The clinical measurements, including PD, mSBI, mPI and PICF volume, all showed a significant improvement over time. However, between-group comparisons were only significant for PD at week-1 and -2 as well as mSBI at week-1, -4 and -6. All these significant between-group differences in PD and mSBI were lower in PS group. Regarding cytokine productions in PICF, significant time-dependent reductions were observed in OPG and IL-1ß on both PM and PS sides. Nevertheless, between-group comparisons showed no significant difference of any target cytokine productions at any time points. Likewise, the results of gene expression displayed no significant difference on any targeted genes between PS and PM sides at any time point examined. However, intra-group comparisons showed only PS side with significantly lower levels of IL-6 and CRP genes at week-6 as compared to week-0. Moreover, both PS and PM sides presented significantly higher levels of RANKL, periostin, and peroxidasin genes at week-6. Conclusion: Within the limits of this study, dental implants with PS design rendered significant benefits over PM implants in terms of PD and mSBI reduction during 6-week healing. However, these benefits could not be reflected in the cytokine and gene expressions because no significant differences were detected between PS and PM sides in this study. It seems that peri-implant soft tissue response around PM and PS implants is mostly similar during the early healing period, especially in respect of cytokine productions and gene expressions.

The concept of platform switching was proposed to reduce crestal bone loss around implants. Purpose: This study was attempted to study the influence of platform switching on the marginal bone level alterations around implantsMaterials and Methods: 60 implant sites were divided into two groups i.e a test group and a control group. The test group comprised of platform switched implants and the control group comprised of platform matched implants Radiologic assessment was done at varying time intervals after uncovering of the implant.Results: A Comparison of mean bone loss between platform matched and platform switched abutment as noted on the mesial side showed a non-significant difference (p value=0.92) while on the distal side statistically significant difference was noted. Comparison of the results between 2 groups at 1,2 4,6 months of post abutment placement on the distal side showed a statistically significant difference. (p value=0.024).Conclusion: It was noted that bone loss is more in platform matched abutments as compared to platform switched abutments. It can thus be concluded that platform switching results in peri-implant bone preservation and also has esthetic benefits.

Platform switched abutment elicits statistically significant change in mesial and distal papilla level and buccal peri-implant mucosa level at different time interval. Conclusion: This study suggests that platform switching can provide increase in mesial and distal papilla and buccal peri-implant mucosa level along with bone remodeling around implant.

The aim of this study was to systematically review the combined effect of platform switching with implant placement depth on marginal bone loss.An electronic search was conducted up to March 2017. One-piece implants and immediate post-extractive implants were excluded. The mean differences between groups were calculated using the inverse variance method with a random effects model.Twenty studies were included in the meta-analysis. More marginal bone loss was observed around platform-matched implants at 12 months after prosthetic loading (-0.38mm (-0.49; -0.27)). Among platform-switched implants, more marginal bone loss was observed around implants placed subcrestally (-0.67mm (-1.31; -0.03)).Further studies are needed to compare marginal bone loss around (1) platform-matched implants placed at different depths and (2) platform-switched implants placed at crestal versus supracrestal levels.

This randomized clinical trial aimed at evaluating the soft and hard tissues changes in the esthetic region around immediate implants placed in fresh extraction sockets that received u2013 during the first 4 months of healing - a customized (test) or a standard (control) healing abutment without restoration.Materials and Methods: Patients, who required a single tooth extraction and who received an immediate implant with an alloplastic graft (in situ hardening biphasic calcium phosphate), were randomly assigned to two groups: test group (n=18) that received a customized healing abutment or control group (n=18) that received a standard healing abutment. Clinical and intraoral radiographic examinations were taken immediately after implant placement (baseline) and at 4 and 12 months after implant placement, to assess the peri-implant tissues changes. Results:41 patients were preliminary enrolled, after tooth extractions 5 out of 41 extraction sockets sites were evaluated as not suitable for an immediate implant placement and were excluded from the study. In total 36 patients were randomized in the 2 experimental groups. There were 17 females and 19 males (ranging in age from 23.4 to 77.9), with a mean age of 58 years at the time of implant placement. In total, 28 (77.7%) patients reported to be non-smokers and 8 patients (32,3%) reported to be light smokers (less than 10 cigarettes/day). The two study groups were not balanced for confounding factors, even though at baseline no significant differences were observed between the two groups. No prosthetic or implant failure were registered during the study period with a 100% of survival rate. The test group presented better value of Pink Esthetic Score (PES) at distal papilla sites at 12 months when compared to the control group (p = 0.0002). The mesial and distal Papilla Index (PI) in the customized group were significantly higher than in the standard group at 3 and 12 months (p 0.0002). Regarding the bone tissue, the marginal bone loss at mesial sites was significantly higher in the control group than in the test group (p 0.0014).Conclusion: The use of customized healing abutment demonstrated the most favorable results regarding soft and hard tissue in case of immediate implant in addition to peri-implant bone grafting.

In addition to osseointegration process, healing of peri-implant soft tissues, as well as soft tissue stability are of a great importance, for the long-term implant success. Routinely, HA made of titanium (Ti) are used. Due to some material related limitations, the aim is to find novel materials that would have the same mechanical properties, but show less biofilm-adhesion affinity and lower local inflammatory soft tissue response. Polyetheretherketone (PEEK) is a thermoplastic polymer that has favorable mechanical characteristics, high biocompatibility and low submissiveness to microorganism adhesion. It has been introduced in implant dentistry as a material for various prosthetic components. The Aim: To compare the formation of dental plaque on PEEK and Ti HA and examine the local inflammatory response of peri-implant soft tissues.Materials and Methods: The study included 11 patients with at least two missing teeth in the posterior regions. In each patient, 2 implants were installed, with PEEK abutment (test group) placed on one implant, while on the other implant, Ti abutment (control group) was placed. 6 months after implant placement, both HA were replaced with new ones and a thin layer of peri-implant soft tissue was sent to histological analysis. For microscopic histological evaluation, biopsies were fixed in 10% neutral-buffered formalin. Three 5-u03bcm-thick paraffin-embedded horizontal sections were cut, dyed with a haematoxylinu2013eosin (HE) stain and analyzed by light microscopy and microscope Nikon Eclipse Ci with control unit DS-L3 and pre-installed software. Following morphological changes were analyzed: presence, localization and intensity of the inflammation. Presence in epithelial and subepithelial tissue was categorized qualitatively (yes/no) and intensity semi-qualitatively (light, medium, intensive).Results: All 11 patients successfully completed the study, without implant loss or implant related complications. Histological immunohistochemical analysis of samples of peri-implant soft tissues showed that all the samples had inflammation in subepithelial tissue. However, inflammatory cells in epithelium were present only in the test group. Semiquantitative analysis showed wary similar distribution of inflammation intensity. Inflammatory responses were classified as u201cintensiveu201d, u201cmediumu201d or u201clightu201d. Intensive inflammatory infiltration in subepithelial tissue in the test group was observed in 90.9% of the specimens, while in the control group it was classified as moderate or weak in 63.3% and 36.4%, respectively. Correlation between intensity, localization and inflammatory cell phenotype showed predominant T-cell proliferation in massive inflammation, with lymphoepithelial lesion in the test group.Conclusion: Both types of HA have been shown to be successful in single-stage implantation protocol. However, contrary to the expectations and preliminary findings from the literature, PEEK healing abutments showed a more pronounced inflammatory response of the surrounding soft tissues compared to Titanium abutments.
