamental demographic changes will pose a significant challenge to health care systems, a detailed understanding of age-related cognitive and neurobiological changes is essential in helping elderly populations maintain cognitive performance. In addition, developing sensitive biomarkers to identify those at risk of developing dementia is crucial for early and effective interventions. To make inferences about the ageing process from the animal model back to the human, rigorous behavioral paradigms must be used to ensure that the same function is being examined across species. Given that similar navigational paradigms can easily be applied to humans and animals, recent years have seen an expansion of studies attempting to bridge the gap between age-related changes in animal and human spatial cognition. These studies begin to suggest that disruptions in spatial computations are among the earliest indicators of impending cognitive decline. In addition, although many animal studies have identified pathological mechanisms with paradigms involving spatial navigation, these mechanisms support many nonspatial cognitive functions as well. As a consequence, a successful characterization of how spatial processing changes in the ageing brain could reveal fundamental effects of cognitive ageing that could inform about general mechanisms underlying decline in perception, mnemonic processing and multisensory integration.

Book excerpt: Information and Communication Technologies (ICT) are no longer objects gathering dust on a shelf; instead, they have become intrinsic in our everyday lives. They are now even taking on an indispensable role in many clinical and rehabilitation settings. In the past decade there has been a surge of interest in using ICT with elderly people, both with and without dementia, in various clinical and research settings. On the one hand, ICT can supplement the assessment of functional ability by more precisely evaluating the nature and extent of functional impairment; on the other hand, ICT can be used to support elderly people in their everyday activities, as well as to ameliorate symptoms and improve quality of life through stimulation and rehabilitation. This is the intention driving the development of Serious Games (SG), which are digital applications (often based on Virtual Reality) specifically adapted for purposes other than entertaining, including rehabilitation, training and education. Finally, ICT can also play a key role in the development of interactive educational programs to support caregivers of people living with dementia. A handful of interesting studies have started to investigate the effectiveness of employing ICT in people with different types of dementia, such as Alzheimer’s disease (AD). It is therefore timely to attempt to scope this newly emerging field, as well as to foster a dialogue among the different professionals, including academics, clinicians and computer engineers, working in the area. With this in mind, the Research Topic “ICT for assessment and rehabilitation in Alzheimer’s disease and related disorders” aims to provide new and interesting insights into the current use of ICT in healthy and pathological aging. The intent is also to identify challenges and new perspectives in the field, gather recommendations for the application of ICT in AD and related disorders in clinical practice, and to showcase cutting edge clinical research. The articles included in this Frontier Research Topics have more than achieved this aim and are a perfect illustration of how ICT can be used to enhance the lives of people living dementia and their caregivers.

Book excerpt: Spatial cognition encompasses a wide variety of abilities and requires the interaction of several regions of the brain, including the hippocampus, striatum, and pre-frontal cortex (PFC). (Packard & McGaugh, 1996; Reuter-Lorenz et al., 2000). Given that these areas atrophy in later adulthood (Golomb et al., 1993; Raz et al., 2003; Aizenstein et al., 2006), it raises the question of how spatial cognition changes with age. It has been found that increased task complexity leads to an age-related decline in performance (Nagel et al., 2009). Other factors that lead to a decline in memory performance in older adults include whether the memory task involves allocentric or egocentric memory (Desrocher & Smith, 1998). This study developed a computational model of spatial working memory recall and recognition abilities for young adulthood to late adulthood using a type of neural network---dynamic field theory. This model was used to generate hypotheses of how spatial working memory recall and recognition abilities change from young adulthood to late adulthood. This model also influenced the development of hypotheses of how long-term memory performance changes with age, and how working memory abilities predict long-term memory abilities. Some of the hypotheses were supported, such as performance declined as task complexity increased, and, compared to younger adults, older adults had a greater level of error on the long-term memory task. Other models and hypotheses, such as the prediction that there would not be a significant difference in performance between the two age groups on a spatial working memory recall task, were not supported.

Book excerpt: Previous studies showed that both healthy and pathological aging are associated with changes in brain structure and function of the mature human brain. The most prominent anatomical alteration are changes in prefrontal cortex morphology, volume loss and reduced white-matter integrity and hippocampal atrophy. Cognitive decline affects mainly the performance of episodic memory, speed of sensory information processing, working memory, inhibitory function and long-term memory. It has been also proposed that due to the aforementioned changes the aging brain engages in compensatory brain mechanism such as a broader activation of cortical regions (mainly frontal) rather than specialized activation. Evidence suggests that similar changes occur with pathological aging but to a greater extent. In this case information flow is disrupted due to neurodegeneration, functional activation of posterior (occipito-temporal) regions is decreased and as a consequence the brain fails to process sensorial input in the ventral pathway and cognitive deficits appear. In the last years, functional alterations associated with aging have been studied using the mathematical notion of graph theory that offers an integrative approach since it examines different properties of the brain network: 1) Organization level 2) amount of local information processing, 3) information flow 4) cortical community structure and 5) identification of functional / anatomical hubs. So, graph theory offers an attractive way to model brain networks organization and to quantify their pathological deviations. Previous studies have already employed this mathematical notion and demonstrated that age-related neurodegeneration is often accompanied by loss of optimal network organization either due to diminished local information processing or due to progressive isolation of distant brain regions. They have also found that changes in network properties may be present even in the preclinical phase, which could be taken as a biological marker of disease.
