Book excerpt: April 1996 What is the proper mix of social insurance and social assistance in the transition economies of Eastern Europe and the former Soviet Union? Social security systems in Eastern Europe and the former Soviet Union devote most of their resources to earnings-related pensions and neglect targeted interventions to aid losers from the transition to a market economy. As social insurance systems, they have the characteristic weaknesses of the insurance model, paying benefits on the basis of past contributions or work history rather than need. Yet contributions and benefits are not closely enough related to enable the system to operate as social insurance in a market economy. More attention must be paid to the potential impact of high payroll taxes on employment in a market economy. Some analysts have argued that contributors to the system (future pensioners) should perceive their contributions as savings, in addition to which unemployment benefits and a new social assistance system could be developed. Such a two-tier system would distinguish between social insurance and social assistance. Mabbett discusses an alternative prescription -- a unified system, in which the earnings-related system would be curtailed, making it as flat as possible, thereby establishing a universal categorical age benefit. Other benefits, including unemployment benefits and family allowances, could also form part of the system. In principle, benefits in a unified system could be means-tested, but means-testing is not integral to the system, whereas it is the defining feature of social assistance in the two-tier model. Some analysts have argued for a unified system in the short term (because of fiscal constraints), moving to a two-tier system later in the transition. Both types of system exist in capitalist economies. Indeed, systems in many countries are an untidy mix of insurance and assistance. Mabbett examines some of the issues that arise from the relationship between insurance and assistance in the transition and beyond. She derives three criteria for determining whether the two-tier model is viable, then applies the criteria to the small republic of Moldova. She concludes that Moldova probably does not fulfill the three criteria because it has experienced a severe economic shock and adjustment has been a long drawn-out process. The two-tier prescription may be more problematic for countries of the former Soviet Union than for the Eastern European states. This paper -- a product of the Country Operations Division 2, Europe and Central Asia, Country Department IV -- is part of a larger effort in the region to assist in reorienting social expenditure to help the poor.

Book excerpt: The U.S. and other western economies are experiencing dramatic changes in growth and age structure of their populations. Fluctuations in birth rates are the most important determinants of these changes in the post war period. This paper examines the dynamic effects of baby "booms" and baby"busts" on a range of economic variables using a perfect foresight life cycle simulation model. In addition to describing general transition (as opposed to simply long run) affects of fertility change, the paper considers alter-native Social Security policies for avoiding sharp increases in long run payroll tax rates. These include reductions in benefit replacement rates, advances in Social Security's retirement age, taxation of social security benefits, and the accumulation of a significant Social Security trust fund. According to the simulated demographic transitions, the savings inthe U.S. fertility currently underway can have very major impacts on long run factor returns and produce percipitous short term changes in saving rates. While Social Security policy has important effects on the simulated demographic transitions, these effects are of secondary importance to the long run level of economic welfare. Even if payroll tax rates rise dramatically, long run welfare (measured in terms of levels of adult consumption and leisure) is, nonetheless, substantially higher in the case of a sustained dropin the fertility rate. This reflects, in part, the decline in the number of dependent children per adult; while a sustained decline in the fertility rate eventually means a much larger ratio of elderly per capita, the decline in children per capita means an overall decline in the long run ratio of dependents to prime age workers in the economy. A second explanation for the simulated long run welfare gains is capital deepening associated with lower population growth rates

Book excerpt: There was a time when it was clear what risks social security policy was meant to protect: unemployment, sickness, and occupational disability. For many decades, although true equity remained elusive, a focus on these 'external' risks seemed to be enough. It is to the credit of more recent policy that such previously 'hidden' but all-important matters as excluded minorities, inadequate income, and obstacles to personal development are now on the agenda. Yet, with the globalization of the economy, new and unprecedented risks proliferate, most of them being of the 'manufactured' kind that stem from technological and economic factors that are largely independent of time and place and that do not lend themselves easily to regulation. Social Security in Transition surveys and analyses the forces affecting social security policy today as understood by twenty-one, mainly European, authorities in the field. Although each author focuses on specific issues (such as poverty, migration, retirement schemes, access to health care), a consensus emerges that social security can no longer be viewed as a distinct theoretical entity, but that it must be considered in a broad context of social policy that encompasses employment, education, and health care. That the state will remain the last resort for the resolution of dangerous social disparities seems inevitable; yet some transnational standards of fairness (including concerted action for the prevention of and combating the worst evils, such as poverty and social exclusion of migrants) are crucial if we are to develop meaningful state and collective arrangements that will not only support all individuals as they take responsibilities in life, enhance their opportunities, and make meaningful choices , but also safeguard the necessary level of social cohesion in society. Social Security in Transition builds on papers that were originally presented at a June 2001 symposium in The Hague to mark the centenary of the Dutch Occupational Accidents Act 1901 and at the same time of Dutch social security. The symposium was an initiative of the Social Security 2001 Foundation, which was set up by a group of Dutch ministries, administrations and supervisory agencies, and social partners.

Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.
